    Killing two birds with one stone. Is the COVID-19 vaccination campaign an opportunity to improve adherence to cancer screening programmes? The challenge of a pilot project in a large local health authority in Rome

    The COVID-19 pandemic has affected health services worldwide. The suspension of cancer screening programs during the lockdown period, coupled with the other measures taken to limit the SARS-CoV-2 spread, contributed to the idea that cancer preventive interventions are deferrable. In this opinion paper, we present some data on cancer screening coverage in one of the largest Local Health Authorities in Italy in recent years. Within this context, we introduce the benefits of a pilot project in which we took advantage of the great attention on the COVID-19 vaccination campaign to improve screening uptake. In this project, we offered men and women eligible for cancer screening the opportunity to book appointments while waiting to be vaccinated. In addition, trained healthcare personnel were available on-site to discuss any barriers to participation with the attendees. Despite the project having only just started, preliminary results are encouraging, with positive feedback from the attendees. In conclusion, we advocate for the need to adopt a comprehensive approach when it comes to population health, and we use this project as an example to discuss how it is possible to contribute to minimizing the long-term impact of the COVID-19 pandemic with resources already in place

    The use of mobile phone while driving: Behavior and determinant analysis in one of the largest metropolitan area of Italy

    The use of mobile phones while driving is one of the main causes of road accidents and it is a phenomenon in continuous growth. The key aim of this study is to analyse simultaneously knowledge, attitudes, and behavior toward the use of mobile phones while driving in one of the largest and populous metropolitan areas of Italy, Naples. The data acquired from 774 questionnaires - administered to subjects evenly divided by gender and with an average age of 39 years - revealed that 69 % have used their mobile phone while driving at least once in their lifetime. Among those who used the phone, 63.6 % use it to make phone calls while 75.2 % only to answer them; 49.1 % read messages and only 33.3 % write them. It is also notable that 34.1 % do not stop to answer a call and only 10 % do not value the use of headsets while driving as fundamental. The results indicate that cell phone usage while driving is common in the study population, despite many having university-level education and satisfactory risks awareness. The multiple linear regression analysis shows how knowledge is not correlated to the behavior held. On the contrary, attitudes are strongly correlated to knowledge and behavior, meaning that good attitudes bring forth positive behavior. According to the collected data and statistical analysis, it is possible to identify factors that can greatly affect the use of mobile phone while driving and establish targeted prevention programs

    Proactive case investigation and contact tracing, testing, home isolation, in parallel with systematic home care for patients with mild symptoms have served as cornerstones of a territorial response strategy, where an established network of public health and primary care services existed. The Italian National Health Service (NHS) entrusts many management competences to the Regions, with the result of a different management of the Covid-19 epidemic between the Regions and sometimes even among the Local Health Authorities (LHAs - in Italian: Aziende Sanitarie Locali). In Italy, the Department of Prevention (DP) plays a pivotal role in structuring the collaboration between territorial health services, hospitals and all the network of the NHS. DP is the technical structure of LHA, with the task of promoting health, preventing the onset of disease and improving the quality of life of the population. The Covid-19 pandemic has had a considerable impact on the DPs, which have necessarily had to reorganize their services in a very short time in order to provide an appropriate response to the renewed health concerns

    Estimation of polycyclic aromatic hydrocarbons pollution in mediterranean sea from volturno river, southern italy: Distribution, risk assessment and loads

    This study reports the data on the contamination caused by polycyclic aromatic hydrocarbons (PAHs) drained from the Volturno River. The seasonal and spatial distribution of PAHs in water and sediment samples was assessed. The 16 PAHs were determined in the water dissolved phase (DP), suspended particulate matter (SPM), and sediments. A multidimensional statistical approach was used to identify three pollution composite indicators. Contaminant discharges of PAHs into the sea were calculated in about 3158.2 kg/year. Total concentrations of PAHs varied in ranges 434.8 to 872.1 ng g−1 and 256.7 to 1686.3 ng L−1 in sediment samples and in water (DP + SPM), respectively. The statistical results indicated that the PAHs mainly had a pyrolytic source. Considering the sediment quality guidelines (SQGs), the water environmental quality standards (USEPA EQS), and risk quotient (RQ), the Volturno River would be considered as an area in which the environmental integrity is possibly at risk

