A retirada do nervo sural para utilização como enxerto autólogo em cirurgias de nervos em geral produz sintomas de repercussão clínica pouco intensa e de duração fugaz. Raramente este procedimento leva a formação de neuroma sintomático no coto proximal. Os sintomas deste tipo de complicação frequentemente cessam após o tratamento clínico e o tratamento cirúrgico é reservado para os raros casos nos quais houve falha terapêutica. Neste estudo, apresentamos o caso de um paciente que foi submetido a tratamento cirúrgico desta patologia, com a utilização de uma variação da anastomose centro-central, descrita para o tratamento de neuromas de cotos de amputação. A utilização deste tratamento resultou na remissão da sintomatologia dolorosa. São discutidas as diversas opções de tratamento cirúrgico para essa rara entidade.<br>The harvesting of the sural nerve for autologous grafting usually produces symptoms of low intensity and short duration. In rare occasions that procedure may lead to the formation of a symptomatic neuroma in the proximal stump. The symptons of this complication are usually controlled by clinical treatment and the surgical procedure is left for the therapeutic failures. In this paper we present the case of a patient with a sural nerve neuroma submitted to surgical treatment by a variant of the centro-central anastomosis technique, developed for the treatment of amputation neuromas, that resulted in remission of the painful symptomatology. The different options of surgical treatment for this rare entity are discussed
Cisto sinovial intraneural do nervo fibular: relato de caso Intraneural synovial cyst of the peroneal nerve: case report
Os autores apresentam um caso de um cisto sinovial intraneural ocorrendo no nervo fibular comum. O paciente apresentou-se com quadro de dor e dificuldade para a flexão dorsal e eversão do pé sendo submetido a exploração cirúrgica com exérese do cisto. Neste estudo são discutidos a patogênese, manifestações clínicas e tratamento desta rara lesão.<br>The authors report the case of an intraneural synovial cyst of the peroneal nerve. The initial symptom was pain and weakness of dorsiflexion and eversion of the right foot. The patient was operated on and the cyst was totally resected. The authors discuss the pathogenesis, clinical diagnosis as well as the treatment of that uncommon lesion
Organocatalysts for enantioselective synthesis of fine chemicals: definitions, trends and developments
Organocatalysis, that is the use of small organic molecules to catalyse organic transformations, has been included among the most successful concepts in asymmetric catalysis and it has been used for the enantioselective construction of C-C, C-N, C-O, C-S, C-P, and C-halide bonds. Since the seminal works in early 2000, the scientific community has been paying an ever-growing attention to the use of organocatalysts for the synthesis, with high yields and remarkable stereoselectivities, of optically active fine chemicals of interest for the pharmaceutical industry.
A brief overview is here presented about the two main classes of substrate activation by the catalyst: covalent organocatalysis and non-covalent organocatalysis, with a more stringent focus on some recent outcomes in the field of the latter and of hydrogen-bond-based catalysis. Finally, some successful examples of heterogenisation of organocatalysts are also discussed, in the view of a potential industrial exploitation
