    A Case of Suspected Lisdexamfetamine (Vyvanse) Drug-Induced Liver Injury

    Amphetamines are a well-established cause of liver injury though the typical presentation is that of illicit drug abuse rather than liver injury occurring in a prescribed setting.1 Lisdexamfetamine (Vyvanse) is one of the most commonly prescribed stimulant medications used for the treatment of Attention Deficit Hyperactive Disorder (ADHD). The authors were only able to find a single case of lisdexamfetamine-related drug-induced liver injury (DILI) occurring in the pediatric population in their review of the literature. Here we present a case of suspected lisdexamfetamine DILI in an adult patient

    Genetic diversity analysis of some Bangladeshi aromatic rice (Oryza sativa L.) using simple sequence repeat markers (SSRM)

    In order to germplasm management, conservation, parental identiļ¬cation and proper utilization of aromatic rice germplasm of Bangladesh genetic diversity assessment and molecular characterization is necessary. We used ten microsatellite markers across twenty aromatic rice landraces along with four improved varieties to discriminate and characterize among them. The number of alleles per locus ranged from 2 to 8, with an average of 4.30 alleles across 10 loci. A total of 43 polymorphic alleles were detected. The values of Polymorphic information content (PIC) ranged from 0.217 to 0.835 (average 0.495) which indicate high genetic diversity among the studied aromatic rice genotypes. It was concluded by the PIC value of RM5339 that it might be the finest marker for diversity estimation and characterize of these aromatic rice genotypes, followed by RM334, RM414 and RM28502 markers. The UPGMA cluster dendrogram constructed in this study identiļ¬ed seven clusters with a correlation coefficient 0.874. Molecular characterization of aromatic rice landraces of Bangladesh exhibited large variations among the genotypes. Five rice genotypes namely BRRI dhan38, BRRI dhan50, Bashmoti safed, Malaysira, Khas-kani showed highest genetic dissimilarity among the studied rice genotypes. The ļ¬ndings of this study would be useful for background selection in backcross breeding programs for aromatic rice improvement as well as identification of genetically distant and genetically close accessions for maintenance and conservation

    A simulation assessment of the height of light shelves to enhance daylighting quality in tropical office buildings under overcast sky conditions in Dhaka, Bangladesh

    The objective of this paper is to highlight the effectiveness of light shelves in tropical office buildings to enhance interior daylighting quality. Daylight simulation was performed for custom light shelves for a typical office floor of Dhaka City in Bangladesh, to determine the best possible location under overcast sky conditions. Six alternative models of a 3m high study space were created with varying heights of light shelves. The 3D models were first generated in the Ecotect to study the distribution and uniformity of daylight in the interior space with split- flux method. These models were then exported to a physically-based backward raytracer, Radiance Synthetic Imaging software to generate realistic lighting levels for validating and crosschecking the Ecotect results. The results showed that for achieving light levels closest to specified standards, light shelves at a height of 2m above floor level perform better among the seven alternatives studied including the alternative where no light shelves are present. Finally, the decisions were verified with DAYSIM simulation program to ensure the compliance of the decisions with dynamic annual climate-based daylight performance metrics

    Daylight simulation for sustainable urban office building design in Dhaka, Bangladesh: decision-making for internal blind configurations

    In an urban site, daylight strategies increasingly depend on the availability of natural light, which is influenced largely by the immediate surroundings of the building, particularly the presence of natural and manmade obstructions. Under such conditions the latitude of the site and its regional climatic conditions, such as ambient outdoor daylight levels and sunshine probability are found to have diminished direct impacts on the interior daylight potential. In these highly specific urban environments created by built-up surroundings, no generalised way exists to describe or predict the luminous microclimate. However, simulation can be used as a design tool for sustainable daylight design. Using simulation programs, this paper examines the significant impact of surroundings on daylight probability on urban buildings. Daylight simulation was performed in this study by creating the virtual urban environment based on the information of a true site urban office building in Dhaka, Bangladesh, a tropical location, with predominantly overcast skies. The 3D models were first generated for computer simulation in the Ecotect program to calculate the amount of daylight incident on a generated grid point on the work-plane. These models were then exported to Radiance Synthetic Imaging software to generate realistic lighting levels and finally verified with DAYSIM simulation program for annual performance evaluation. The results show that daylight entering from different sides of the building is affected vastly by surroundings and this influences the indoor illuminance and luminance distribution. This paper also demonstrates a case of decision-making between two most popular configurations (vertical and horizontal) of internal blinds used in urban offices of Dhaka. The focus of the paper is to highlight the importance of daylight simulation in sustainable urban office building design, while at the same time gives a general methodology for decision-making regarding daylight design elements

    Feasibility and acceptability of telepathology system among the rural communities of Bangladesh: A pilot study

    Context: Telepathology is a promising tool for remote communities to receive pathology services where professional diagnosis services are inadequate. Aims: We aimed to clarify how effective telepathology was when compared with conventional pathology service among rural communities of Bangladesh. Methods and Materials: We conducted a cross-sectional study in suburban and rural areas of Bangladesh between June and August 2020. We enrolled 117 participants who received both telepathology services from Thakurgaon Eye Hospital and conventional pathology service experience. The participant's satisfaction with the accessibility and perceptions were statistically compared. In addition, we summarized descriptive statistics using the frequencies and percentages of participants' responses. Statistical Analysis Used: Wilcoxon's Signed-rank test using SPSS statistic software version 25.00. Results: Among the study participants, service cost, travel cost, travel time, waiting time, and travel distance were significantly higher for conventional pathology than telepathology (P < 0.001). The majority of participants (94%) were satisfied with the telepathology experience; however, one out of 117 participants was dissatisfied with this service when their travel distance was far away (ā‰„50 km). Among the participants, 91.5% thought that telepathology service was effective for their treatment, and 98.3% wanted to continue this service in their community. On an average, participants saved 58% (95% CI, 53.4ā€“61.5) of cost using telepathology rather than conventional pathology service. Conclusions: Remote under-resourced communities received professional pathology services with less time-consuming and significantly lower costs using the telepathology approach. Where pathology services are absent/insufficient, telepathology is efficacious for primary diagnosis, screening, and referral through professional pathologists for the satisfactory treatment of unreached communities

    Cancer is a disorder that rigorously affects the human population worldwide. There is a steady demand for new remedies to both treat and prevent this life-threatening sickness due to toxicities, drug resistance and therapeutic failures in current conventional therapies. Researchers around the world are drawing their attention towards compounds of natural origin. For decades, human beings have been using the flora of the world as a source of cancer chemotherapeutic agents. Currently, clinically approved anticancer compounds are vincristine, vinblastine, taxanes, and podophyllotoxin, all of which come from natural sources. With the triumph of these compounds that have been developed into staple drug products for most cancer therapies, new technologies are now appearing to search for novel biomolecules with anticancer activities. Ellipticine, camptothecin, combretastatin, curcumin, homoharringtonine and others are plant derived bioactive phytocompounds with potential anticancer properties. Researchers have improved the field further through the use of advanced analytical chemistry and computational tools of analysis. The investigation of new strategies for administration such as nanotechnology may enable the development of the phytocompounds as drug products. These technologies have enhanced the anticancer potential of plant-derived drugs with the aim of site-directed drug delivery, enhanced bioavailability, and reduced toxicity. This review discusses mechanistic insights into anticancer compounds of natural origins and their structural activity relationships that make them targets for anticancer treatments
