Клинический случай успешного лечения тяжелого рефрактерного подагрического артрита
Цель данной работы — демонстрация дополнительных возможностей лечения тяжелого рефрактерного
подагрического артрита, для чего мы приводим клинический случай.
Пациент находился на стационарном лечении в терапевтическом отделении Центра реконструктивной
и восстановительной медицины (университетская клиника) Одесского национального
медицинского университета по поводу тяжелого рефрактерного подагрического артрита,
полиартрита с поражением обоих голеностопных суставов, лучезапястных суставов, мелких суставов
кистей и стоп.
После неудачных попыток лечения пациента «стандартными» методами, мы успешно применили
пульс-терапию метилпреднизолоном.
Лечение подагрического артрита остается сложной задачей, решение которой требует комплексного
подхода с учетом многих факторов, в том числе индивидуальной реакции на назначаемую
терапию. Пульс-терапия метилпреднизолоном может быть эффективной в лечении тяжелого
острого рефрактерного подагрического артрита.The purpose of this work was to demonstrate additional treatment options for severe refractory
gouty arthritis.
To achieve this goal we present a clinical case. The patient was on inpatient
treatment at the Therapeutic Department of the Center of Reconstructive and Restorative Medicine
(University Clinic) of the Odessa National Medical University due to severe refractory gouty arthritis,
polyarthritis with injury of both ankle joints, wrist and small joints of the hands and feet.
In order to treat the acute attack of gouty arthritis in this patient, we consistently used:
colchicine, different non-steroidal anti-inflammatory drugs, glucocorticosteroids in mild-to-moderate
doses and different analgetics. After unsuccessful attempts to treat the patient with standard methods,
we successfully applied pulsed therapy with methylprednisolone. Treatment of gouty arthritis remains a complex task, the solution of which requires
an integrated approach taking into account many factors, including individual response to the prescribed
therapy. Pulse therapy with methylprednisolone can be effective in treating severe acute refractory
gouty arthritis
Комплексное обследование и лечение пациента с инфарктом миокарда IV типа с использованием высокочувствительного экспресс-теста на тропонин І
Исследование уровней сердечного тропонина служит «золотым стандартом» для диагностики
острого коронарного синдрома и инфаркта миокарда.
Цель исследования — оценить возможность сокращения времени диагностики острого поражения
миокарда за счет использования экспресс-теста на тропонин І производства ООО «Виробнича
фірма Сіместа». Для сравнения разницы в динамике результатов экспресс-тестов на
тропонин І разными методиками приведен клинический случай пациента с острым инфарктом
миокарда. Использование высокочувствительного теста на тропонин І производства ООО «Виробнича
фірма Сіместа» может быть перспективным в качестве метода ранней диагностики инфаркта
миокарда для уменьшение времени до проведения процедуры реваскуляризации.The measurement of cardiac troponin levels is a gold standard of the diagnosis of acute coronary
syndrome and myocardial infarction. Qualitative tests of troponins begin to implement in clinical practices
in Ukraine. Unfortunately there is a lack of standardization of monoclonal antibodies on which
the quantitative determination of troponins are based, item the tests of different manufacturers have
various absolute values of concentration, sensitivity, specificity and significant levels for diagnosis.
Furthermore, immune enzyme analyzers of different manufacturers are used in a clinic. Such situation
complicates the clinician’s decision for earlier diagnosis of acute coronary syndrome and estimation
of time-dependent dynamics of the troponin levels.
The aim of the study was to evaluate the possibility of reducing the time of diagnosis of acute
myocardial injury by using the express test for troponin І produced by LLC “Virobnicha firma Simesta”,
Ukraine.
To compare the differences in time-dependent dynamics of two differ express tests of the troponin І,
the clinical case of the patient with acute myocardial infarction of the IV type is demonstrated.
The using of Troponin I high-sensitivity rapid assay produced by LLC "Virobnicha firma Simesta",
Ukraine, may have some perspectives as a method of earlier diagnosis of myocardial infarction to
reduce the time prior to revascularization procedure
Features of Phase Formation in the CsOH–H<sub>2</sub>SO<sub>4</sub>–H<sub>3</sub>PO<sub>4</sub>–H<sub>2</sub>O System and the Growth of the Cs<sub>6</sub>(SO<sub>4</sub>)<sub>3</sub>(H<sub>3</sub>PO<sub>4</sub>)<sub>4</sub> Crystals
In this paper, the conditions of phase formation in the system CsOH–H2SO4–H3PO4–H2O are considered for the first time. The phase formation of Cs6(SO4)3(H3PO4)4 at t = 50 °C has been studied extensively. The main concentration boundary conditions for this compound are considered for the first time. The solubility congruence of Cs6(SO4)3(H3PO4)4 is shown. Conditions and approaches for obtaining crystals by isothermal evaporation and saturated solution temperature reduction methods are considered. The results of obtaining Cs6(SO4)3(H3PO4)4 crystals with maximum dimensions of ~20 mm are presented
Implementation of Phase Transitions in Rb<sub>3</sub>H(SO<sub>4</sub>)<sub>2</sub> under K Substitution
A series of solid acid compounds, representing the large family MmHn(AO4)(m + n)/2·yH2O (where M = K, Rb, Cs, NH4; AO4 = SO4, SeO4, HPO4, HAsO4), is characterized by high values of own proton conductivity, which arises as a result of a phase transition through the formation of a dynamically disordered hydrogen bond network. Such superprotonic phase transitions are observed, however, not for all compounds of the family and Rb3H(SO4)2 is one of them. The occurrence of superprotonic phase transitions has been experimentally demonstrated in the (KxRb1−x)3H(SO4)2 solid solutions through cation substitution. The high-temperature phases are unstable towards decomposition reaction, and their temperature range of existence is about 1–7 °C. The implementation of superprotonic transitions is discussed in terms of hydrogen bond lengths
