Román Ferreras imparte en la E.I.T.O.P., se solicita de aquellos la presentación de un estudio, que al suponer trabajo personal tiene gran repercusión en la calificación final. De entre los realizados últimamente hemos seleccionado el presente, que contiene aportaciones de indudable interés al conocimiento de nuestro acervo arquitectónico y refleja las posibilidades del análisis armónico como herramientas de diseño

    Improving the Advantages of Single Port in Right Hemicolectomy: Analysis of the Results of Pure Transumbilical Approach with Intracorporeal Anastomosis

    Background. Single-port laparoscopic surgery has recently emerged as a method to improve patient recovery and cosmetic benefits of laparoscopic surgery. The evolution of our technique has led us to move from a periumbilical incision to a transumbilical one, avoiding the use of drain and maintaining a pure single-port approach with intracorporeal anastomosis in order to maintain the incision as smaller as possible. Method. We report a prospective clinical analysis of our first 38 patients. Oncological surgical steps were followed as during the standard laparoscopic approach, performing the anastomosis intracorporeally in all cases. Results. Mean age of 68,39 years old and an average BMI of 27,88%. (range 19,81–41,5). Most lesions were adenocarcinoma (65,8%), while the remaining were polyps (31,5%) and one a mucocele of the appendix. We moved from a periumbilical incision, initial 14 cases, into a transumbilical one, (medium size of the incision 3,25 cm). Average surgical time was 117,42 minutes. Drains was only used in our first 3 cases. Mean hospital stay was 5,2 days, (86,5% stayed less than 5 days). Total morbidity was 13%. Histological exams of the specimens showed that the oncological criteria were preserved. Conclusions. Single-port right hemicolectomy with intracorporeal anastomosis is feasible and safe. The advantages of a total intracorporeal anastomosis include that there is no need to enlarge the umbilical incision and avoid traction of the pedicle of the mesenterium of the transverse colon during the extracorporeal anastomosis. A transumbilical incision offers better cosmetic results, and the use of drains can be avoided, which increase, patient's satisfaction

    Transient-Based Rotor Cage Assessment in Induction Motors Operating With Soft Starters

    [EN] The reliable assessment of the rotor condition in induction motors is a matter of increasing concern in the industry. Although rotor damages are more likely in line-started motors operating under high inertias, some cases of broken rotor bars in motors supplied via soft starters have been also reported in the industry. Motor current signature analysis (MCSA) is the most widely spread approach to diagnose such failures. However, its serious drawbacks in many real industrial applications have encouraged investigation on alternative methods enhancing the reliability of the diagnosis. This paper extends a recently introduced diagnosis methodology relying on the startup current analysis to the case of soft-starter-operated motors. The approach has proven to provide very satisfactory results, even in cases where the classical MCSA does not lead to correct diagnosis conclusions. However, its extension to operation under soft starters was still a pending issue.     Necrosis avascular secundaria al tratamiento de la luxación congénita de cadera: relación entre factores terapéuticos y secuelas radiológicas

