The efficacy, tolerance and adverse events during treatment with VMP in plasma cell myeloma patients
Przedstawiono opis dwojga chorych ze szpiczakiem plazmocytowym (PCM) leczonych w I linii za pomocą schematu VMP (bortezomib, melfalan, prednizon). Pierwszy opis przypadku dotyczy 76-letniej pacjentki zakwalifikowanej do leczenia według schematu VMP. Chora otrzymała łącznie 3 cykle leczenia, które były powikłane infekcjami, zakrzepicą żył kończyny dolnej prawej, epizodem drgawkowym oraz pancytopenią. Następnie pacjentkę poddano radioterapii zmian w kościach czaszki okolicy ciemieniowej (20 Gy/g). Ze względu na wiek chorej, liczne powikłania związane z leczeniem VMP i odstępstwa od programu terapeutycznego z użyciem bortezomibu pacjentkę zakwalifikowano do leczenia cyklofosfamidem z prednizonem. Drugi przypadek dotyczy 68-letniego pacjenta z ustalonym rozpoznaniem PCM oraz towarzyszącą niewydolnością nerek leczonego według schematu VMP. Po 5 cyklach leczenia chory ten uzyskał bardzo dobrą odpowiedź częściową. Oba przypadki ilustrują skuteczność, tolerancję oraz powikłania związane z leczeniem VMP w grupie chorych w starszym wieku oraz u osób z niewydolnością nerek.In this paper we describe two patients with plasma cell myeloma (PCM) treated with bortezomib-containing regimen VMP (bortezomib, melphalan, prednisone); the first case was 76-years-old woman who was qualified to VMP treatment. She obtained 3 cycles of chemotherapy, which were complicated by infections, deep venous thrombosis right lower limb, seizure and pancytopenia. Following she was qualified to radiation therapy because of changes in the parietal bone of skull (20 Gy/g). Due to patient’s age, many adverse events associated with VMP treatment and defections from therapeutic program with bortezomib patient was qualified to the treatment with cyclophosphamide and prednisone. The second case of 68-year-old patient with PCM and concomitant renal failure was treated with VMP. He achieved very good partial response after 5 cycles of chemotherapy. This report reveals therapeutic efficacy, tolerance and adverse events associated with VMP treatment in elderly patients and accompanying renal failure with plasma cell myeloma
Adult patients after coarctation of the aorta repair - exercise induced hypertension
Wstęp U większości pacjentów po skutecznej operacji
koarktacji aorty (CoAo, coarctation of aorta) obserwuje
się nadciśnienie tętnicze, a części z nich nadciśnienie
tętnicze indukowane wysiłkiem. Celem
pracy była ocena spiroergometryczna wydolności wysiłkowej
oraz echokardiograficzna ocena wielkości
masy lewej komory (LVM, left ventricular mass)
u dorosłych chorych po chirurgicznej korekcji CoAo
w zależności od obecności nadciśnienia tętniczego,
ze szczególnym uwzględnieniem osób z nadciśnieniem
tętniczym indukowanym wysiłkiem.
Materiał i metody Badano 74 pacjentów (29 kobiet)
w średnim wieku 31,2 ± 9,8 roku operowanych
w średnim wieku 10,4 ± 6,8 roku. Grupę kontrolną
stanowiło 30 osób (12 kobiet) w średnim wieku
32,2 ± 6,6 roku. Badaniem echokardiograficznym oceniono
LVM i wskaźnik masy lewej komory (LVMI,
LVM index). Wykonano maksymalny test wysiłkowy
na bieżni ruchomej według zmodyfikowanego protokółu
Bruce’a oraz spirometrię spoczynkową, oceniając
szczytowe pochłanianie tlenu (peak VO2),
wskaźnik wentylacji do perfuzji (VE/VCO2, ventilation
volume to CO production). Określono wartości
spoczynkowego i wysiłkowego ciśnienia tętniczego.
Za nadciśnienie tętnicze indukowane wysiłkiem
uznano ciśnienie skurczowe na szczycie wysiłku powyżej
200 mm Hg u pacjenta z prawidłowymi wyjściowymi
wartościami ciśnienia tętniczego.
