PSYCHOLOGICAL DISTRESS AND SOCIAL FACTORS IN PATIENTS WITH ASTHMA AND CHRONIC OBSTRUCTIVE LUNG DISEASE
Chronic respiratory diseases which embrace asthma and chronic obstructive pulmonary disease (COPD) are common in the
population. In a large number of cases they are diagnosed very late. Statistics of deaths, especially in the case of COPD, are
underestimated because morbidity and mortality can be affected by other comorbid conditions, for example cardiovascular disease.
Asthma and COPD impair not only physical functioning of patients but also affect their psychological state. Mood disorders and
cognitive function impairment are more often observed in this group than in the general population. It has been proven that the
coexistence of psychological dysfunction significantly worsens the functioning of patients (affects, among others, their work, family
and social lives) and has an impact on the course of the treatment of the somatic illness. First of all, patients with cognitive deficits
have greater problems in applying treatment recommendations.
Accordingly, it seems to be important to pay more attention to the problem of mental disorders in patients with obstructive lung
diseases. There is a clear need for a multidisciplinary approach that will enable prevention, early detection and effective treatment of
the psychological disorders in that group of patients
PSYCHOGENIC SPEECH DISORDER – A CASE REPORT
Background: Conversion (dissociative) disorder is a psychiatric disorder in which somatic symptoms or deficits are present in
the absence of a definable organic cause. The etiology of this disorder is not yet fully understood. The most characteristic
presentations are: pseudosensory syndromes, pseudoseizures, psychogenic movement disorders and pseudoparalysis. Psychogenic
speech disorder is a rare form of conversion (dissociative) disorder. The aim of present case study is to complete the knowledge on
this subject.
Subject and methods: The article presents a case of a fifty year old woman who developed psychogenic disorder of speech after
being degraded to a lower position at work. After excluding organic background of observed symptoms, the diagnosis of conversion
(dissociative) disorder was stated and adequate therapy was implemented, within the capabilities of the Ward.
Results: Partial remission of presented symptoms was achieved as a result of psychopharmacotherapy and psychotherapy.
Conclusions: Before stating the diagnosis of conversion (dissociative) disorder, possible somatic causes of the observed
symptoms should be excluded. Special attention should be drawn to the importance of studying the psychological and family context
of this case and the patient’s difficulty to understand and accept that produced symptoms might be triggered by a psychogenic factor
Application of metformin in type 1 diabetes mellitus

typu 2 od lat 20. XX wieku, a historia jej wykorzystania
w lecznictwie sięga średniowiecza. Na
ostatnie 20 lat przypada jej kliniczny „renesans”.
Wielokrotnie wykazywane korzystne działanie metaboliczne
metforminy w terapii cukrzycy typu 2,
w stanie przedcukrzycowym i w innych stanach insulinooporności
spowodowało, że podjęto próby
zastosowania tego preparatu w wybranych grupach
chorych na cukrzycę typu 1. Metformina, poprzez
redukcję zapotrzebowania na egzogenną insulinę,
zmniejszenie masy ciała, korzystną modyfikację profilu
lipidowego i hemostazy, a także z powodu ograniczonych
działań niepożądanych i bezpieczeństwa
terapeutycznego, może znaleźć zastosowanie również
w terapii cukrzycy typu 1. Jednak mimo wykazania
niewątpliwych korzyści metabolicznych i poprawy
kontroli cukrzycy, a tym samym - możliwości opóźniania
rozwoju przewlekłych powikłań tej choroby,
włączenie metforminy do leczenia cukrzycy typu 1 jest
jeszcze ciągle rozważane z ostrożnością. Do upowszechnienia
tego typu terapii jest konieczne przeprowadzenie
dalszych badań obejmujących większe grupy pacjentów.Metformin has been used in type 2 diabetes mellitus
since the early twenties of the 20th century, but the
history of its application goes back to the middle
ages. In the last twenty years the revival of metformin
has been observed. Its multiple-proven profitable
metabolic effect in type 2 diabetes, prediabetes and
other insulinresistant states caused the research of
usefulness of metformin in type 1 diabetes to be
conducted in chosen groups of patients with type 1
diabetes. Metformin, due to the reduction of
exogenus insulin requirement, the decrease of body
mass, profitable modification of a lipid profile and
haemostasis as well as few side effects and its
therapeutic safety, could be used in type 1 diabetes
mellitus. Although indubitable metabolic
advantages, better controlled diabetes, and the delay
of chronic diabetic complications have been show,
metformin is still used very cautiously in type 1
diabetes. To introduce a general application of
metformin in this type of therapy it is necessary to
conduct more research on a larger group of patients
ANALYSIS OF PSYCHOEMOTIONAL STATE AND INTELLECTUAL ABILITIES IN PATIENTS WITH ASTHMA AND CHRONIC OBSTRUCTIVE PULMONARY DISEASE - PRELIMINARY RESULTS
Background: In the literature we can find examples of comorbidity of the diseases of the respiratory tract and mental disorders.
Among them a particularly significant position is occupied by chronic obstructive pulmonary disease (COPD) and asthma, which
may be accompanied by anxiety, depressive and cognitive symptoms. The present research project was aimed to establish a
connection between psycho-intellectual functioning and suffering from the aforementioned diseases.
Subjects and methods: The patients were divided into 3 groups. In the asthma group there were 11 people, mean age 54, who
met the GINA criteria for asthma. The group of patients with COPD was formed by 12 people, mean age 67. The control group
included 13 people, mean age 48. Patients from all the groups underwent spirometry, sputum induction and the following tests: Mini-
Mental State Examination (MSSE), Trail Making Test (TMT A and B), Beck Depression Inventory – BDI (Beck et al. 1961) and State-
Trait Anxiety Inventory for Adults - STAI 1 and 2.
Results: In the TMT tests results were the following: We can presume some deficiency when the time required by a patient to
complete the task is longer than 78 seconds for Part A and 273 seconds for Part B. In our research the best mean time was obtained
in control group (Part A - 30.04 s, Part B – 67.37 s), then in the asthma group (Part A - 35.54 s, Part B – 98.81 s) and in the COPD
group (Part A - 42.80 s, Part B – 107.79 s). In our research study the lowest score for the Beck Depression Inventory was obtained in
the control group (mean 6.15), then in asthma (mean 9.63) and in COPD (mean 13.61). Results for State-Train Anxiety Inventory
were distributed as follows: mean score in the asthma group was 36.48 in Part 1 and 43.27 in Part 2, in the COPD group 36.41 in
Part 1 and 42.66 in Part 2 and in the control group 32.61 in Part 1 and 36.75 in Part 2.
Conclusions: In our research the level of anxiety and depression measured by self-assessment questionnaires was higher in the
study groups than in the control group. Also cognitive functions were worse than in the healthy controls, especially among COPD
patients

