    Audit of perinatal mortality at SSMCHRC-(Rural teaching hospital) a retrospective study

    Objective: To estimate magnitude, determinants of perinatal mortality and suggest remedial measures for its reduction. Background: Perinatal mortality is mirror reflection of maternal and child health and socioeconomic environment of community. It is influenced by various medicosocial preventable causes. It can be reduced by improving maternal and child health services and by health education. Methods: A retrospective hospital based study of perinatal deaths among 2333 deliveries was conducted from June 2008 to June 2010 in our hospital. Fetomaternal factors like maternal age, religion, residence, parity, mode of delivery, booking status, antenatal complications, baby’s sex, birth weight, congenital anomalies, neonatal complications influencing perinatal mortality rate were tabulated and analyzed. Cause of perinatal death was assessed. Results: perinatal mortality rate was 127.4/1000 total births. Maternal factors like age more than 35 years, muslim religion, inadequate antenatal care, primiparity, grand multiparity, induced deliveries and neonatal factors like low birth weight, prematurity were associated with increased perinatal mortality. The leading cause of stillbirth was antepartum hemorrhage and prematurity for neonatal mortality. Conclusion: Apart from clinical causes high perinatal mortality was due to poverty, illiteracy, lack of health awareness, inadequate antenatal care and delayed referral. Health education, identification of high risk mothers, timely referral, advanced life support of preterm neonates should significantly help to reduce perinatal deaths

    A comparative study on efficacy and safety of intravenous iron sucrose and oral iron among anaemic pregnant females

    Background: Iron deficiency anaemia (IDA) is the most common medical problem in pregnancy. Both parenteral and oral iron therapies are modalities used for management of anaemia in pregnant females. The present study aimed to compare the efficacy of oral and intravenous iron therapy in improving iron deficiency anaemia in pregnancy and restoring iron stores, compare the pregnancy outcome in the two groups and evaluate their safety.Methods: This prospective randomized clinical trial conducted among pregnant women between 14 and 36 weeks with established IDA who were treated with IVIS (Intravenous Iron Sucrose) or OI (ferrous sulphate). All patients were monitored for laboratory response and adverse effects. Independent sample-t test, Chi square test and ANOVA were used for statistical analysis. P <0.05 was considered significant.Results: From first follow up till the end of the study the mean hemoglobin level of IV Group was found to be significantly higher as compared to that of oral Group. The MCV (mean corpuscular volume) values increased to a significant level in the intravenous group as compare to oral. After therapy increase transferrin saturation in the intravenous group were significantly higher at all-time interval than the oral group. The mean neonatal hemoglobin at birth was significantly higher in IV group 16.88±1.96 gm/dl than oral group 16.88±1.96 (p= p<0.0001).Conclusions: Intravenous iron sucrose is comparatively more efficient in improving haemoglobin than oral iron and has better pregnancy outcomes

    ASSESSMENT OF LAPAROSCOPIC APPENDECTOMY IN PATIENTS OF COMPLICATED APPENDICITIS IN TERTIARY CARE TEACHING HOSPITAL OF CENTRAL INDIA

    Background: Appendicitis is the commonest abdominal surgical emergency in the world which may lead to complications like appendicular abscess or mass, gangrene, perforation and peritonitis. Present research aimed to evaluate the wellbeing and the effectiveness of laparoscopy for managing complicated appendicitis. Material and methods: Present study was carried out on 50 patients at tertiary care institution of Chhattisgarh in Central India for the period of 2 year. This study is including patients of complicated appendicitis undergoing laparoscopic management. Parameters studied included Age, Gender, WBC count, wound infection and hospital stay. Results: There was increase of total leucocytic count in most of the patients; Mean WBCs was 12.71±5.37. 33 patients had pus free IPF collection and perforated appendicitis (PA), 11 patients had turbid free IPF collection with AA (highly inflamed appendix), 2 case was mucocele of the appendix, 2 cases of appendicular abscess (3.3%) and 2 cases of gangrenous appendix. Conclusion: Management of complicated appendicitis laparoscopically is practicable, secure and can present a small occurrence of infectious impediments, fewer post-operative pain, fast revival and improved cosmesis on the cost of extended operating time than Open Appendectom

    A comprehensive curated resource for follicle stimulating hormone signaling

    <p>Abstract</p> <p>Background</p> <p>Follicle stimulating hormone (FSH) is an important hormone responsible for growth, maturation and function of the human reproductive system. FSH regulates the synthesis of steroid hormones such as estrogen and progesterone, proliferation and maturation of follicles in the ovary and spermatogenesis in the testes. FSH is a glycoprotein heterodimer that binds and acts through the FSH receptor, a G-protein coupled receptor. Although online pathway repositories provide information about G-protein coupled receptor mediated signal transduction, the signaling events initiated specifically by FSH are not cataloged in any public database in a detailed fashion.</p> <p>Findings</p> <p>We performed comprehensive curation of the published literature to identify the components of FSH signaling pathway and the molecular interactions that occur upon FSH receptor activation. Our effort yielded 64 reactions comprising 35 enzyme-substrate reactions, 11 molecular association events, 11 activation events and 7 protein translocation events that occur in response to FSH receptor activation. We also cataloged 265 genes, which were differentially expressed upon FSH stimulation in normal human reproductive tissues.</p> <p>Conclusions</p> <p>We anticipate that the information provided in this resource will provide better insights into the physiological role of FSH in reproductive biology, its signaling mediators and aid in further research in this area. The curated FSH pathway data is freely available through NetPath (<url>http://www.netpath.org</url>), a pathway resource developed previously by our group.</p

    Integrating transcriptomic and proteomic data for accurate assembly and annotation of genomes

Complementing genome sequence with deep transcriptome and proteome data could enable more accurate assembly and annotation of newly sequenced genomes. Here, we provide a proof-of-concept of an integrated approach for analysis of the genome and proteome of Anopheles stephensi, which is one of the most important vectors of the malaria parasite. To achieve broad coverage of genes, we carried out transcriptome sequencing and deep proteome profiling of multiple anatomically distinct sites. Based on transcriptomic data alone, we identified and corrected 535 events of incomplete genome assembly involving 1196 scaffolds and 868 protein-coding gene models. This proteogenomic approach enabled us to add 365 genes that were missed during genome annotation and identify 917 gene correction events through discovery of 151 novel exons, 297 protein extensions, 231 exon extensions, 192 novel protein start sites, 19 novel translational frames, 28 events of joining of exons, and 76 events of joining of adjacent genes as a single gene. Incorporation of proteomic evidence allowed us to change the designation of more than 87 predicted noncoding RNAs to conventional mRNAs coded by protein-coding genes. Importantly, extension of the newly corrected genome assemblies and gene models to 15 other newly assembled Anopheline genomes led to the discovery of a large number of apparent discrepancies in assembly and annotation of these genomes. Our data provide a framework for how future genome sequencing efforts should incorporate transcriptomic and proteomic analysis in combination with simultaneous manual curation to achieve near complete assembly and accurate annotation of genomes
