    INTERRELATIONSHIP BETWEEN MANDIBULAR CONDYLAR HEAD POSITION IN TRANSCRANIAL VIEW AND ARTICULAR DISC POSITION

    This study was designed to evaluate the interrelationship between the condylar head position in transcranial view and the articular disc position in the arthrography. The condylar positions were assessed by subjective method and linear measurement method on the transcranial view. The subjects for this study consisted of 24 symptomatic joints with normal disc position, 37 joints with anterior disc displacement with reduction and 44 joints with anterior disc displacement without reduction that were classified by arthrotomography under the fluoroscopic guidance. The interrelationship between the condylar head position in transcranial view and the articular disc position in the arthrography was evaluated by Chi square test. The obtained results were as follows : 1. There was no significant interrelationship between the position of condylar head in closed mouth state on transcraniaJ view and articular disc position in the arthrography (p>0.05). 2. There was no significant interrelationship between the changes of interarticular distance in 1 inch opening state and articular disc position in the arthrography (p>0.05). 3. There was no significant interrelationship between the position of condylar head related to the apex of articular eminence in 1 inch opening state and articular disc position in the arthrography(p>0.05). 4. There was significant interrelationship between the changes of interarticular distance that is assessed by linear measurement method in maximum opening state and articular disc position in the arthrography(p0.05). 5. There was significant interrelationship between the degree of condylar translation in maximum opening state and articular disc position in the arthrography(p<0.05). 6. The correlation coefficient between two methods to assess the position of condylar head were 0.7989: the condylar head position in articular fossa in closed state, 0.6847: interarticular space in 1 inch opening state, 0.8965: the degree of condylar translation in 1 inch opening state, 0.5944: the changes of interarticular space in maximum opening state, 0.9215: the degree of condylar translation in maximum opening state

    Benign lymphoepithelial lesion arising in the parotid gland

    The authors diagnosed a 33 years old female as benign lymphoepithelial lesion after undergoing clinical, radiological and histopathological examinations and the characteristics were as follows : 1. Clinically, the patient complained of painless bilateral swelling of the parotid glands and dryness of the palate. Rheumatoid factor was detected in her serum. 2. Sialograms showed punctate or globular collections of contrast media distributed evenly throughout the parotid glands in so-called 'cherry blossom' or 'leafless fruit-laden tree' appearance. 3. A salivary gland scan showed no uptake of radioisotopes by the parotid glands. 4. At Tl-weighted imaging of PNS MRI, the lesions had the same signal intensity as the rest of the gland. At TI-weighted imaging, the lesions could be seen as high signal intensity 1.3 cm and 2.1 cm in diameter in the left and the right parotid gland respectively. 5. Ultrasonogram showed sonolucent lesions 20Γ—15mm and 17Γ—14mm in size in the lower part of the left parotid gland and another 18Γ—11mm in size in the lower part of the parotid gland as well as many other small sonolucent lesions. 6. Histopathologically, lymphocytic infiltration replacing the normal acini and lymphoid follicles containing germinal centers could be seen. Epimyoepithelial islands were scattered throughout the lesion and benign lymphoepithelial cysts were also observed

    ULTRASTRUCTURAL STUDY OF THE FIBROBLAST REMODELING IN THE RADIATION- IMPAIRED WOUND HEALING IN THE TONGUE OF THE RAT

    Radiation-impaired wound healing in animal experiments was believed to be an another logical experimental model to understand the wound healing mechanism in patients. The purpose of this study was to reveal the block point which would result in impaired healing. Twenty four rats(Sprague-Dawley strains) were divided into two groups according to the time interval between irradiation and wounding. Group I, observing the healing effect on the 1st day and Group II are the healing effects on the 7th days after irradiation to the wound of the rat tongue. Experimental animals were sacrificed 3, 6, 12, and 24 hours after wounding. The specimens were examined by the light microscope and transmission electron microscope. The following results were obtained 1. Fibroblasts in both groups showed degenerative changes which were dilated mitochondria and rER, reduced microorganelle, vacuoles and little cytoplasmic process. 2. Average length between bands and Quantity of the newly produced collagen fibers around fibroblasts remained unchanged against control group. 3.     Correlation of condylar mobility and arthrotomography in patients with internal derangements of the temporomandibular joint