Ich szerokie spektrum działania obejmuje wpływ: hipolipemiczny, hipotensyjny, antyoksydacyjny, hepatoprotekcyjny oraz antyaterogenny przez co oleje te mogą być pomocne w leczeniu zaburzeń metabolicznych związanych z otyłością

Zarówno nadwaga jak i otyłość są coraz częstszym problemem spotykanym wśród pacjentów z cukrzycą typu 1, co niesie za sobą poważne konsekwencje zdrowotne. Szeroko pojęty zdrowy styl życia oraz regularny wysiłek fizyczny są nieodłącznym elementem prewencji i leczenia nadmiernej masy ciała. Leczenie żywieniowe chorych na cukrzycę bazuje na zasadach zdrowej racjonalnej diety ― powinna się opierać na odpowiednio zbilansowanych, regularnych posiłkach. Aby osiągnąć redukcję nadmiernej masy ciała, należy zastosować dietę o obniżonej ― w stosunku do zapotrzebowania ― kaloryczności połączonej z regularnym wysiłkiem fizycznym. W leczeniu nadwagi i otyłości u chorych na cukrzycę typu 1 „złotym standardem” pozostaje dieta o ograniczonej podaży węglowodanów. Niezmiernie ważne, aby produkty stosowane w codziennej diecie miały niski indeks glikemiczny i charakteryzowały się wysoką zawartością błonnika pokarmowego, gdyż wykazano, że regularne spożywanie tych produktów wiąże się między innymi z lepszym wyrównaniem metabolicznym cukrzycy, niższym stosunkiem obwodu talii do obwodu bioder (WHR) oraz mniejszą masą ciała. Co najmniej 150 minut umiarkowanego wysiłku tygodniowo fizycznego jest zalecane pacjentom z cukrzycą typu 1, jednak wielu chorych nie spełnia tych kryteriów. Najczęstszą obawą przed podjęciem aktywności fizycznej jest strach przed hipoglikemią. Niezmiernie ważna jest odpowiednia edukacja pacjenta na temat przygotowania się do podjęcia aktywności fizycznej tak, aby podczas trwania wysiłku fizycznego nie doszło do hipo- lub hiperglikemii. Is the retinol-binding protein 4 a possible risk factor for cardiovascular diseases in obesity?
Although many preventive and treatment approaches have been proposed, cardiovascular disease (CVD) remains one of the leading causes of deaths worldwide. Current epidemiological data require the specification of new causative factors, as well as the development of improved diagnostic tools to provide better cardiovascular management. Excessive accumulation of adipose tissue among patients suffering from obesity not only constitutes one of the main risk factors of CVD development but also alters adipokines. Increased attention is devoted to bioactive adipokines, which are also produced by the adipose tissue. The retinol-binding protein 4 (RBP4) has been associated with numerous CVDs and is presumably associated with an increased cardiovascular risk. With this in mind, exploring the role of RBP4, particularly among patients with obesity, could be a promising direction and could lead to better CVD prevention and management in this patient group. In our review, we summarized the current knowledge about RBP4 and its association with essential aspects of cardiovascular disease—lipid profile, intima-media thickness, atherosclerotic process, and diet. Why are western diet and western lifestyle pro-inflammatory risk factors of celiac disease?
The prevalence of celiac disease increased in recent years. In addition to the genetic and immunological factors, it appears that environmental determinants are also involved in the pathophysiology of celiac disease. Gastrointestinal infections impact the development of celiac disease. Current research does not directly confirm the protective effect of natural childbirth and breastfeeding on celiac disease. However, it seems that in genetically predisposed children, the amount of gluten introduced into the diet may have an impact on celiac disease development. Also western lifestyle, including western dietary patterns high in fat, sugar, and gliadin, potentially may increase the risk of celiac disease due to changes in intestinal microbiota, intestinal permeability, or mucosal inflammation. Further research is needed to expand the knowledge of the relationship between environmental factors and the development of celiac disease to define evidence-based preventive interventions against the development of celiac disease.
