Comparison of measurements from digital cephalometric radiographs and 3D MDCT-synthetized cephalometric radiographs and the effect of head position

Purpose : To investigate the reproducibilities and compare the measurements in digital and MDCT-synthesized cephalometric radiograph, and to investigate the effect of head position on the measurement during imaging with MDCT. Materials and Methods : Twenty-two dry skulls (combined with mandible) were used in this study. Conventional digital cephalometric radiograph was taken in standard position, and MDCT was taken in standard position and two rotated position (10°left rotation and 10°right tilting). MDCT data were imported in OnDemand® and lateral cephalometric radiograph were synthesized from 3D virtual models. Two types of rotated MDCT data were synthesized with default mode and with corrected mode using both ear rods. For all six images, sixteen angular and eleven linear measurements were made in V-Ceph® three times. Reproducibility of measurements was assessed using repeated measures ANOVA and ICC. Linear and angular measurements were compared between digital and five MDCTsynthesized images by Student t-test. Results : All measurements in six types of cephalometric radiograph were not statistically different under ICC examination. Measurements were not different between digital and MDCT-synthesized images (P> .05). Measurements in MDCT-synthesized image in 10°left rotation or 10°right tilting position showed possibility of difference from digital image in some measurements, and possibility of improvement via realignment of head position using both ear rods. Conclusion : MDCT-synthesized cephalometric radiograph can substitute conventional cephalometric radiograph. The error on head position during imaging with MDCT have possibility that can produce measurement errors with MDCT-synthesized image, and these position error can be corrected by realignment of the head position using both ear rods. Development and application of stent-based image guided navigation system for oral and maxillofacial surgery

Purpose : The purpose of this study was to develop a stent-based image guided surgery system and to apply it to oral and maxillofacial surgeries for anatomically complex sites. Materials and Methods : We devised a patient-specific stent for patient-to-image registration and navigation. Threedimensional positions of the reference probe and the tool probe were tracked by an optical camera system and the relative position of the handpiece drill tip to the reference probe was monitored continuously on the monitor of a PC. Using 8 landmarks for measuring accuracy, the spatial discrepancy between CT image coordinate and physical coordinate was calculated for testing the normality. Results : The accuracy over 8 anatomical landmarks showed an overall mean of 0.56±0.16 mm. The developed system was applied to a surgery for a vertical alveolar bone augmentation in right mandibular posterior area and possible interior alveolar nerve injury case of an impacted third molar. The developed system provided continuous monitoring of invisible anatomical structures during operation and 3D information for operation sites. The clinical challenge showed sufficient accuracy and availability of anatomically complex operation sites. Conclusion : The developed system showed sufficient accuracy and availability in oral and maxillofacial surgeries for anatomically complex sites. Explaining the trend in economic old-age support from children in Korea: A focus on coresidence and private transfers