    Se presentan 108 casos de Luxación Congénita de Cadera (LCC) unilateral tratados con la misma metódica terapéutica: tracción más reducción abierta o cerrada dependiendo de los hallazgos artrográficos. Tras un seguimiento medio de 7 años (Rango: 5-13), 5 (5%) tenían una coxa magna, 14 (13%) mostraban disminución de la altura epifisaria, 22 (20%) tenían una coxa magna con disminución de la altura epifisaria 10 (9%) presentaban lesión fisaria residual. El análisis estadístico demostró asociación significativa (p<0,05 ) entre el desarrollo de coxa magna con disminución de la altura epifisaria y la ausencia de descenso cefálico al terminar la tracción, así como con la reducción abierta. La lesión fisaria residual, se encontró asociada significativamente a LCC Tipo IV de Tönnis, caderas que estuvieron más de 5 semanas en tracción, fallo en el descenso cefálico al finalizar la tracción y reducción abierta. En conclusión, se recomienda la tracción preoperatoria «efectiva», que desciende la cabeza femoral a nivel del cotilo, para disminuir las alteraciones radiológicas finales, secuelas de necrosis avascular.A total of 108 patients with unilateral congenital dislocation of the hip treated by the same therapeutic approach, are reviewed. The protocol for treatment consisted in traction and open or closed reduction, depending of the arthrographic findings. After 7-year follow-up (range, 5-13), 5 (5%) had coxa magna, 14 (13%) showed a decrease in epiphyseal height, 22 (20%) exhibited both coxa magna and decreased epiphyseal height, and 10 (9%) showed physeal damage. The statistical analysis revealed a significant relationship (p < 0,05) between the development of coxa magna with decreased epiphyseal height and both an absence of femoral head descent after traction and an open reduction of the hip. Physeal damage was found to be associated to Tönnis type-IV congenital dislocation, to hips undergoing more than 5 weeks traction, to failed cephalic descent following traction and an open reduction procedure. In conclusion, a effective preoperative hip traction allowing an appropriate descent of the femoral head to the acetabulum is recommended in order to prevent radiological alterations induced by avascular necrosis

    Efficacy and safety of trabectedin in metastatic uterine leiomyosarcoma: A retrospective multicenter study of the Spanish ovarian cancer research group (GEICO)

    Objective: We assessed trabectedin in patients with advanced uterine leiomyosarcoma (uLMS) in real-life clinical practice given according to the marketing authorization. Methods: Thirty-six women from 11 tertiary hospitals across Spain who received trabectedin after anthracyclinecontaining regimen/s were retrospectively analyzed. The primary endpoint was progression-free survival (PFS). Results: Median PFS and overall survival (OS) since starting trabectedin treatment were 5.4 (95%CI: 3.5–7.3) and 18.5 months (95%CI: 11.5–25.6), respectively. Median OS was significantly higher (P = 0.028) in patients receiving trabectedin in ≤ 2nd line (25.3 months) than in ≥ 3rd (15.1 months) and with ECOG performance status ≤ 1 at trabectedin start (19.8 months) than ECOG 2–3 (6.0 months, P = 0.013). When calculating OS since diagnosis, patients had longer OS with localized disease at diagnosis (87.4 months) vs. locally advanced (30.0 months) or metastatic (44.0 months, P = 0.041); and patients who received adjuvant therapy (87.4 months) compared with those who did not (30.0 months, P = 0.003), especially when receiving radiochemotherapy (106.7 months, P = 0.027). One patient (2.8%) had a complete response (CR) and nine patients (25.0%) achieved a partial response (PR) for an objective response rate of 27.8% with median response duration of 11 months (range: 4–93). Eighteen patients (50.0%) had disease stabilization for a disease control rate (DCR) of 77.8%. More patients receiving trabectedin in 1st-line of advanced disease achieved CR (16.7%) and PR (50.0%) than those in ≥ 2nd line/s (0.0% and 20.0%), whereas the DCR was similar across treatment lines. Reversible neutropenia was the most common grade 3/4 laboratory abnormality (19.4%). Conclusions: Trabectedin confers clinical benefit in patients with recurrent/metastatic uLMS, given after failure to an anthracycline-based regimen being comparable to those reported in clinical trials and with a manageable safety profile

    MTOC translocation modulates IS formation and controls sustained T cell signaling

