    将来の改造を考慮した高齢者住宅の計画に関する研究

    Japan is experiencing a rapid ageing in its society and various styles of housings are being offered to the elderly people. A large number of the ageing population wish to continue to live in their same homes even when their physical abilities are weakened. Ministries in charge are publishing design guidelines for the dwellings for elderly. However, it is quiet difficult to incorporate into the original design plan all the future needs coming from the ageing of the residents. There should be many cases where it is more appropriate to make changes as needs arise. In this study, we have conducted two surveys. 1) Design plans of the 256 houses shown in the catalogs of the house building companies and the plans of the 60 houses actually built were studied to assess the current status of adopting the "barrier-free" design. We have made a checklist for this study and have used this checklist for assessment. 2) For the existent 60 houses, we made renovation plans accommodate to the living with wheelchairs. Then, we listed up the changes needed to make. We also studied the reasons for not being able to accommodate such changes in the original design plan of the house, taking the individual circumstance of each family into considerations. Through the analysis of the survey results, we have re-classified the contents of the guidelines based on the needs for the future renovation. Physical capabilities of the aged residents have been classified into three (3) levels and appropriate application of the guidelines to each level has been clarified

    Spatial Evolution of Wave‐Particle Interaction Region Deduced From Flash‐Type Auroras and Chorus‐Ray Tracing

    In-situ observations of spatial variations of the wave-particle interaction region require a large number of satellite probes. As an alternative, flash-type auroras, a kind of pulsating aurora, driven by discrete chorus elements, can be used to investigate the interaction region with a high spatial resolution. We estimated the spatial extent of wave-particle interaction region from ground-based observations of flash aurora at Gakona (62.39°N, 214.78°E), Alaska at subauroral latitudes, and found that the auroral expansion was predominantly to the low-latitude side. The spatial displacement is thought to be caused by the propagation effects of chorus waves in the magnetosphere. Using ray tracing analysis to take into account chorus wave propagation, we reconstructed the spatiotemporal evolution of the volume emission rate and confirmed that the predominant expansion is toward the lower-latitude side in the ionosphere. This study shows that chorus wave propagation in the magnetosphere gives new insight for characterizing the transverse size (across the geomagnetic field line) of wave-particle interaction regions. The calculated spatial scale of the column auroral emission shows a correlation with the magnetic latitude of the resonance region at magnetic latitudes within 10° of the equatorial plane of the magnetosphere. Our results suggest that the spatial scale of a flash aurora is indirectly related to the chorus amplitude because the latitudinal range of the wave-particle interaction is important for the growth of wave amplitude

    An Autopsy Case of Multiple Jejunal Diverticula Showing Severe Malabsorption

    A rare autopsy case of multiple jejunal diverticula showing severe malabsorption is reported. A 56-year-old man was admitted due to vomiting and leg edema. On admission, his height was 160cm, his body weight was 39kg, and laboratory data revealed severe hypoproteinemia (TP: 4.0g/dl, ALB: 2.1g/dl). On the 14th day of admission, agonal breathing and disturbance of consciousness occurred after massive vomiting of gastric juice, and the patient died of respiratory failure. At autopsy, on abdominal sectioning, multiple diverticula situated on the mesenterium side of the enteron extending 70cm in length from the proximal jejunum were identified. However there were no findings suggesting perforation or diverticulitis. Histologically, the diverticula were lined by ordinal jejunum mucosa associated with muscularis mucosa, but the muscularis propria was not involved in the diverticular walls. The diverticula were identified as false diverticula. In both lower lungs, aspiration pneumonia was widely seen. The cause of death was considered to be aspiration pneumonia due to the vomiting caused by multiple jejunal diverticula.Only 16 case reports of multiple jejunal diverticulosis in Japan could be found in the literature however most of the reported complications were perforation and diverticulitis, and there were no reports of malabsorption. Therefore, the present case is significant concerning the cause of malabsorption in routine explorations

    A Pilot Study for the Detection of Protozoa Infections of the Gut at Autopsy

    In order to investigate the incidence of latent infections by parasites within the human digestive tract, we examined fresh stool samples from the colon of 31 patients (37-90 years, median age 71; 26 men and 5 women) collected within 12 hours of death. These subjects had been admitted to a university hospital in Yokohama, Japan, and died between April 2007 and December 2008 from causes other than parasitosis. Stool samples were fixed and stained for microscopic analysis, and PCR analysis for Entamoeba histolytica was performed for the parasite-positive samples. Results showed that ten out of 31 subjects were infected by E. histolytica only, one subject was infected by Giardia intestinalis only, and four subjects were infected by both E. histolytica and G. intestinalis. These findings are in contrast to conventional theories concerning general parasite infection in Japan, and indicate continuous or latent infection within the human digestive tract. The presence of pathogens such as E. histolytica and G. intestinalis in elderly or immuno-compromised patients is a serious issue and warrants further attention as a public health issue, particularly in relation to its mode of transmission

    Magnetic Conjugacy of Pc1 Waves and Isolated Proton Precipitation at Subauroral Latitudes: Importance of Ionosphere as Intensity Modulation Region

    Pc1 geomagnetic pulsations, equivalent to electromagnetic ion cyclotron waves in the magnetosphere, display a specific amplitude modulation, though the region of the modulation remains an open issue. To classify whether the amplitude modulation has a magnetospheric or ionospheric origin, an isolated proton aurora (IPA), which is a proxy of Pc1 wave-particle interactions, is compared with the associated Pc1 waves for a geomagnetic conjugate pair, Halley Research Base in Antarctica and Nain in Canada. The temporal variation of an IPA shows a higher correlation coefficient (0.88) with Pc1 waves in the same hemisphere than that in the opposite hemisphere. This conjugate observation reveals that the classic cyclotron resonance is insufficient to determine the amplitude modulation. We suggest that direct wave radiation from the ionospheric current by IPA should also contribute to the amplitude modulation

    A Rare Case of Peripheral Primitive Neuroectodermal Tumor Arising from the Minor Salivary Gland in a Young Woman

    We report here a case of peripheral primitive neuroectodermal tumor (PNET) arising from the minor salivary gland. A 22-year-old woman was admitted to our hospital for surgical excision of a small painless cheek tumor with a 7-month history. Macroscopically, the tumor measured 10 × 5 × 6mm and was located in the minor salivary gland. Microscopically, the tumor comprised proliferating, small, round cells with scant cytoplasm and high nuclear cytoplasmic ratios. The tumor cells showed some mitotic figures and Homer-Wright-type rosettes. Immunohistochemically, the tumor cells were immunopositive for CD99, synaptophysin, CD56, S-100 protein, and vimentin. Based on these findings, the patient was diagnosed as having PNET arising from the minor salivary gland. There are very few case reports of PNET in the head and neck region, and to the best of our knowledge, this is the first case report of PNET arising from the minor salivary gland
