    Novel methodological approaches in the system of early diagnosis of mastopathy with the use of computer dermography

    Aim. Optimization of the early detection of mastopathy on screening examinations due to the inclusion of data received with the use of computer dermography into the algorithm of complex diagnosis. Methods. Integral evaluation of the health state was performed with the use of computer dermography as part of the diagnostic complex DgKTD-01. Fertile females (n=107) were divided into three groups. The study group (n=36) included the female patients with differentiated mastopathy. The control group (n=51) united the females complaining of discomfort in breasts, engorgement and tenderness. Parameters of the baseline level in the study group and the control group were studied; a prognostic model of mastopathy was developed. Comparative analysis of identified characteristics was performed in the comparison group. Results. Females from the study group are in the group of adaptation risk with formation of premorbid states and diseases. The prognostic model of mastopathy was consistent disorder of the tonic activity of basic functions F2' and F3', functions F5-1', F5-2', F5-3' with the extremum in segments Th4-6 in regimen 2 which allowed determining females with the risk of the development of this pathology in comparison group. Conclusion. Method of computer dermography serves as a methodological approach to revealing pre-nosological states which alllows using it as an additional non-invasive diagnostic screening test to determine mastopathy risk groups; the obtained results give ground to continue the further studies and improvement of the diagnosis on a large clinical material and to development of computed system for interpretation of the obtained data

    Diagnostic value of structural and functional changes in the carotid and vertebral arteries in arterial hypertension, associated with benign paroxysmal positional vertigo

    Objective of the study was to assess the diagnostic significance of hemodynamic disorders in the area of carotid and vertebral arteries in patients with hypertension combined with benign paroxysmal positional vertigo (DPPG) using ultrasound duplex scanning of neck vessels.Material and methods. Thirty eight patients of the clinic of Federal Research Center for Fundamental and Translational Medicine with hypertension and concomitant dizziness were examined. The state of carotid and vertebral arteries, blood flow in the arteries were assessed by duplex ultrasound scanning using the Vivid E9 apparatus.Results and discussion. The influence of the blood flow at the level of carotid and vertebral arteries on the severity of clinical symptoms of dizziness in patients with hypertension with concomitant DPPG was shown. The clinical significance of atherosclerotic changes in carotid arteries, tortuosity of carotid and vertebral arteries of the effectiveness of treatment and prevention of dizziness in patients with hypertension and DPPG was revealed. It is shown that in patients with hypertension and comorbid DPP, the severity of the relationship of hemodynamically significant changes in the carotid and vertebral arteries with clinical manifestations of dizziness is greater than in patients with hypertension without DPP. The use of ultrasound duplex scanning of brachiocephalic arteries in hypertension with comorbid vertigo allows for differential diagnosis of benign paroxysmal positional vertigo and cerebrovascular dyscirculatory disorders, the degree of their participation in clinical manifestations of dizziness. A new medical technology for the diagnosis of pathogenic mechanisms of dizziness in patients with hypertension, using ultrasound duplex scanning of carotid and vertebral arteries, allows to personalize treatment and prevention in such patients

    Influence of fallow and tillage types on the indicators of fertility and productivity of winter rye in the long-term experience

    Long-term use of moldboardless tillage has reduced the yield of winter rye by 0.66 t/ha compared with moldboard tillage (2.77 t/ha) in a field experiment (the beginning of the second rotation). In the lower part of the arable layer (10-20 cm) nitrification capacity decreased by 0.76 mg / kg, exchangeable potassium content - by 44 mg/kg, soil moisture - by 3.2%, bulk density increased up to 1.29 g/cm3, the density of productive stalks decreased by 22.1%, weed infestation increased by 2.3 times. The combined system (plowing in the fallow after four years of moldboardless treatment) at fertility indicators equal to moldboard system has reduced grain yield by 0.27 t/ha due to the thinness of productive stalks by 10% and increase the air-dry mass of weed component in the biocenosis by 12%. Entering of manure into fallow and a clover green-manure fallow have provided the greatest yield of winter rye - 2.59 and 2.61 t/ha for the whole experiment at the expense of increasing of the soil nitrification capacity by 36 and 83%, larger content of exchangeable potassium by 52 and 29%, increase plant density by 35 and 11 pcs./m2. In the control without fertilizers productivity was 2.42 t/ha. At manure plowing the yield of winter rye grain was 3.17-2.69 t/ha; at surface sealing of manure, its effectiveness has decreased by 33-57% (2.02 t/ha) due to highest weed infestation - 215.8 g/m2. Reduce of rye yields after mustard green manure fallow up to 2.22 t/ha was due to the thinness of productive stalks. In the sum of indicators, the cultivation of winter rye is most effective after clover green manure fallow on background of moldboard tillage and spring fertilizing with ammonium nitrate. Thus the yield was 2.98 t/ha, the energy efficiency ratio = 2.53

Lists of vital and essential medicines, «preferential» list of drugs prescribed by a decision of medical commissions, the list of expensive drugs and the list of drugs constituting minimum pharmaceutical assortment are important documents for the pharmaceutical industry and for society as a whole, since the prices of drugs included in these lists are regulated by goverment.