    Trans-generational epigenetic regulation associated with the amelioration of Duchenne Muscular Dystrophy

    Exon skipping is an effective strategy for the treatment of many Duchenne Muscular Dystrophy (DMD) mutations. Natural exon skipping observed in several DMD cases can help in identifying novel therapeutic tools. Here, we show a DMD study case where the lack of a splicing factor (Celf2a), which results in exon skipping and dystrophin rescue, is due to a maternally inherited trans-generational epigenetic silencing. We found that the study case and his mother express a repressive long non-coding RNA, DUXAP8, whose presence correlates with silencing of the Celf2a coding region. We also demonstrate that DUXAP8 expression is lost upon cell reprogramming and that, upon induction of iPSCs into myoblasts, Celf2a expression is recovered leading to the loss of exon skipping and loss of dystrophin synthesis. Finally, CRISPR/Cas9 inactivation of the splicing factor Celf2a was proven to ameliorate the pathological state in other DMD backgrounds establishing Celf2a ablation or inactivation as a novel therapeutic approach for the treatment of Duchenne Muscular Dystrophy

    Conceptual Study of a Thermal Storage Module for Solar Power Plants with Parabolic Trough Concentrators

    The thermal storage technology (TSE) has a relevant strategic importance for the success of solar plants devoted to electric energy and heat production. The major benefits in the use of storage include higher efficiency and reduction in the mean levelled cost of the electric energy unit (LEC). Sensible heat storage systems within solid media have been identified, both technically and economically, as a very promising solution. The development of such a storage technology, adopting concrete, could reduce the specific cost to less than 20\u20ac per kWh of thermal capacity; additionally, such a solution is suitable for small-medium size plants with a power ranging from 1 MW to 5 MW, to be easily introduced in the Italian territory and with reduced operational and maintenance needs. In large size CSP systems, as the ARCHIMEDE plant built by ENEL with ENEA technology, a high temperature fluid storage (between 400 and 500\ub0C) is required. Such a temperature seems at present not adequate to allow for adopting concrete, whereas the production of concrete able to sustain 250-300\ub0C appears as a reachable objective. It is supposed to study a storage system characterised by a parallelepiped structure with appropriate section, selfbearing and supported on its major axis, as well as by a piping system directing the thermovector fluid within the cemented matrix

    Applications of Monte Carlo methods to special radiotherapeutic techniques

    Monte Carlo (MC) methods are considered one of the most powerful and precise approaches to study and solve medical physics issues. They, indeed, can be applied in all the situations where to use deterministic algorithms is infeasible or impossible. Surprising improvements in computer technology have promoted a wide diffusion of this technique, giving rise to the born of several Monte Carlo codes, such as the GEANT4 toolkit. In this paper we show some of the applications we developed using GEANT4. In particular, the simulation of two different radiotherapy techniques, such as proton/ion therapy and stereotactic radiosurgery will be discussed. In the first case we show the main features of our last public version of the GEANT4 Hadrontherapy program, also discussing the issues related to the nuclear fragmentation. In the second case, we show the procedures followed for the simulation of a Gamma Knife device, in order to validate the Treatment Planning System (TPS) used for the dose computation

    Environmental Monitoring of Legionella in Hospitals in the Campania Region: A 5-Year Study

