    EFFECT OF FORMALIN VAPOURS ON PULMONARY FUNCTIONS OF MEDICAL STUDENTS IN ANATOMY DISSECTION HALL OVER A PERIOD OF ONE YEAR

    Abstract : Formalin is extensively used for preservation of cadavers in department of Anatomy. However it is a noxious chemical which vaporises at normal room temperature in the air and may cause respiratory health problems among first year medical students. The study was planned to observe the effect of formalin vapours on lung function tests of first year medical students who are exposed routinely for 2 hrs every day for 6 days per week throughout the year. Following written informed consent clinically healthy 100 medical students between age group 18-23 yrs were subjected to pulmonary function testing by computerised spirometery. The dynamic lung function tests (FVC, FEV1, FEV1%, , PEFR) were measured on four occasions-basal (before exposure), 1 month, 6 months, 11 months of exposure to formalin vapours in anatomy dissection hall. The study revealed statistically significant (P<0.0001) decreases in FVC, FEV1%, FEF , PEFR except FEV1 after 1 month of exposure to anatomy dissection hall. The decrease in all the parameters slowly reverted back towards normal basal values across 6 and 11 months and was statistically significant in all (P<0.0001) except FVC. Acute exposure to formalin vapours at anatomy dissection hall decreases the respiratory functions, however on long term exposure the body corrects the damage. Further studies are required to see the changes at cellular levels and the extent of damage to respiratory system.     Rapid qualitative review of ethical issues surrounding healthcare for pregnant women or women of reproductive age in epidemic outbreaks

    This article describes, categorizes, and discusses the results of a rapid literature review aiming to provide an overview of the ethical issues and corresponding solutions surrounding pregnancies in epidemic outbreaks. The review was commissioned by the World Health Organization to inform responses to the Zika outbreak that began in 2015. Due to the urgency of the response efforts that needed to be informed by the literature search, a rapid qualitative review of the literature published in PubMed was conducted. The search and analysis were based on the operationalization of 3 key concepts: ethics, pregnancy, and epidemic outbreak. Ethical issues and solutions were interpreted within a principlist framework. The data were analyzed using qualitative content analysis. The search identified 259 publications, of which the full text of 23 papers was read. Of those, 20 papers contained a substantive part devoted to the topic of interest and were therefore analyzed further. We clustered the ethical issues and solutions around 4 themes: uncertainty, harms, autonomy/liberty, and effectiveness. Recognition of the identified ethical issues and corresponding solutions can inform and improve response efforts, public health planning, policies, and decision-making, as well as the activities of medical staff and counselors who practice before, during, or after an epidemic outbreak that affects pregnant women or those of reproductive age. The rapid review format proved to be useful despite its limited data basis and expedited review process

    Soil health and fertility are the basis for gaining sustainable profit through higher productivity by the farmers. Using optimal doses of fertilizers and cropping pattern as per the scientific recommendations is the first step towards sustainable farming. Soil organic carbon (SOC) is the most important component in maintaining soil quality because of its role in improving physical, chemical, and biological properties of the soil. Organic matter is an important source of nitrogen, phosphorus and sulfur. Soil Health card is a Government of India's scheme promoted by the Department of Agriculture & Co-operation under the Ministry of Agriculture and Farmers' Welfare, apart from giving the health index of soil, will also indicate fertilizer recommendations and soil amendment required for the farm. Under Govt. of Haryana sanctioned Soil Health Card project, ARF carried out the fertility status study of 3000 acres of land of village Baroda Mor, block Mundlana, Tehsil Gohana, Sonipat, Haryana and distribute the cards well before the harvesting of Rabi crop with proper recommendation on dosage of appropriate fertilizer as per deficiency of essential nutrient parameter.     Ethical issues in public health surveillance: a systematic qualitative review

