    Results of chalazion cryosurgery with an increased risk of complications

    Background. At an appointment with an ophthalmologist, patients with pathological conditions of the eyelids are often dissatisfi ed with the effectiveness of the traditional surgical treatment.The aim: assessment of the functional state of the eye adnexa after cryodestruction of the chalazion with an increased risk of complications using modern cryosurgical equipment.Material and methods. Clinical studies were carried out in 254 patients (277 eyes) with chalazion, including a complicated course of the disease, with cryodestruction of the chalazion using an autonomous cryoapplicator made of porous-permeable titanium nickelide.Results. Cryosystems of the new generation, in comparison with the well-known industrial cryoapparatus, diff er in new properties. They can signifi cantly improve the quality of cryotherapy in surgery due to a clearer localization of cryotherapy and a high rate of heat removal from the surface of altered tissues without damaging the surrounding tissues. Depending on the size of the pathological formation, the duration of the exposure, the frequency of repetitions of the applications during the session, the regression of the chalazion occurred within 1–1.5 months with the preservation of the integrity of the intermarginal space and the functional state of the eyelid.Conclusion. Analysis of the data obtained indicates a high clinical and cosmetic effi ciency of cryosurgery of the eyelid chalazion using an autonomous cryoapplicator made of porous-permeable titanium nickelide

    We've investigate ultrastructural manifestations of cryoapplicator made from a porous and permeable titan nikelid post-traumatic influence on ciliary body tissues in a rabbit's eye wall. During the experiment on 25 adult rabbits 3, 7, 14 and 30 days after local cryoinfluence eye wall fragments taken from experimental and control animals passed the corresponding procedures for electron microscopy. Already received factogrammes primary viewing convinces in specificity of cryoinfluence effect and structural stromal and a ciliary body elements' remodeling. Thus pinpoint morphological analysis of perinecrotic and remote zones of a stroma and ciliary body cryoapplication testifies more expressive their structural integrity, in comparison with usual mechanical damage consequences by the end of the first supervision week.     Experimental estimation of pro- and anti-inflammatory cytokines balance in aqueous eye chamber liquid in case of intraocular implantation of drainage material made of nickelid titanium and intraocular application of a solid retention suture TH-10. Investigation of aqueous eye chamber liquid sampled on the 3rd, 7th and 30th days and in 3 months and in 1,5 year after intraocular implantation with nickelid titanium TH-10 used as a drainage and fixative device have been studied during experiment with 26 mature rabbits. The control group consisted of 6 intact rabbits as well as of 5 rabbits for whom perforating hypotensive surgical interference without drainage device implantation have been administered in various periods of monitoring. The opposite nature of cytokines balance changes and proportion of its pro- and anti-inflammatory element in the dynamics after intraocular implantation of a nickelid titanium and solid suture TH-10, which shows positive anti-inflammatory influence of materials made of nickelid titanium basis, that is manifested in significant reduction of anti-inflammatory component of cytokines balance and shows its maximum in the utmost tardy periods of monitoring, as well in clinically unresponsive postoperative course with animals of experimental group compared to the group of comparison

    Comparative assessment of cytokine profile status in aqueous chamber moisture with experimental rabbits while use of traditional suture material and nickelid titanium monolithic fibers with penetrating wounds of eye cornea has been done. It has been approved within the course of examinations (3, 7, 30 days) that use of conventional suture material does not influence on FNOα concentration; concentration of IL1β is certainly increases from the 3rd day of examinations and remains unchanged up to the end of monitoring; IL2 concentration probably significant increases to the 30th day of examination and concentration of IL10 increased in early period (on the 3rd day), is declining to the same level with noninvolved animals. in any period of examinations after suturing of eye cornea with nickelid titanium monolithic fibers of rabbits in experimental group, level of IL2, IL10, FNOα was fixed on the reference value level, except of obvious reduction of the latter (IL10, FNOα) in early periods of monitoring (3rd day). Herewith, certainly increased IL1β concentration that was studied dynamically, was less expressed compared to that appeared while use of conventional suture materials with postoperative rabbits. Analysis of the obtained data generally grounds advisability and preferable ness of use of suture material of nickelid titanium in cases of penetrating eye cornea wounds that has been confirmed by clinical data in this experiment