    : Legionella is a pathogen that colonizes soils, freshwater, and building water systems. People who are most affected are those with immunodeficiencies, so it is necessary to monitor its presence in hospitals. The purpose of this study was to evaluate the presence of Legionella in water samples collected from hospitals in the Campania region, Southern Italy. A total of 3365 water samples were collected from January 2018 to December 2022 twice a year in hospital wards from taps and showers, tank bottoms, and air-treatment units. Microbiological analysis was conducted in accordance with the UNI EN ISO 11731:2017, and the correlations between the presence of Legionella and water temperature and residual chlorine were investigated. In total, 708 samples (21.0%) tested positive. The most represented species was L. pneumophila 2-14 (70.9%). The serogroups isolated were 1 (27.7%), 6 (24.5%), 8 (23.3%), 3 (18.9%), 5 (3.1%), and 10 (1.1%). Non-pneumophila Legionella spp. represented 1.4% of the total. Regarding temperature, the majority of Legionella positive samples were found in the temperature range of 26.0-40.9 °C. An influence of residual chlorine on the presence of the bacterium was observed, confirming that chlorine disinfection is effective for controlling contamination. The positivity for serogroups other than serogroup 1 suggested the need to continue environmental monitoring of Legionella and to focus on the clinical diagnosis of other serogroups

    The fluorescence spectrum resulting from laser excitation of the A^{2}\Pi_{1/2} - X^{2}\Sigma^{+} (0,0) band of ytterbium monofluoride, YbF, has been recorded and analyzed to determine the Franck-Condon factors. The measured values are compared with those predicted from Rydberg-Klein-Rees (RKR) potential energy curves. From the fluorescence decay curve the radiative lifetime of the A^{2}\Pi_{1/2} state is measured to be 28\pm2 ns, and the corresponding transition dipole moment is 4.39\pm0.16 D.     Rs12979860 and rs8099917 single nucleotide polymorphisms of interleukin-28B gene: simultaneous genotyping in Caucasian patients infected with hepatitis C virus

    Introduction. Recent studies have demonstrated the role of the interleukin 28B (IL28B) polymorphisms in predicting treatment induced and spontaneous clearance from Hepatitis C virus (HCV) infection, suggesting the possibility of tailored therapy in HCV infected patients. Genome-wide association studies have shown that single nucleotide polymorphisms (SNPs) near IL 28B gene on chromosome 19 are strong predictors of sustained virologic response (SVR) to pegylated interferon and ribavirin. This study was aimed at analyzing the co-prevalence of two common and clinically significant SNPs in a cohort of Ligurian patients. Methods. Two SNPs (rs12979860, rs8099917) were genotyped in the IL28B locus from 175 DNA samples collected from HCV- infected consecutive patients in a Laboratory of Liguria Region, northern Italy. A real-time polymerase chain reaction in a Cor- bett Research Termocycler (Rotor Gene 3000A) by fluorescent probes (Fast Set IL 28B©, Arrow Diagnostics) was used for the detection, according to the manufacturer?s instructions. Results. Carriers of rs12979860CT genotype predominated (87/175, 50%), homozygotes for allele C were 68/175 (39%) and the remaining were homozygotes for IFN-resistant allele T (11%). As for the rs8099917 SNP, genotypes were thus distributed: 96/175 (55%) carried the rs8099917 TT genotype, whereas 70/175 (40%) and 9/175 (5%), were heterozygotes or homozygotes for the G allele. The variants rs12979860CC and rs8099917TT were found in 39% and 54% of overall patients with HCV genotype 1, respec- tively. The combined assessment of examined SNPs resulted in three most prevalent genotypes (rs12979860CC/rs8099917TT, rs12979860CT/rs8099917TG and rs12979860CT/rs8099917TT) with a frequency of 35%, 31% and 18%, respectively. Discussion. Recent findings demonstrated that in carriers of rs12979860CT the determination of additional genotype of rs8099917 SNP could significantly improve the prediction of SVR. In our study cohort carriers of rs12979860CT represented 50% of all patients, who could take advantage with respect to SVR prediction by further determination of the rs8099917 SNP. The simultaneous genotyping of two IL28B SNPs should thus be recommended in HCV infected patients prior to treatment initiation
