Collinear cluster tripartition as sequential binary fission in the U(n,f) reaction
The mechanism leading to the formation of the observed products of the
collinear cluster tripartition is carried out within the framework of the model
based on the dinuclear system concept. The yield of fission products is
calculated using the statistical model based on the driving potentials for the
fissionable system. The minima of potential energy of the decaying system
correspond to the charge numbers of the products which are produced with large
probabilities in the sequential fission (partial case of the collinear cluster
tripartition) of the compound nucleus. The realization of this mechanism
supposes the asymmetric fission channel as the first stage of sequential
mechanism. It is shown that only the use of the driving potential calculated by
the binding energies with the shell correction allows us to explain the yield
of the true ternary fission products. The theoretical model is applied to
research collinear cluster tripartition in the reaction U(n,f). Calculations showed that in the first stage of this fission reaction,
the isotopes Ge and Nd are formed with relatively large
probabilities and in the second stage of sequential fission of the isotope Nd
mainly Ni and Ge are formed. This is in agreement with the yield of the isotope
Ni which is observed as the product of the collinear cluster
tripartition in the experiment.Comment: 20 pages, 9 figure
Эндоваскулярная стентовая тромбэктомия у пациентов с оcтрым тромбозом артерий нижних конечностей на фоне COVID-19
AIM Analysis of the outcomes of endovascular stent thrombectomy in patients with acute arterial thrombosis of the lower extremities on the background of COVID-19.MATERIAL AND METHODS This retrospective study for the period from January 1, 2020 to March 1, 2022 included 34 patients with acute lower limb ischemia who were diagnosed with the novel coronavirus infection SARS-COV-2. Endovascular stent thrombectomy was performed according to the standard technique using a Destination 8F guiding sheath (Terumo), an Advantage 0.014`` guidewire (Terumo), and a Casper stent (Microvention, Terumo) as a stent retriever. In case of fragmentation of thrombotic masses in the guide sheath, manual aspiration of thrombi was performed using a standard 50,0 ml syringe. Self-expanding nitinol stents were implanted in 11 clinical cases.RESULTS Intraoperative bleeding from the puncture site of the artery developed in 14.7% of cases, which required additional manipulation to achieve hemostasis. Every tenth (11.8%) patient developed myocardial infarction, in 2.9% of cases — ischemic stroke. In the hospital postoperative period during the first hours after surgery, 26.5% of patients developed rethrombosis which required re-intervention. In 8.8% of cases, retrombectomy was unsuccessful, and limb amputation was performed. A fatal outcome occurred in 67.6% of cases, which was due to an increase in multiple organ failure and the development of sepsis.CONCLUSION Endovascular stent thrombectomy is characterized by a low risk of rethrombosis and amputation in the context of COVID-19.ЦЕЛЬ Анализ результатов эндоваскулярной стентовой тромбэктомии у пациентов с острым тромбозом артерий нижних конечностей на фоне COVID-19.МАТЕРИАЛ И МЕТОДЫ В настоящее ретроспективное исследование за период с 1 января 2020 года по 1 марта 2022 года вошли 34 пациента с острой ишемией нижних конечностей, у которых была диагностирована новая коронавирусная инфекция SARS-COV-2. Эндоваскулярную тромбэкстракцию выполняли по стандартной методике с применением проводникового катетера Destination 8F (Terumo), проводника Advantage 0,014” (Terumo) и стента Casper (Microvention, Terumo), используемого в качестве стент-ретривера. При фрагментации тромботических масс в проводниковом катетере производили мануальную аспирацию тромбов при помощи стандартного 50,0 мл шприца. В 11 клинических наблюдениях произведена имплантация нитиноловых саморасширяющихся стентов.РЕЗУЛЬТАТЫ Интраоперационно в 14,7% случаев развилось кровотечение из места пункции артерии, что потребовало дополнительной манипуляции для достижения гемостаза. У каждого десятого (11,8%) развился инфаркт миокарда, в 2,9% случаев — ишемический инсульт. В госпитальном послеоперационном периоде в течение первых часов после операции у 26,5% пациентов развился ретромбоз, потребовавший повторного вмешательства. В 8,8% случаев ретромбэктомия оказалась неуспешной, была выполнена ампутация конечности. Смертельный исход развился в 67,6% случаев, что было обусловлено нарастанием полиорганной недостаточности, развитием сепсиса.ЗАКЛЮЧЕНИЕ Эндоваскулярная стентовая тромбэктомия характеризуется низким риском развития ретромбоза и ампутации в условиях COVID-19