    The translocation of the microtubule-organizing center (MTOC) toward the nascent immune synapse (IS) is an early step in lymphocyte activation initiated by T cell receptor (TCR) signaling. The molecular mechanisms that control the physical movement of the lymphocyte MTOC remain largely unknown. We have studied the role of the dynein–dynactin complex, a microtubule-based molecular motor, in the process of T cell activation during T cell antigen–presenting cell cognate immune interactions. Impairment of dynein–dynactin complex activity, either by overexpressing the p50-dynamitin component of dynactin to disrupt the complex or by knocking down dynein heavy chain expression to prevent its formation, inhibited MTOC translocation after TCR antigen priming. This resulted in a strong reduction in the phosphorylation of molecules such as ζ chain–associated protein kinase 70 (ZAP70), linker of activated T cells (LAT), and Vav1; prevented the supply of molecules to the IS from intracellular pools, resulting in a disorganized and dysfunctional IS architecture; and impaired interleukin-2 production. Together, these data reveal MTOC translocation as an important mechanism underlying IS formation and sustained T cell signaling

    Effect of different quality irrigation water on the growth, mineral concentration and physiological parameters of Viburnum tinus plants

    Treated wastewater may be considered an alternative source of water and fertilizer nutrients for landscape plants. However, NaCl, the principal compound in this water, can be detrimental to plants. Viburnum tinus plants were submitted for 4 months to 4 irrigation treatments with water from different sources: control (Control) (EC < 0.9 dS m-1); NaCl solution (NaCl) (EC = 4 dS m-1); irrigation water normally used in the area (IW) (EC = 1.2-1.8 dS m-1) and reclaimed water (RW) (EC = 4 dS m-1). During a recovery period of two months, all the plants were irrigated with the control water. The results showed that biomass was affected in NaCl, IW and RW treatments, both leaf area and height decreased at the end of saline period. These changes were more pronounced in the NaCl treatment, which also caused a decrease in stem diameter and root/shoot ratio. The similar growth alterations founded after the recovery period showed that salts continued to be present in the substrate. Compared with the control, NaCl and RW plants showed a greater rate of Na+ and Cl- absorption by roots. Stem water potential was mainly affected by the NaCl treatment in the last weeks of the saline period. Through the experiment, the stomatal conductance and photosynthesis values were the lowest in plants that received the highest amount of salts, especially NaCl treatment which was the only treatment that did not recover at the end of experiment. Plants of the IW treatment showed slight changes in stomatal conductance and photosynthetic rate with respect to the control, although the reduction in the growth and size of these plants suggests that slight increases in EC could be very toxic for this species. Hence, using different sources of water with similar EC, (NaCl and RW) it is important to know the exact composition, since the toxic effects produced by high concentrations of Na+ and Cl- might be offset by the effect of other ions like magnesium, potassium and phosphorus. In the physicochemical analysis of water, the highest concentrations of these ions were observed in RW and as consequence, their concentrations in plants were not reduced by the Na+ and Cl- effect, phosphorus even increased, improving the plant nutritional balance.This work was supported by the projects: CICYT (AGL 2008- 05258-CO2-1-2- AGR and AGL 2011-30022-CO2-01-02), Fundación Séneca (15356/PI/10) and Vías y Construcción, S.     Integración de técnicas de usabilidad y accesibilidad en el proceso de desarrollo de software de las mipymes

    El creciente uso de productos usables y accesibles, ha transformado la forma de desarrollar software. Estos aspectos hacen par te del Diseño centrado en el Usuario DCU, como guía par a mejorar la calidad de vida, lograr un mejor entendimiento, ampliar el mercado e incrementar la satisfacción del cliente. A pesar de esto, la importancia del DCU dentro del proceso de desarrollo de software solo se considera como un atributo final del producto y no como un elemento inherente e incluyente durante el transcurso del desarrollo. Esta ausencia de aplicación se presenta aun más en los procesos de desarrollo de las micro, pequeñas y medianas empresas productor as de software MiPyMEs PS, que algunas veces no cuentan con un proceso bien definido. La utilización de técnicas de usabilidad y accesibilidad se puede ver reflejado en beneficios de tiempo, costo y calidad, ejes centrales en la continuidad y prosperidad del los objetivos del negocio. Este tr abajo, aún en desarrollo, muestra los principios básicos del DCU junto con el papel que puede jugar en el éxito de un producto al incluir lo en el proceso de desarrollo de software