Wyniki Odnotowano: brak nadciśnienia (32 osoby),
nadciśnienie tętnicze utrwalone (31) i nadciśnienie
tętnicze indukowane wysiłkiem (10). U chorych
z nadciśnieniem tętniczym indukowanym wysiłkiem
stwierdzono mniejszą konsumpcję szczytową tlenu
(p = 0,01) i większy VE/VCO2 (p = 0,01) niż
u pacjentów bez tego schorzenia. Wartości LVM oraz
LVMI u osób z nadciśnieniem tętniczym indukowanym
wysiłkiem były większe niż u chorych bez tego
schorzenia (odpowiednio: p = 0,03 i p = 0,04).
Wnioski 1. U dorosłych pacjentów po operacji CoAo
z nadciśnieniem tętniczym indukowanym wysiłkiem
oraz u osób z utrwalonym nadciśnieniem tętniczym
zaobserwowano występowanie negatywnych czynników
rokowniczych pod postacią upośledzenia tolerancji
wysiłku oraz powiększenia LVM. 2. Powyższe
obserwacje sugerują, że chorzy po operacji koarktacji
aorty z nadciśnieniem tętniczym indukowanym
wysiłkiem wymagają szczególnej wnikliwej obserwacji
w celu ustalenia ewentualnych wskazań do wczesnego
wdrożenia leczenia przeciwnadciśnieniowego.Background The majority of the patients after successful
operation of coarctation of the aorta (CoAo) suffer from
arterial hypertension (AH) and some from exercise induced
arterial hypertension (AHex). The aim of the study
was evaluate exercise capacity in cardiopulmonary exercise
test and left ventricular mass on echocardiography examination
in adult patients after surgical repair of CoAo
according to presence of AH and AHex.
Material and methods the analysis of 74 patients
(29 females) aged 31.2 ± 9.8 years operated at mean age
10.4 ± 6.6 years. Control group: 30 volunteers (12 females)
aged 32.2 ± 6.6 years.
On echocardiography LV mass (LVM) and LV mass index
(LVMI) were calculated. All patients performed a maximal,
symptom-limited treadmill exercise test according to
modified Bruce protocol. The maximal oxygen consumption
(peak VO2), ventilation/carbon dioxide slope (VE/
/VCO2) were measured. Arterial hypertension was measured
at rest and during max. exercise. AHex was defined as
systolic pressure at peak exercise > 200 mm Hg in patients
with initially normal blood pressure.
Results without AH (AH–) - 32 patients, AH - 31 patients,
AHex - 10 patients. AHex patients had lower peak
VO2 (p = 0.01), higher VE/VCO2 (p = 0.01) than AH(–)
LVM and LVMI in AHex group were greater than in AH(–)
group (respectively p = 0.03 and p = 0.04).
Conclusions 1. Adult patients after operation of CoAo with
AHex have similar negative risk factors: lower exercise capacity
and LV enlargement of as patients with persistent
AH. 2. Above results suggest that patients after operation of CoAo with AHex need special insightful observation.
Additionary research are necessary to establish potential
indication for early antihypertension treatment in this
group of patients
Protozoan Intestinal Parasitic Infection in Patients with Hematological Malignancies
The aim of this study was to evaluate the frequency of gastrointestinal protozoan infection in patients with hematological malignancies (HMs) undergoing intensive hemato-oncological treatment and to determine the influence of certain biological factors on the incidence of intestinal parasite infection. Stool samples were collected from hematological malignancy patients (n = 50) hospitalized at the Department of Hematology and Transplantology of the Pomeranian Medical University in Szczecin. The control group consisted of 50 healthy participants. We used a direct smear examination and a commercial immunoenzymatic test. Intestinal protozoans were detected in 16% of patients with hematological malignancies and in 6% of individuals in the control group. In stool samples from patients with HM, cysts of Giardia intestinalis (2%), oocysts of Cryptosporidium spp. (10%), vacuolar forms of potentially pathogenic Blastocystis spp. (2%), and cysts of nonpathogenic Entamoeba coli (2%) were found. Cryptosporidium spp. and Giardia intestinalis coproantigens were detected in 5 (10%) and 1 (2%) patients with HM, respectively. In three participants from the control group, vacuolar forms of Blastocystis spp. were found. In the patients with HM, a significantly higher prevalence of intestinal parasite infection was found in individuals working in the garden without protective gloves and those in contact with animals. In patients with hematological malignancies, intestinal parasites should be excluded, even during intensive chemotherapy treatment
Cadmium, lead and mercury in human kidneys
The development of civilization and economic activities
affect the increase in the number and variety of xenobiotics,
including some heavy metals in the environment. Hence,
human is constantly exposed to many harmful substances
in air, water, soil and food. It is well known that cadmium
(Cd), lead (Pb) and mercury (Hg) are the most toxic socalled
heavy metals (density ¤4.5 g/cm3) [1]. The critical
organs accumulating these metals are kidneys, which are
involved in the detoxification process. The concentration of
metals in the kidneys is affected by environmental factors,
such as smoking, diet, or amalgam dental fillings (mercury),
and some biological factors – as age and gender. Recently,
research was conducted on the impact of the Renal Cell
Carcinoma (RCC) on the concentration of these metals in
kidneys. Patients with kidney cancer accumulate less cadmium
than healthy ones. Therefore it is very important to
monitor the concentrations of highly toxic metals in the
excretory organ, particularly in patients with nephrological
disorders.Rozwój cywilizacji i działalności gospodarczej wpływają
na wzrost ilości i różnorodności ksenobiotyków, w tym pewnych
metali ciężkich, w środowisku. Dlatego człowiek stale
narażony jest na kontakt z wieloma szkodliwymi substancjami
znajdującymi się w powietrzu, wodzie, glebie i pożywieniu.
