Party control, party competition and public service performance
publication-status: Acceptedtypes: ArticleThis article assesses party effects on the performance of public services. A policy-seeking model,
hypothesizing that left and right party control affects performance, and an instrumental model, where
all parties strive to raise performance, are presented. The framework also suggests a mixed model in
which party effects are contingent on party competition, with parties raising performance as increasing
party competition places their control of government at increasing risk. These models are tested against
panel data on English local governments’ party control and public service performance. The results
question the traditional account of left and right parties, showing a positive relationship between rightwing
party control and performance that is contingent on a sufficiently high level of party competition.
The findings suggest left–right models should be reframed for the contemporary context
Reassessing Britain’s ‘post-war consensus’: the politics of reason 1945–1979
Since the late-1970s, scholars have been engaged in a vibrant debate about the nature of post-war British politics. While some writers have suggested that the three decades that succeeded the Second World War witnessed a bi-partisan consensus on key policy questions, others have argued that it was conflict, not agreement, that marked the period. This article offers a novel contribution to this controversy by drawing attention to the epistemological beliefs of the Labour and Conservative parties. It argues that once these beliefs are considered, it becomes possible to reconcile some of the competing claims made by proponents and critics of the ‘post-war consensus’ thesis. Labour and Conservative leaders may have been wedded to different beliefs, but they also shared a common enthusiasm for empiricist reasoning and were both reluctant to identify fixed political ‘ends’ that they sought to realise. Consequently, they were both committed to evolutionary forms of change, and they eschewed the notion that any social or political arrangement was of universal value

Factors associated with self-reported discrimination against men who have sex with men in Brazil
OBJECTIVE: To estimate self-reported discrimination due to sexual orientation among men who have sex with men (MSM) in Brazil and to analyze associated factors. METHODS: A cross-sectional study of 3,859 MSM recruited in 2008–2009 with respondent driven sampling. Data collection conducted in health centers in 10 Brazilian cities. A face-to-face questionnaire was used and rapid HIV and syphilis tests conducted. Aggregated data were weighted and adjusted odds ratio estimated to measure the association between selected factors and self-reported discrimination due to sexual orientation. RESULTS: The sample was predominantly young, eight plus years of schooling, pardo (brown), single, low-income, and identified themselves as gay or homosexual. The prevalence of self-reported discrimination due to sexual orientation was 27.7% (95%CI 26.2–29.1). Discrimination was independently associated with: age < 30 years, more years of schooling,community involvement and support, history of sexual and physical violence, suicidal thoughts, and unprotected receptive anal intercourse. CONCLUSIONS: The prevalence of self-reported discrimination among MSM in Brazil is high. These results challenge the assumptions that MSM-specific prevention and support programs are not required or that health professionals do not need special training to address MSM needs
Successful recruitment to trials: findings from the SCIMITAR plus Trial
BACKGROUND: Randomised controlled trials (RCT) can struggle to recruit to target on time. This is especially the case with hard to reach populations such as those with severe mental ill health. The SCIMITAR+ trial, a trial of a bespoke smoking cessation intervention for people with severe mental ill health achieved their recruitment ahead of time and target. This article reports strategies that helped us to achieve this with the aim of aiding others recruiting from similar populations. METHODS: SCIMITAR+ is a multi-centre pragmatic two-arm parallel-group RCT, which aimed to recruit 400 participants with severe mental ill health who smoke and would like to cut down or quit. The study recruited primarily in secondary care through community mental health teams and psychiatrists with a smaller number of participants recruited through primary care. Recruitment opened in October 2015 and closed in December 2016, by which point 526 participants had been recruited. We gathered information from recruiting sites on strategies which led to the successful recruitment in SCIMITAR+ and in this article present our approach to trial management along with the strategies employed by the recruiting sites. RESULTS: Alongside having a dedicated trial manager and trial management team, we identified three main themes that led to successful recruitment. These were: clinicians with a positive attitude to research; researchers and clinicians working together; and the use of NHS targets. The overriding theme was the importance of relationships between both the researchers and the recruiting clinicians and the recruiting clinicians and the participants. CONCLUSIONS: This study makes a significant contribution to the limited evidence base of real-world cases of successful recruitment to RCTs and offers practical guidance to those planning and conducting trials. Building positive relationships between clinicians, researchers and participants is crucial to successful recruitment
Mister Mary Somerville: Husband and Secretary
Mary Somerville’s life as a mathematician and savant in nineteenth-century Great Britain was heavily influenced by her gender; as a woman, her access to the ideas and resources developed and circulated in universities and scientific societies was highly restricted. However, her engagement with learned institutions was by no means nonexistent, and although she was 90 before being elected a full member of any society (Società Geografica Italiana, 1870), Somerville (Figure 1) nevertheless benefited from the resources and social networks cultivated by such institutions from as early as 1812. A key intermediary between Somerville and these societies was her husband, Dr. William Somerville, whose mediation was vital to her access to knowledge and her subsequent career as a scientific author. In this paper we will consider how spousal cooperation enabled the overcoming of gendered barriers to scientific institutions in the nineteenth century
