Coil-Assisted Retrograde Transvenous Obliteration (CARTO) for the Treatment of Portal Hypertensive Variceal Bleeding: Preliminary Results.

ObjectivesTo describe the technical feasibility, safety, and clinical outcomes of coil-assisted retrograde transvenous obliteration (CARTO) in treating portal hypertensive non-esophageal variceal hemorrhage.MethodsFrom October 2012 to December 2013, 20 patients who received CARTO for the treatment of portal hypertensive non-esophageal variceal bleeding were retrospectively evaluated. All 20 patients had at least 6-month follow-up. All patients had detachable coils placed to occlude the efferent shunt and retrograde gelfoam embolization to achieve complete thrombosis/obliteration of varices. Technical success, clinical success, rebleeding, and complications were evaluated at follow-up.ResultsA 100% technical success rate (defined as achieving complete occlusion of efferent shunt with complete thrombosis/obliteration of bleeding varices and/or stopping variceal bleeding) was demonstrated in all 20 patients. Clinical success rate (defined as no variceal rebleeding) was 100%. Follow-up computed tomography after CARTO demonstrated decrease in size with complete thrombosis and disappearance of the varices in all 20 patients. Thirteen out of the 20 had endoscopic confirmation of resolution of varices. Minor post-CARTO complications, including worsening of esophageal varices (not bleeding) and worsening of ascites/hydrothorax, were noted in 5 patients (25%). One patient passed away at 24 days after the CARTO due to systemic and portal venous thrombosis and multi-organ failure. Otherwise, no major complication was noted. No variceal rebleeding was noted in all 20 patients during mean follow-up of 384±154 days.ConclusionsCARTO appears to be a technically feasible and safe alternative to traditional balloon-occluded retrograde transvenous obliteration or transjugular intrahepatic portosystemic shunt, with excellent clinical outcomes in treating portal hypertensive non-esophageal variceal bleeding

Reconstruction of Criminal Sanction and Rehabilitation Combating On Narcotic’s Victims Based On Religious Justice

During this time, law enforcement officials tend to impose criminal sanctions for victims of drug without rehabilitation but instead provide criminal sanctions such as prison. Without the detoxification process through medical and social rehabilitation process, they will soon be back looking for narcotics so out of prisons because of the association in the increasingly severe prison, As described in Act No. 35 of 2009 on Narcotics Article 54 that the rehabilitation of abusers of Narcotics; “Addicts Narcotics and Narcotics abuse on victims to undergo mandatory medical rehabilitation and social rehabilitation”. It is strengthened by the Supreme Court Circular No. 04 of 2010 concerning the Placement, Victims and Narcotic Addict Into The Rehabilitation Institute of Medical and Social Rehabilitation. Because of their position in this case is the victim, not the dealer let alone active users is appropriate that they get special treatment before the law. Law enforcement officials are supposed to be representatives of God on earth to be a judge and establishing the truth, fairest, it erred in sentencing. SUMBANGAN ENERGI DAN PROTEIN MAKAN PAGI TERHADAP ANGKA KECUKUPAN GIZI YANG DIANJURKAN (AKG) BERDASARKAN POLA ASUH ANAK

Latar Belakang : Dalam Upaya peningkatan gizi dan kesehatan anak sekolah, slah satu indikatornya adalah "keluarga biasa makan pagi". Kebiasaan makan pagi pada anak sekolah tidak akan berhasil tanpa dukungan keluarga khususnya peran seorang ibu. Data dari Puskesmas Kaligangsa tahun 2005, sebesar 32,9 % anak SD/MI tidak diasuh oleh ibunya karena ibu usaha Warung Tegal (Warteg) di Jakarta. Tujuan: untuk menganalisis perbedaan sumbangan energi dan protein makan pagi terhadap Angka Kecukupan Gizi yang dianjurkan (AKG) berdasarkan pola asuh pada anak Madrasah Ibtidaiyah (MI) Nurul Hikmah. Metode: penelitian ini adalah penelitian analitik dengan pendekatan cross sectional. Populasi penelitian ini adalah siswa kelas IV, V, VI MI Nurul Hikmah yang berjumlah 99 anak. Sampel sebesar 70 siswa yang diambil dengan metode "Stratified Random Sampling", masing-masing kelompok diambil secara proporsional. Analisis data secara deskriptif dan analitik dan untuk menguji Perbedaan pada kedua kelompok (diasuh ibu dan tidak diasuh ibu) digunakan "Independent Sample t Test dan Mann-Whitney U test". Hasil: menunjukkan rata-rata sumbangan energi makan pagi terhadap AKG pada anak yang diasuh ibu sebesar 23,8% sedangkan pada anak yang diasuh bukan ibu sebesar 20,3 %. rata-rata sumbangan protein makan pagi terhadap AKG pada anak yang diasuh ibu sebesar 25,6 % sedangkan pada anak yang diasuh bukan ibu sebesar 24,4 %. Hasil uji tidak ada perbedaan yang signifikan sumbangan energi maupun protein makan pagi terghadap AKG berdasarkan pola asuh. Kesimpulan: sumbangan makan pagi diharapkan dapat mencukupi sebesar 20-30% AKG, karena diperlukan untuk aktifitas di sekolah. Dengan demikian hasil ini merupakan kondisi yang baik dan perlu dipertahankan.Kata kunci: energi, protein, AKG, pola asuh anak