학위논문 (박사)-- 서울대학교 대학원 : 사회과학대학 사회복지학과, 2018. 8. 구인회.전통 사회의 사적 부양이 약화되었지만 현대적인 공적 노후소득보장제도가 충분히 발전하지 못한 한국에서 노인빈곤이 심각한 사회 문제로 제기되고 있다. 이에 대응하는 노후소득보장체계를 마련하기 위해서는 사적 부양이 감소한 이유를 정확하게 파악할 필요가 있다. 본 연구는 고령화연구패널조사 데이터를 활용하여 2008~2014년 동안 노인에 대한 자녀의 경제적 부양이 변화한 원인을 밝히고자 한다. 본 연구의 연구 문제는 다음과 같다. 첫째, 노인과 자녀 특성의 시간에 따른 변화가 노인과 자녀의 동거 분포를 어떻게 변화시키는가? 둘째, 노인과 자녀 특성 및 동거의 시간에 따른 변화가 노인이 자녀로부터 받는 사적이전 분포를 어떻게 변화시키는가? 이러한 연구 문제에 답하기 위해 본 연구는 재가중 방법을 사용하여 설명변수 분포 변화가 결과변수 분포 변화에 미치는 영향을 분석하였다. 동거와 사적이전의 추이는 다음과 같이 분석되었다. 노인을 대상으로 분석한 결과, 자녀와 동거하는 노인의 비율이 37.3%에서 30.3%로 7.0%p 감소하여 동거가 빠르게 감소하는 추이가 분명하게 확인되었다. 반면 노인이 받은 사적이전의 평균이 82.7만원/년에서 93.4만원/년으로 10.8만원/년 증가한 것으로 나타나, 사적이전의 감소 추이는 확인되지 않았다. 하지만 노인의 총소득 평균 대비 사적이전 평균의 비율은 14.4%에서 12.5%로 감소하여 노후소득보장체계에서 사적이전의 중요성이 점차 감소하고 있음을 알 수 있다. 한편 자녀를 대상으로 분석한 결과, 노부모와 동거하는 자녀의 비율이 11.9%에서 10.7%로 1.2%p 감소하였고, 자녀가 노부모에게 제공하는 사적이전의 평균이 31.4만원/년에서 39.5만원/년으로 8.1만원/년 증가한 것으로 나타났다. 동거와 사적이전 추이의 영향요인을 분석한 결과는 다음과 같이 요약할 수 있다. 첫째, 노인의 사회경제적 특성 변화가 대체로 부양을 감소시켰다. 노인의 고용률이 증가하고 시장소득수준이 향상되면서 사적이전이 감소하였고, 추가 분석에서 동거가 감소하였다는 결과도 관찰되었다. 또한, 자산이 존재하지 않아 주거와 생계를 자녀에게 의존해야 하는 노인의 비율이 감소하면서 동거가 감소하였다. 이와 같은 분석 결과는 시간에 따라 노인의 경제력 수준이 향상되면서 부양이 감소하였다는 사실을 보여준다. 둘째, 자녀의 사회경제적 특성 변화가 부양을 증가시키는 효과와 감소시키는 효과가 모두 발견되었다. 상대적으로 많은 사적이전을 제공하는 고학력 자녀와 취업 자녀의 비율이 증가하면서 사적이전이 증가하였지만, 상대적으로 많은 사적이전을 제공하는 주택 소유 자녀의 비율이 감소하면서 사적이전이 감소하였다. 전반적으로 자녀 경제력이 부양에 영향을 미친다는 사실은 분명하지만, 교육수준과 고용률이 증가한 반면 주택 소유 비율이 감소하였기 때문에 자녀 경제력 변화의 방향을 확정하기는 어렵다. 셋째, 공적연금 수급률이 약 20%p 증가하는 등 공적연금소득이 크게 증가하여 사적이전이 감소한 것으로 나타나, 공적연금 확대의 사적이전 구축 효과가 실증되었다. 한편, 내생성 때문에 공적연금 외 공적이전 증가의 영향을 분석하는 데는 한계가 있었다. 넷째, 시간에 따른 출산율 하락이 부양 변화에 영향을 미쳤다. 노인을 대상으로 분석하면, 자녀가 많은 노인이 상대적으로 많은 사적이전을 받기 때문에, 노인의 자녀 수가 감소하여 사적이전이 감소하였다. 자녀를 대상으로 분석하면, 형제가 많은 자녀가 상대적으로 적은 부양을 제공하기 때문에, 자녀의 형제 수가 감소하여 동거와 사적이전이 증가하였다. 이는 출산율 하락에 따른 인구구조 변화가 노인과 자녀 모두에게 부담으로 작용하고 있음을 의미한다. 다섯째, 노인과 자녀의 연령, 성, 결혼상태 변화의 영향이 확인되었다. 우선 다른 특성을 통제하지 않으면 노인의 고령화가 동거와 사적이전을 증가시켰지만, 자녀 연령을 포함한 여러 특성을 통제하면 이러한 영향이 사라졌다. 그리고 상대적으로 동거 성향이 강한 20~30대 자녀의 비율이 감소하여 동거가 감소하는 영향이 추가 분석에서 확인되었다. 한편, 여성 무배우 노인의 비율이 감소하는 변화는 주로 사적이전을, 부분적으로 동거를 감소시켰고, 무배우 자녀의 비율이 증가하는 변화는 동거를 증가시켰다. 마지막으로, 자녀의 형제 수를 통제하지 않으면 장남 비율이 증가하여 사적이전이 증가하였지만, 자녀의 형제 수를 통제하면 장남 비율이 감소하고 딸 비율이 증가하여 사적이전이 감소하였다. 여섯째, 산업화와 도시화의 영향은 분명하지 않았다. 전반적인 분석 결과를 종합할 때, 다른 특성을 통제하지 않으면 도시화․산업화가 동거와 사적이전을 증가시키지만, 여러 특성을 통제하면 이러한 영향이 사라지는 것으로 판단된다. 일곱째, 다른 특성 변화로 설명되지 않는 동거 감소가 사적이전을 증가시키는 영향이 노인 대상 분석과 자녀 대상 분석에서 모두 확인되었다. 단, 동거 비율 감소량보다 사적이전 수혜/제공 비율 증가량이 훨씬 작게 나타나, 동거가 감소하여 사적이전이 증가하는 효과의 크기는 제한적인 것으로 판단된다. 마지막으로, 본 연구의 영향요인으로 설명되지 않는 동거 비율 감소량이 상당히 크게 나타났다. 이는 본 연구에서 제대로 다루지 못한 자녀 경제력과 부양의식 변화가 동거를 감소시켰을 가능성을 시사한다. 특히 시간에 따라 개인주의가 확산되는 문화적 변화가 동거 감소에 중요한 영향을 미쳤을 것으로 판단된다. 본 연구는 주로 횡단적 영향요인 분석에 집중된 노인 부양 관련 선행연구의 한계를 극복하고 동거와 사적이전을 함께 고려하여 체계적으로 노인 부양 추이의 영향요인을 분석하기 위해 노력하였다. 본 연구의 분석 결과는 가족, 시장, 국가의 역할 분담을 조정하여 사적 부양 감소에 대응하는 노후소득보장체계를 마련하는 데 의미 있는 정책적 시사점을 제공해준다. 현재 한국 사회에서 사적 부양제도를 유지하려는 노력이 성공할 가능성은 크지 않은 것으로 판단되고, 시장을 통한 노인의 자립과 국가를 통한 공적 부양을 함께 강화해나가야 할 것이다.제 1 장 서론 1 제 1 절 문제 제기 1 제 2 절 연구 문제 및 모형 5 제 2 장 이론적 논의 및 선행연구 검토 7 제 1 절 노인 부양의 실태와 추이 7 제 2 절 노인 부양의 영향요인에 관한 논의 10 1. 이타주의 모델과 교환 모델 11 2. 인구학적 요인 20 3. 산업과 지역 요인 25 4. 부양문화와 가부장적 가족관계 27 5. 기타 논의 31 제 3 절 노인 부양 추이의 영향요인에 관한 논의 32 1. 노인의 사회경제적 특성 변화 32 2. 자녀의 사회경제적 특성 변화 38 3. 공적이전의 확대 40 4. 인구학적 변화 42 5. 산업화와 도시화 45 제 3 장 연구 방법 47 제 1 절 분석 자료 47 제 2 절 변수의 측정 54 1. 결과변수 54 2. 설명변수 57 제 3 절 분석 방법 63 1. 동거와 사적이전의 추이 분석 63 2. 동거와 사적이전의 영향요인 횡단 분석 67 3. 동거와 사적이전 추이의 영향요인 분석 68 제 4 절 분석 모델 71 제 4 장 분석 결과 77 제 1 절 노인과 자녀 특성의 추이 77 1. 노인 대상 분석 77 2. 자녀 대상 분석 85 제 2 절 동거와 사적이전의 추이 90 1. 노인 대상 분석 90 2. 자녀 대상 분석 97 3. 노인 대상 분석과 자녀 대상 분석의 관계 100 제 3 절 동거와 사적이전의 횡단적 영향요인 105 1. 노인 대상 분석 105 2. 자녀 대상 분석 120 제 4 절 동거와 사적이전 추이의 영향요인 126 1. 노인 대상 분석 126 2. 자녀 대상 분석 150 3. 추가 분석 162 제 5 장 결론 179 제 1 절 분석 결과 요약 179 제 2 절 이론적 함의 182 제 3 절 정책적 함의 183 제 4 절 연구의 한계 188 참고문헌 190 부표 207 Abstract 209Docto

