Immuno-modulatory and anti-Inflammatory effects of dihydrogracilin A, a trpene derived from the marine sponge Dendrilla membranosa.
We assessed the immunomodulatory and anti-inflammatory effects of 9,11-dihydrogracilin
A (DHG), a molecule derived from the Antarctic marine sponge Dendrilla membranosa. We used
in vitro and in vivo approaches to establish DHG properties. Human peripheral blood mononuclear
cells (PBMC) and human keratinocytes cell line (HaCaT cells) were used as in vitro system, whereas
a model of murine cutaneous irritation was adopted for in vivo studies. We observed that DHG
reduces dose dependently the proliferative response and viability of mitogen stimulated PBMC.
In addition, DHG induces apoptosis as revealed by AnnexinV staining and downregulates the
phosphorylation of nuclear factor kappa-light-chain-enhancer of activated B cells (NF-B), signal
transducer and activator of transcription (STAT) and extracellular signal\u2013regulated kinase (ERK)
at late time points. These effects were accompanied by down-regulation of interleukin 6 (IL-6)
production, slight decrease of IL-10 and no inhibition of tumor necrosis factor-alpha (TNF-)
secretion. To assess potential properties of DHG in epidermal inflammation we used HaCaT cells;
this compound reduces cell growth, viability and migration. Finally, we adopted for the in vivo study
the croton oil-induced ear dermatitis murine model of inflammation. Of note, topical use of DHG
significantly decreased mouse ear edema. These results suggest that DHG exerts anti-inflammatory
effects and its anti-edema activity in vivo strongly supports its potential therapeutic application in
inflammatory cutaneous diseases
The Oncolytic Caprine Herpesvirus 1 (CpHV-1) Induces Apoptosis and Synergizes with Cisplatin in Mesothelioma Cell Lines: A New Potential Virotherapy Approach.
Malignant mesothelioma (MM) is an aggressive asbestos-related cancer, against which no curative modalities exist. Oncolytic virotherapy is a promising therapeutic approach, for which MM is an ideal candidate; indeed, the pleural location provides direct access for the intra-tumoral injection of oncolytic viruses (OVs). Some non-human OVs offer advantages over human OVs, including the non-pathogenicity in humans and the absence of pre-existing immunity. We previously showed that caprine herpesvirus 1 (CpHV-1), a non-pathogenic virus for humans, can kill different human cancer cell lines. Here, we assessed CpHV-1 effects on MM (NCI-H28, MSTO, NCI-H2052) and non-tumor mesothelial (MET-5A) cells. We found that CpHV-1 reduced cell viability and clonogenic potential in all MM cell lines without affecting non-tumor cells, in which, indeed, we did not detect intracellular viral DNA after treatment. In particular, CpHV-1 induced MM cell apoptosis and accumulation in G0/G1 or S cell cycle phases. Moreover, CpHV-1 strongly synergized with cisplatin, the drug currently used in MM chemotherapy, and this agent combination did not affect normal mesothelial cells. Although further studies are required to elucidate the mechanisms underlying the selective CpHV-1 action on MM cells, our data suggest that the CpHV-1-cisplatin combination could be a feasible strategy against MM
Lenalidomide Maintenance with or without Prednisone in Newly Diagnosed Myeloma Patients: A Pooled Analysis
We conducted a pooled analysis of two phase III trials, RV-MM-EMN-441 and EMN01,
to compare maintenance with lenalidomide-prednisone vs. lenalidomide in newly diagnosed
transplant-eligible and -ineligible myeloma patients. Primary endpoints were progression-free survival, progression-free survival 2 and overall survival with both regimens. A secondary aim
was to evaluate the impact of duration of maintenance on overall survival and on outcome after
relapse. A total of 625 patients (lenalidomide-prednisone arm, n = 315; lenalidomide arm, n = 310)
were analyzed. The median follow-up was 58 months. Median progression-free survival (25 vs.
19 months; p = 0.08), progression-free survival 2 (56 vs. 49 months; p = 0.9) and overall survival
(73 months vs. NR; p = 0.08) were not significantly different between the two arms. Toxicity profiles
of lenalidomide-prednisone and lenalidomide were similar, with the exception of neutropenia that
was higher in the lenalidomide arm (grade ≥ 3: 9% vs. 19%, p < 0.001), without an increase in the rate
of infections. Overall survival (median NR vs. 49 months, p < 0.001), progression-free survival from
relapse (median 35 vs. 24 months, p = 0.004) and overall survival from relapse (median not reached
vs. 41 months, p = 0.002) were significantly longer in patients continuing maintenance for ≥2 years.
We showed that the addition of prednisone at 25 or 50 mg every other day (eod) to lenalidomide
maintenance did not induce any significant advantage
Fast relapse and high drop out rate of 48 weeks daily interferon monotherapy in HIV-infected patients with chronic hepatitis C
BACKGROUND: The standard of care for HCV Hepatitis is the combination of interferon (IFN) plus Ribavirin. In HIV patients the use of this combination therapy may induce drug interactions, and reduces the adherence to HAART. The aim of this study is to evaluate safety and efficacy of a 48 weeks daily dose IFN schedule. METHODS: We evaluated 50 coinfected patients; alpha IFN 2a was administered at a dose of 3 MU daily. The baseline values were the following : CD4+ 515 cells/mmc (mean); HIV-RNA <50 copies/ml in all patients; HCV-RNA 28, 3 × 106 copies/ml. RESULTS: At 48 weeks, 10 patients (20%) achieved a biochemical and virological response according to an intention to treat analysis. Twenty four patients (48%) underwent a drop-out mainly by side effects related to overlapping toxicity of interferon and antiretroviral therapy. All the patients, who responded to the treatment, showed a fast relapse one month after the end of treatment. CONCLUSION: Although our results demonstrated a very poor outcome and a bad tolerance to interferon monotherapy, this approach should not be dropped out, mainly in patients at high risk for side effects and in those with cirrhosis who do not tolerate or are at increased risk for the use of ribavirin