    Background: Public health surveillance is not ethically neutral and yet, ethics guidance and training for surveillance programmes is sparse. Development of ethics guidance should be based on comprehensive and transparently derived overviews of ethical issues and arguments. However, existing overviews on surveillance ethics are limited in scope and in how transparently they derived their results. Our objective was accordingly to provide an overview of ethical issues in public health surveillance;in addition, to list the arguments put forward with regards to arguably the most contested issue in surveillance, that is whether to obtain informed consent. Methods: Ethical issues were defined based on principlism. We assumed an ethical issue to arise in surveillance when a relevant normative principle is not adequately considered or two principles come into conflict. We searched Pubmed and Google Books for relevant publications. We analysed and synthesized the data using qualitative content analysis. Results: Our search strategy retrieved 525 references of which 83 were included in the analysis. We identified 86 distinct ethical issues arising in the different phases of the surveillance life-cycle. We further identified 20 distinct conditions that make it more or less justifiable to forego informed consent procedures. Conclusions: This is the first systematic qualitative review of ethical issues in public health surveillance resulting in a comprehensive ethics matrix that can inform guidelines, reports, strategy papers, and educational material and raise awareness among practitioners

    Developing the ethics of implementation research in health

    Implementation research (IR) is growing in recognition as an important generator of practical knowledge that can be translated into health policy. With its aim to answer questions about how to improve access to interventions that have been shown to work but have not reached many of the people who could benefit from them, IR involves a range of particular ethical considerations that have not yet been comprehensively covered in international guidelines on health research ethics. The fundamental ethical principles governing clinical research apply equally in IR, but the application of these principles may differ depending on the IR question, context, and the nature of the proposed intervention. IR questions cover a broad range of topics that focus on improving health system functioning and improving equitable and just access to effective health care interventions. As such, IR designs are flexible and often innovative, and ethical principles cannot simply be extrapolated from their applications in clinical research. Meaningful engagement with all stakeholders including communities and research participants is a fundamental ethical requirement that cuts across all study phases of IR and links most ethical concerns. Careful modification of the informed consent process may be required in IR to permit study of a needed intervention. The risks associated with IR may be difficult to anticipate and may be very context-specific. The benefits of IR may not accrue to the same groups who participate in the research, therefore justifying the risks versus benefits of IR may be ethically challenging. The expectation that knowledge generated through IR should be rapidly translated into health policy and practice necessitates up-front commitments from decision-makers to sustainability and scalability of effective interventions. Greater awareness of the particular ethical implications of the features of IR is urgently needed to facilitate optimal ethical conduct of IR and uniform ethical review

    Gene drive research is progressing towards future field evaluation of modified mosquitoes for malaria control in sub-Saharan Africa. While many literature sources and guidance point to the inadequacy of individual informed consent for any genetically modified mosquito release, including gene drive ones, (outside of epidemiological studies that might require blood samples) and at the need for a community-level decision, researchers often find themselves with no specific guidance on how that decision should be made, expressed and by whom. Target Malaria, the Kenya Medical Research Institute and the Pan African Mosquito Control Association co-organised a workshop with researchers and practitioners on this topic to question the model proposed by Target Malaria in its research so far that involved the release of genetically modified sterile male mosquitoes and how this could be adapted to future studies involving gene drive mosquito releases for them to offer reflections about potential best practices. This paper shares the outcomes of that workshop and highlights the remaining topics for discussion before a comprehensive model can be designed

    Single Neuron Imaging Reveals Metabotropic Glutamate Receptor-Mediated Bursting and Delay in Calcium Oscillation in Hippocampal Neurons

    Measurement of cytosolic calcium activity in single neuron and quantification of response to drug treatment over a longer period remain challenging. Especially, analysis of amplitude, frequency, and delay in calcium response in G-protein coupled receptors (GPCR) targeting drug-treated neurons is complicated due to inherent heterogeneity. To address this, we have utilized k- means clustering to identify various sub-populations. Here we focus on measuring drug dose response in the single neuron by means of high-resolution confocal imaging. Metabotropic glutamate receptor was activated using 3,5-dihydroxyphenylglycine (DHPG). The result reveals an unusual delay in calcium response for higher dose particularly in responding neurons. The proposed methodology can be used for selection of drug dose and optimization of the time window for neurodegenerative diseases