Recurrent odontogenic keratocyst within the masticatory space

The odontogenic keratocyst (OKC) is a developmental odontogenic cyst typically occurring in the jaws. Since the first description of OKC was published in 1956, the lesion has been of particular interest because of its specific histopathologic features, high recurrence rate, and aggressive behavior. Recurrences most commonly arise within bone at the site of the original cyst. However, as lining cells may find their way into surrounding tissues either from implantation during surgery or from cortical perforation recurrences may arise at a distance from the original cyst. Here, we report a rare case of recurrent OKC which was first developed in mandible and recurred within the masticatory space

Evaluation of masticatory muscles of mandibular asymmetry patients by computed tomography

- Purpose: To compare the size of the masseter and lateral pterygoid muscle between the affected and the unaffected side of the patients who have the chief complaints of the mandibular asymmetry. - Materials and Methods: Twenty two patients (male: 4, female: 18, average age: 21.3 year-old) were radiographed using posterior-anterior (P-A) cephalography and computed tomography (CT). On P-A cephalography, the degree of deviation was determined by the distance from the mentum to the vertical reference line through the crista galli and the anterior nasal spine. On the scanned tracing papers of the maximum cross-sectional area of the masseter and lateral pterygoid muscle using axial CT images, the pixel number was measured. The ratio of the affected : unaffected sides were obtained. For the masseter and lateral pterygoid muscle, the relationship between the muscular volume and degree of skeletal hypoplasia was studied. - Results : The half cases showed no skeletal asymmetry. The lateral pterygoid muscle of the affected side was larger significantly than unaffected side (p<0.05). However, there was no significant difference between two sides in the cases of skeletal asymmetry. There was only significant difference in the cases without skeletal asymmetry (p<0.05). - Conclusions : To some extent, the slight mandibular hypoplasia could affect the growth of some masticatory muscles