Z tak zwanych metali ciężkich (o gęstości ¤4,5 g/cm3)
za najbardziej toksyczne uznaje się kadm (Cd), ołów (Pb)
i rtęć (Hg), które nazywane są „trójką metali śmierci” [1].
Narządem krytycznym kumulującym te metale są nerki,
które w dużym stopniu biorą udział w procesie detoksykacji.
Na stężenie metali w nerkach mają wpływ czynniki środowiskowe,
takie jak palenie tytoniu, dieta czy posiadanie
amalgamatowych wypełnień stomatologicznych (rtęć),
a także czynniki biologiczne – wiek i płeć. Od niedawna
prowadzone są także badania o wpływie raka jasnokomórkowego
na stężenie tych metali w nerce. Okazuje się, że
nerki osób z nowotworem tego narządu w mniejszym stopniu
kumulują kadm, niż nerki osób zdrowych. Istotne jest
zatem poznanie i dalsze monitorowanie stężeń wybitnie toksycznych
metali w tym narządzie wydalniczym, szczególnie
u osób z rozpoznanymi schorzeniami nefrologicznymi, łącznie
z tymi, które doprowadziły do usunięcia nerki
Neurotrophic Factors in Experimental Cerebral Acanthamoebiasis
To date, no studies have addressed the role of neurotrophins (NTs) in Acanthamoeba spp. infections in the brain. Thus, to clarify the role of NTs in the cerebral cortex and hippocampus during experimental acanthamoebiasis in relation to the host immune status, the purpose of this study was to determine whether Acanthamoeba spp. may affect the concentration of brain-derived neurotrophic factor (BDNF), nerve growth factor (NGF), neurotrophin-3 (NT-3), and neurotrophin-4 (NT-4) in brain structures. Our results suggest that at the beginning of infection in immunocompetent hosts, BDNF and NT-3 may reflect an endogenous attempt at neuroprotection against Acanthamoeba spp. infection. We also observed a pro-inflammatory effect of NGF during acanthamoebiasis in immunosuppressed hosts. This may provide important information for understanding the development of cerebral acanthamoebiasis related to the immunological status of the host. However, the pathogenesis of brain acanthamoebiasis is still poorly understood and documented and, therefore, requires further research

The Activity of Matrix Metalloproteinases (MMP-2, MMP-9) and Their Tissue Inhibitors (TIMP-1, TIMP-3) in the Cerebral Cortex and Hippocampus in Experimental Acanthamoebiasis
The pathological process occurring within the central nervous system (CNS) as a result of the infection by Acanthamoeba spp. is not fully understood. Therefore, the aim of this study was to determine whether Acanthamoeba spp. may affect the levels of matrix metalloproteinases (MMP-2,-9), their tissue inhibitors (TIMP-1,-3) and MMP-9/TIMP-1, MMP-2/TIMP-3 ratios in the cerebral cortex and hippocampus, in relation to the host’s immunological status. Our results showed that Acanthamoeba spp. infection can change the levels of MMP and TIMP in the CNS and may be amenable targets for limiting amoebic encephalitis. The increase in the activity of matrix metalloproteinases during acanthamoebiasis may be primarily the result of inflammation process, probably an increased activity of proteolytic processes, but also (to a lesser extent) a defense mechanism preventing the processes of neurodegeneration