    Arthrography of the temporomandibular joint is a useful method of demonstrating the soft tissue abnormalities related to disc dysfunction. In this study, 19 asymptomatic joints, 31 joints with anterior disc displacement with reduction, 31 Joints with anterior disc displacement without reduction which were classified by arthrotomography under fluoroscopy were evaluated to determine the linear measurement of anterior recess of inferior joint space and the relationship between the condylar anterior translation and the severity of the internal derangements. Their fluoroscopic images were also evaluated to describe the characteristics of condylar paths in infernal derangements of the temporomandibular joints. The results were as follows ; 1. The mean lengths of the anterior recess in asymptomaic group, reduction group, and non-reduction group were 8.7Β±1.6 ㎜, 11.2Β±1.7㎜, 12.8Β±1.7㎜ respectively. The length of the anterior recess was increased according to the severity of the internal derangements(P<0.05). 2. Linear measurements of anterior movement of condyle on maximum mouth opening were 13.1Β±4.2㎜, 15.9Β±4.1㎜, 5.0Β±3.7㎜ in asymptomatic group, reduction group, and non-reduction group respectively. Compared with aymptomatic group, reduction group showed hypermobolity of the condyle and non-reduction group showed hypomobility. 3. Condyles moved beyond the crest of articular eminence in 80% of reduction group and did not reach it in 70% of non-reduction group. 4. The condyle moved mainly superiorly in reduction group(66%) and horizontally in asymptomatic group(47%).     RADIATION EFFECT ON THE STAGES OF AMELOGENESIS IN THE RAT INCISOR

    The purpose of this study was to investigate the radiation effect on the stages of amelogenesis. Twenty ll-day-old rats which were irradiated by 4Gy of garmna radiation on the 19th prenatal day were used for the experimental group and twenty ll-day-old rats which were not irradiated were used for the control group. The length of each zone of amelogenesis were measured on the sagittal section using a light microscopic enlargement at 400 x the normal view while the morphologic changes of ameloblasts of each zone were observed electronΒ­microscopically. The obtained results were as followed : 1. The length of the region of facing pulp and facing dentin of the zone of presecretion were increased by 11.5οΌ…(P0.05). 4. Electron-microscopically, enlargement of the cell membrane, rER, mitochondria, and nuclear membrane were observed.     Evaluation of compression ratios using JPEG 2000 on diagnostic images in dentistry

    Purpose : To find out the proper compression ratios without degrading image quality and affecting lesion detectability on diagnostic images used in dentistry compressed with JPEG 2000 algorithm. Materials and Methods : Sixty Digora periapical images, sixty panoramic computed radiographic (CR) images, sixty computed tomographic (CT) images, and sixty magnetic resonance (MR) images were compressed into JPEG 2000 with ratios of 10 levels from 5:1 to 50:1. To evaluate the lesion detectability, the images were graded with 5 levels (1 : definitely absent; 2: probably absent; 3: equivocal; 4: probably present; 5: definitely present), and then receiver operating characteristic analysis was performed using the original image as a gold standard. Also to evaluate subjectively the image quality, the images were graded with 5 levels (1 definitely unacceptable; 2: probably unacceptable; 3: equivocal, 4: probably acceptable; 5Β· definitely acceptable), and then paired t-test was performed. Results : In Digora, CR panoramic and CT images, compressed images up to ratios of 15 : 1 showed nearly the same lesion detectability as original images, and in MR images, compressed images did up to ratios of 25 : 1. In Digora and CR panoramic Images, compressed images up to ratios of 5 : 1 showed little difference between the original and reconstructed images in subjective assessment of image quality In CT images, compressed images did up to ratios of 10: 1 and in MR images up to ratios of 15 : 1 Conclusion : We considered compression ratios up to 5 : 1 in Digora and CR panoramic images, up to 10 : 1 in CT images, up to 15 : 1 in MR images as clinically applicable compression ratios

