Preventing cardiovascular disease after hypertensive disorders of pregnancy: Searching for the how and when
Background: Women with a history of a hypertensive disorder during pregnancy (HDP) have an increased risk of cardiovascular events. Guidelines recommend assessment of cardiovascular risk factors in these women later in life, but provide limited advice on how this follow-up should be organized. Design: Systematic review and meta-regression analysis. Methods: The aim of our study was to provide an overview of existing knowledge on the changes over time in three major modifiable components of cardiovascular risk assessment after HDP: blood pressure, glucose homeostasis and lipid levels. Data from 44 studies and up to 6904 women with a history of a HDP were compared with risk factor levels reported for women of corresponding age in the National Health And Nutrition Examination Survey, Estudio Epidemiólogico de la Insuficiencia Renal en España and Hong Kong cohorts (N = 27,803). Results: Compared with the reference cohort, women with a HDP presented with higher mean blood pressure. Hypertension was present in a higher rate among women with a previous HDP from 15 years postpartum onwards. At 15 years postpartum (±age 45), one in five women with a history of a HDP suffer from hypertension. No differences in glucose homeostasis parameters or lipid levels were observed. Conclusions: Based on our analysis, it is not possible to point out a time point to commence screening for cardiovascular risk factors in women after a HDP. We recommend redirection of future research towards the development of a stepwise approach identifying the women with the highest cardiovascular risk
Bone health and coronary artery calcification: The Rotterdam Study
Objectives: Vascular calcification has been associated inconsistently to low bone mineral density and fractures. The aims of the present study were to investigate the associations between coronary artery calcification (CAC) and BMD change, BMD and fracture risk in elderly subjects of the population-based Rotterdam Study. Methods: BMD was assessed through dual-energy X-ray absorptiometry and CAC through Electron-Beam Computed Tomography in 582 men and 694 women. We investigated the associations between BMD change (6.4 years follow-up) and CAC at follow-up and between BMD and CAC (measured simultaneously). In sensitivity analyses we stratified analyses for estradiol levels in women. The association between CAC and fracture risk (9 years follow-up) was tested through competing-risks models. Models were sex-stratified and adjusted for age, body mass index, smoking, bisphosphonate use and age at menopause. Results: There was no association between BMD change and CAC in men. In women, each 1% increase in annual BMD loss was significantly associated with higher follow-up CAC [β = 0.22 (0.06-0.38), p. =. 0.006; prevalence ratio: 4%]. Stratified analyses showed significant associations between BMD loss and follow-up CAC only in women with lower estradiol levels. We found no association between CAC and fracture risk and no association between BMD and CAC cross-sectionally. Conclusions: BMD loss was associated with higher follow-up CAC in women, which might be related to low estrogen levels. No association between CAC and BMD or fracture risk was found. Further studies are required to elucidate the mechanisms that might underlie the association between BMD change and coronary calcification in women
Child Care Time, Parents’ Well-Being, and Gender: Evidence from the American Time Use Survey
This study used data from the ‘Well Being Module’ of the 2010 American Time Use Survey (N = 1699) to analyze how parents experience child care time in terms of meaning and stress levels. Multivariate multilevel regressions showed clear differences by gender and the circumstances of child care activities. Mothers experienced child care time as more stressful than fathers, and fathers as slightly more meaningful. Interactive child care was experienced as more meaningful and less stressful than routine child care, whereas these differences were stronger among fathers than among mothers. Mothers experienced child care with a minor child as highly meaningful, and with an adolescent as particularly stressful. Fathers experienced child care with an infant as highly stressful, and with an offspring in middle childhood as disproportionally meaningful. The spouse’s presence was moderately associated with higher senses of meaning and lower levels of stress during child care, but these differences were modest, and only visible among fathers. Paid work hours increased mothers’ stress levels during child care activities, but reduced fathers’ stress levels. Meanwhile, nonemployed fathers reported child care time as less meaningful than non-employed mothers. Overall, this study has important scientific and practical implications for `understanding the gendered nature of parents’ child care time and well-being

Effect of diet-induced weight loss on lipoprotein(a) levels in obese individuals with and without type 2 diabetes