    Perspectives of using a microsurgical device made of titanium nickelide in complicated cataract surgery

    Purpose. To examine the possibility of using a tool with variable shape geometry of the operational section of titanium nickelide for small incision surgery to remove complicated cataract.Material and methods. Cadaveric eyes of 7 patients with complicated cataract were used for the study. Nickel titanium alloy was selected as the material for lens destruction, as it meets the medical requirements for design solutions of this class of surgical tools in cycles of their deformation in active and transportation conditions. The instrument operational mechanism is shaped as a loop of superelastic thin thread of titanium nickelide with a diameter of 60 microns with a porous surface coating, is used for mechanical fragmentation of the lens nucleus in complicated cataracts.Results. The size of tissue incision in the access zone is reduced. There is a decrease in capsular bag mobility at the stages of nucleus lens fragmentation. The risk of complications caused by a negative energy and other impacts on eye structures related to ultrasound fragmentation of lens nucleus in complicated cataracts is reduced.Conclusion. Use of such opportunities as material superelasticity, plasticity and strength, and its cutting properties when changing the shape of operational part of titanium nickelide micro-tool contributes to improving the intervention efficiency under complicated cataracts in small incision surgery

    Level of local cytokine production in the event of nickelid titanium suture material use in experimental surgical hypotension eye operation

    Comparative assessment of cytokine moisture in aqueous chamber with experimental rabbits while use of nickelid titanium suture and in case of traditional suture material in hypotension operations. During the experiment, moisture in aqueous chamber has been studied with 10 adult rabbits, the samples were taken on 3rd, 7th and 30th days and in 3 months after hypotension surgery administered with nickelid titanium suture used as a retention suture. Control group consisted of 5 rabbits, for whom hypotension surgery has been performed with traditional suture material and of 6 intact rabbits. Amount of cytokines in biological fluid (aqueous chamber moisture - ACM) of the animals has been investigated in with enzyme immunodetection method to identify content of cytokines (IL-1β, TNF-α, IL-2, IL-10). The adverse nature of cytokines balance change and quotient of its pro - and anti-inflammatory components in the dynamics after intraocular implantation with TH-10 suture, that indicates positive anti-inflammatory influence of retention suture of nickelid titanium and becomes evident in significant reduction of anti-inflammatory component in cytokine balance, that develops in the farthermost monitoring terms, as well as in clinically unresponsive post-operative course with animals of test group compared to comparison group

    LEVEL OF LOCAL PRODUCTION OF CYTOKINES IN THE CLINIC OF TRADITIONAL TREATMENT OF DEMODECTIC BLEPHARITIS AND IN CONDITIONS OF LID CRYOTHERAPY USE

    Aim. A comparative cytokine profile assessment of the lacrimal fluid of the eyes of patients in the clinic of traditional treatment of demodectic blepharitis, and using the course of cryotherapy of the eyelids by an autonomous cryoapplicator made of titanium nickelide. Materials and methods. Clinical studies were conducted in 65 patients with demodectic blepharoconjunctivitis (DBC). A tear of the eye was taken on the 3rd, 7th and 30th days after the traditional treatment, and using cryotherapy of the eyelids by an autonomous cryoapplicator made of porous permeable titanium nickelide cooled in liquid nitrogen. In the studied biological fluid (tear) of patients with DBC, the content of cytokines (IL1β, IL8, IFNα, IL2, IL10) was evaluated by an enzyme immunoassay. Results. The opposite nature of the changes in the balance of cytokines and the ratio of its pro and anti-inflammatory links in the time of observation was revealed. A significant dynamic increase of IFNa and IL2 content in the lacrimal fluid of the examined after cryostimulation can evidence for an adequate activation of cellular immune link under its interaction with Demodex mite antigens, as well as for reinforcing the regeneration mechanisms. Conclusion. The analysis of obtained data evidences for the high efficiency of clinical and immunological effect of lid cryotherapy method for DBC treatment