    A CASE REPORT OF PRIMARY INTRA-OSSEOUS CARCINOMA OF THE MAXILLA

    The primary intra-osseous carcinoma (PIOC) is a very rare lesion. PIOC is an odontogenic carcinoma defined as a squamous cell carcinoma arisinig within a jaw having no initial connection with the oral mucosa, and presumably developing from residues of the odontogenic epithelium. The authors diagnosed a 51-year-old female as primary intra-osseous carcinoma after undergoing clinical, radiological and histological examinations. The characteristics were as followed : 1. The patient complained of gingival bleeding on the premolar area in the left maxilla 2. The conventional radiograms showed a relatively well-defined unilocular radiolucent lesion from the mesial aspect of the upper left canine to the mesial aspect of the upper left 1st molar. The 2nd premolar was separated from the 1st molar and the floor of the maxillary sinus was elevated by the lesion. There was a external root resorption of the upper left canine, the 1st premolar, and the 2nd premolar. 3. On the computed tomograms, the osteolytic bony lesion expanded the cortical plate of the left maxilla and displaced the margin of the left maxillary sinus upwards. But the bony lesion was separated from the maxillary sinus by a bony septum. 4. Bone scintigram with /sup 99m/Tc demonstrated the increased uptake in the left maxilla. Sonograms in the neck area and chest P-A radiogram didn't show any abnormalities. 5. Histologically, the tumor islands infiltrating into the surrounding bone increased in alveolar pattern, composed of the malignant cells, and there was a necrosis in the center of the tumor islands

    INFLUENCE OF CENTRAL PANORAMIC CURVE DEVIATION ON THE MANDIBULAR IMAGE RECONSTRUCTION IN THE IMPLANT CT

    The purpose of this study was to investigate an influence of the change of central panoramic curves on the image reconstruction in the dental implant CT. The author designed three experimental groups according to the location of central panoramic curve. In group A, central panoramic curve was determined as the curve connecting the center of roots from the first premolar to the first molar. In group B, central panoramic curve was determined as the line connecting the lingual cortical plate at the level of the mesial aspect of the first premolar with the buccal cortical plate at the level of the mesial aspect of the first molar. In Group C, central panoramic curve was determined as the line connecting the buccal cortical plate at the level of the mesial aspect of the first premolar with the lingual cortical plate at the level of the mesial aspect of the first molar. Twenty four reformatted CT images was acquired from four mandibles embedded in the resin block and twenty four contact radiographs of dog specimens were acquired. Each Image was processed under Adobe Photoshop program analysed by MSPA(mandible/maxilla shape pattern analysis) variables such as MXVD, MXHD, UHD, MHD, and LHD. The obtained results were as follows ; 1. The mean of MXVD variable was 19.9, 20.2, and 20.0 in group A, B, and C, respectively, which were smaller than actual value 20.5. But, there was no significant difference among 3 groups (p>0.05). 2. The mean of MXHD, UHD, MHD, and LHD variables in group A, B, and C was 11.9, 12.2, and 12.3; 9.3, 9.5, and 9.6; 10.0, 10.3, and 10.3; 9.2, 9.3, and 9.4 respectively which were equal to or greater than the actual value 11.8, 9.3, 10.0, and 9.2. But, there was no significant difference among 3 groups (p>0.05). 3. The number of noneffective observations with difference over or under 1 mm with comparison to the actual value was 24(20%), 58(48.3%), and 52(43.3%), respectively, in group A, B, and C. 4. In group A, the number of observations over 1 mm and under 1 mm was 9 and 15, respectively, but in group Band C, the number of observations over 1 mm was more than under 1 mm

    A CLINICO-RADIOLOGIC STUDY OF THE VARIOUS LESIONS IN THE ORAL AND MAXILLOFACIAL REGION OF CHILDREN

    Two thousand-two hundred-forty-three cases of various lesions of the oral and maxillofacial region in children under 16 years of age were reviewed. The lesions were classified by 10 groups; osteomyelitis, cysts, benign tumors, malignant tumors, fibro-osseous lesions, developmental disturbances, antral lesions, TMJ lesions, salivary gland lesions, and other lesions. The obtained results were as follows; 1. Developmental disturbances, especially cleft alveolus and palate, was the most common lesions (25.7οΌ…), followed by cysts (22.5οΌ…), antral lesions (12.7οΌ…), benign tumors (11.9οΌ…) and osteomyelitis (9.3οΌ…). 2. With the time, the incidence of osteomyelitis and malignant tumors has been decreased, but that of developmental disturbances, cysts and antral lesions has been increased. 3. The sex distribution was relatively equal for the entire series, with male predominence in cysts, malignant tumors, developmental disturbanes and salivary gland lesions and with female predominence in TMJ lesions. 4. Children with malignant tumors and osteomyelitis were somewhat younger and those with TMJ lesions were somewhat older. 5.
