Trajectories of Burden and Depression in Caregivers Following Traumatic Injury: The Role of Resilience
As part of an effort to understand psychological consequences among family members of patients sustaining a traumatic injury, medical research has turned to the role of resilience – or the ability to bounce back from and maintain psychological well-being in the wake of an adverse event— in mitigating the potential distress (i.e., depression and burden) of caregiving (Bonanno, 2004; Roberson et al., under review). This study sought to examine the ability for trait-resilience to predict trajectories of distress over the course of a year among 124 family members and loved ones of patients admitted to a Level I Trauma Center. A cross-lagged path model examining resilience, burden, and depression at baseline, 3, 6, and 12 months after injury showed that, while depression strongly predicted later burden, resilience was not a significant predictor of either outcome in the model. When depression and burden were subjected to a person-centered analysis (i.e., latent growth curve analysis), two major classes were identified: caregivers with high, chronic distress (33% of the sample) and low-moderate distress that declined over time. A three-class solution for caregiver burden further identified a moderate, increasing trajectory class. Predictive discriminant analyses revealed that trait-resilience was a major differentiating trait between class membership (rs = .23 for depression; rs = .32 for burden); further, presence of PTSD symptoms at baseline, gender, and history of depression were shown to be strong factors in distinguishing class membership across both outcomes. This study helps shed insight into the well-being of caregivers in the wake of a loved one's traumatic injury, in addition to possible identifying risk factors while patients are still admitted in the ICU. Lastly, the study provides alternatives for analyses that focus on longitudinal outcomes, particularly person- vs. variable-centered solutions
Invariance of the Bifactor Structure of Mild Traumatic Brain Injury (mTBI) Symptoms on the Rivermead Postconcussion Symptoms Questionnaire Across Time, Demographic Characteristics, and Clinical Groups: A TRACK-TBI Study
This study aimed to elucidate the structure of the Rivermead Postconcussion Symptoms Questionnaire (RPQ) and evaluate its longitudinal and group variance. Factor structures were developed and compared in 1,011 patients with mild traumatic brain injury (mTBI; i.e., Glasgow Coma Scale score 13-15) from the Transforming Research and Clinical Knowledge in TBI study, using RPQ data collected at 2 weeks, and 3, 6, and 12 months postinjury. A bifactor model specifying a general factor and emotional, cognitive, and visual symptom factors best represented the latent structure of the RPQ. The model evinced strict measurement invariance over time and across sex, age, race, psychiatric history, and mTBI severity groups, indicating that differences in symptom endorsement were completely accounted for by these latent dimensions. While highly unidimensional, the RPQ has multidimensional features observable through a bifactor model, which may help differentiate symptom expression patterns in the future

Satisfaction with Life after Mild Traumatic Brain Injury: A TRACK-TBI Study
Identifying the principal determinants of life satisfaction following mild TBI (mTBI) may inform efforts to improve subjective well-being in this population. We examined life satisfaction among participants in the Transforming Research and Clinical Knowledge in Traumatic Brain Injury (TRACK-TBI) study who presented with mTBI (Glasgow Coma Scale [GCS] score = 13-15;
 = 1152). An L1-regularization path algorithm was used to select optimal sets of baseline and concurrent symptom measures for prediction of scores on the Satisfaction with Life Scale (SWLS) at 2 weeks and 3, 6, and 12 months post-injury. Multi-variable linear regression models (all
 = 744-894) were then fit to evaluate associations between the empirically selected predictors and SWLS scores at each follow-up visit. Results indicated that emotional post-TBI symptoms (all b = -1.27 to -0.77, all
 < 0.05), anhedonia (all b = -1.59 to -1.08, all
 < 0.01), and pain interference (all b = -1.38 to -0.89, all
 < 0.001) contributed to the prediction of lower SWLS scores at all follow-ups. Insomnia predicted lower SWLS scores at 2 weeks, 3 months, and 6 months (all b = -1.11 to -0.83, all
s < 0.01); and negative affect predicted lower SWLS scores at 2 weeks, 3 months, and 12 months (all b = -1.38 to -0.80, all
 < 0.005). Other post-TBI symptom domains and baseline socio-demographic, injury-related, and clinical characteristics did not emerge as robust predictors of SWLS scores during the year after mTBI. Efforts to improve satisfaction with life following mTBI may benefit from a focus on the detection and treatment of affective symptoms, pain, and insomnia. The results reinforce the need for tailoring of evidence-based treatments for these conditions to maximize efficacy in patients with mTBI
Societal Participation of People With Traumatic Brain Injury Before and During the COVID-19 Pandemic: A NIDILRR Traumatic Brain Injury Model Systems Study
Objective: To examine the effect of the COVID-19 pandemic on societal participation in people with moderate-to-severe traumatic brain injury (TBI).
Design: Cross-sectional retrospective cohort.
Setting: National TBI Model Systems centers, United States.
Participants: TBI Model Systems enrollees (N=7003), ages 16 and older and 1-30 years postinjury, interviewed either prepandemic (PP) or during the pandemic (DP). The sample was primarily male (72.4%) and White (69.5%), with motor vehicle collisions as the most common cause of injury (55.1%).
Interventions: Not applicable.
Main outcome measure: The 3 subscales of the Participation Assessment with Recombined Tools-Objective: Out and About (community involvement), Productivity, and Social Relations.
Results: Out and About, but not Productivity or Social Relations, scores were appreciably lower among DP participants compared to PP participants (medium effect). Demographic and clinical characteristics showed similar patterns of association with participation domains across PP and DP. When their unique contributions were examined in regression models, age, self-identified race, education level, employment status, marital status, income level, disability severity, and life satisfaction were variably predictive of participation domains, though most effects were small or medium in size. Depression and anxiety symptom severities each showed small zero-order correlations with participation domains across PP and DP but had negligible effects in regression analyses.
Conclusions: Consistent with the effect of COVID-19 on participation levels in the general population, people with TBI reported less community involvement during the pandemic, potentially compounding existing postinjury challenges to societal integration. The pandemic does not appear to have altered patterns of association between demographic/clinical characteristics and participation. Assessing and addressing barriers to community involvement should be a priority for TBI treatment providers. Longitudinal studies of TBI that consider pandemic-related effects on participation and other societally linked outcomes will help to elucidate the potential longer-term effect the pandemic has on behavioral health in this population