_Aims/hypothesis:_ Elevated levels of lipoprotein(a) [Lp(a)] are an independent risk factor for cardiovascular disease (CVD), particularly in individuals with type 2 diabetes. Although weight loss improves conventional risk factors for CVD in type 2 diabetes, the effects on Lp(a) are unknown and may influence the long-term outcome of CVD after diet-induced weight loss. Individualized dosing of evinacumab is predicted to yield reductions in drug expenses
Background: Evinacumab is a first-in-class inhibitor of angiopoietin‐like protein 3 (ANGPTL3) for treatment of the rare disease homozygous familial hypercholesterolemia (HoFH). With projected drug costs of $450,000 per person per year, the question rises if cost-efficacy of evinacumab can be further improved. Objectives: To develop an individualized dosing regimen te reduce drug expenses. Methods: Using the clinical and pharmacological data as provided by the license holder, we developed an alternative dosing regimen in silico based on the principles of reduction of wastage by dosing based on weight bands rather than a linear milligram per kilogram body weight (mg/kg) dosing regimen, as well as dose individualization guided by low density lipoprotein cholesterol (LDL-C) response. Results: We found that the average quantity of drug used for a dose could be reduced by 34% without predicted loss in efficacy (LDL-C reduction 24 weeks after treatment initiation). Conclusion: Dose reductions without compromising efficacy seem feasible. We call for implementation and prospective evaluation of this strategy to reduce treatment costs of HoFH.</p
Lipid metabolism during pregnancy:Consequences for mother and child
Purpose of review:Accommodating fetal growth and development, women undergo multiple physiological changes during pregnancy. In recent years, several studies contributed to the accumulating evidence about the impact of gestational hyperlipidemia on cardiovascular risk for mother and child. This review aims to provide a comprehensive overview of the current research on lipid profile alterations during pregnancy and its associated (cardiovascular) outcomes for mother and child from a clinical perspective.Recent findings:In a normal pregnancy, total and LDL-cholesterol levels increase by approximately 30-50%, HDL-cholesterol by 20-40%, and triglycerides by 50-100%. In some women, for example, with familial hypercholesterolemia (FH), a more atherogenic lipid profile is observed. Dyslipidemia during pregnancy is found to be associated with adverse (cardiovascular) outcomes for the mother (e.g. preeclampsia, gestational diabetes, metabolic syndrome, unfavorable lipid profile) and for the child (e.g. preterm birth, large for gestational age, preatherosclerotic lesions, unfavorable lipid profile).Summary:The lipid profile of women during pregnancy provides a unique window of opportunity into the potential future cardiovascular risk for mother and child. Better knowledge about adverse outcomes and specific risk groups could lead to better risk assessment and earlier cardiovascular prevention. Future research should investigate implementation of gestational screening possibilities.</p
Efficacy and safety of lomitapide in homozygous familial hypercholesterolaemia:the pan-European retrospective observational study
Blood pressure patterns during pregnancy and early postpartum in preeclampsia with severe features:predictors of chronic hypertension
Rationale: Women with a history of preeclampsia are at increased risk of chronic hypertension and subsequently cardiovascular disease. Early identification of high-risk patients facilitates personalized interventions to mitigate these risks. Reliable predictors for chronic hypertension after preeclampsia are lacking. This study analyzed blood pressure patterns during pregnancies with preeclampsia in relation to development of chronic hypertension. Methods: This prospective cohort study included only women with preeclampsia with severe features, treated at the Erasmus Medical Center between 2010 and 2022, who attended a follow-up visit 1 to 3 years postpartum. Maternal characteristics, pregnancy outcomes and blood pressure measurements were collected from hospital records. Blood pressure patterns were analyzed using linear mixed regression models and antihypertensive medication usage was evaluated with standardized defined daily doses. Results: Among 72 participants, 29 (40 %) developed chronic hypertension, defined as blood pressure >140/>90 mmHg or antihypertensive medication use at follow-up (mean 1.78 years postpartum). Significant blood pressure differences were observed between weeks 8 and 24 of pregnancy peaking at week 18, where systolic blood pressure averaged 131 mmHg [95 % CI 127–134] in women with chronic hypertension at follow-up, versus 117 mmHg [114–120] in those without. Furthermore, women who developed chronic hypertension used higher antihypertensive dosages during pregnancy (3.8 [95 % CI 2.9–5.6] vs. 2.6 [1.5–3.7]). Conclusion: A higher blood pressure before 24 weeks of pregnancy is significantly associated with chronic hypertension after preeclampsia. These findings suggest a role for blood pressure patterns in postpartum risk assessment, enabling personalized follow-up for women at increased risk for chronic hypertension.</p
