The Effect of Core Strengthening Exercises and Sciatic Nerve Glides on 65 Year-Old Female with Low Back Pain and Lower Extremity Radicular Symptoms: A Case Report
Background/purpose: Low back pain is the single leading cause of disability worldwide and is frequently accompanied by numbness, tingling, and burning sensation in the lower extremities. Oftentimes, LBP with lower extremity symptoms is treated with core stabilization exercises and nerve gliding activities, but limited research exists for the use of nerve glides for radicular lower extremity pain.
Case Description: The patient was a 67 year-old female with a chronic episode of LBP and left lower extremity sciatic pain. The patientâs symptoms included pain in the lower back and posterior thigh after sitting, standing, or ambulating for more than 10 minutes.
Outcomes: Patient completed 15 outpatient treatment sessions and showed a considerable decrease in LBP as evidenced by a clinically significant improvement in score on the Modified Oswestry survey. She also demonstrated an increase in bilateral multifidus strength as well as reports of decreased pain and reduction in lower extremity symptoms after the use of sciatic nerve glides.
Discussion: The subjectâs decreased pain and reduced frequency of lower extremity radicular symptoms are indicators of increased core stability and improved postural habits. Sciatic nerve glides helped reduce lower extremity radicular symptoms and pain. Future research can help to determine the preferred treatment methods for patients with LBP and lower extremity radicular symptoms
Phase Transitions and Symmetry Breaking in Genetic Algorithms with Crossover
In this paper, we consider the role of the crossover operator in genetic algorithms. Specifically, we study optimisation problems that exhibit many local optima and consider how crossover affects the rate at which the population breaks the symmetry of the problem. As an example of such a problem, we consider the subset sum problem. In so doing, we demonstrate a previously unobserved phenomenon, whereby the genetic algorithm with crossover exhibits a critical mutation rate, at which its performance sharply diverges from that of the genetic algorithm without crossover. At this critical mutation rate, the genetic algorithm with crossover exhibits a rapid increase in population diversity. We calculate the details of this phenomenon on a simple instance of the subset sum problem and show that it is a classic phase transition between ordered and disordered populations. Finally, we show that this critical mutation rate corresponds to the transition between the genetic algorithm accelerating or preventing symmetry breaking and that the critical mutation rate represents an optimum in terms of the balance of exploration and exploitation within the algorithm
