Comparison of financial performances in public hospitals before and after the incorporation

보건학과/석사[한글]시·도립병원은 그의 중요성에 비추어보아 보건의료 체계의 구조적 복잡성, 공공병원에 대한 투자 및 정책적 선도의 미흡등으로 의료시설은 영세성을 벗어나지 못하였고, 운영상의 경직성, 시설과 인력의 부족등으로 기능이 점차 약화되었다. 이에 정부는 병원경영의 전문성 및 보건사업의 효율성을 높이기 위하여 시·도립병원을 지방공사 체재로 전환하였다. 본 연구는 1983년 7월 1일부터 지방공사 의료원으로 전환 운영중인 강원도 소재 5개 지방공사 의료원을 대상으로 공사화 전·후의 경영성과에 대하여 재무상태를 중심으로 비교, 검토 하였다. 결과를 요약하면 다음과 같다. 1. 환자진료 실적은 지방공사화 이후 계속 증가하였고, 환자의 구성비 변화는 일반 환자의 경우는 감소하였고, 의료보험 환자의 경우는 증가하였으며, 의료보호 환자의 구성비 변화는 비슷하였다. 2. 의료수익 의료이익률이 지방공사화 이후 증가한 지방공사 의료원은 3개 기관이며, 2개 의료원은 뚜렷한 변화가 없었다. 3. 부채비율이 공사화 이후 증가한 지방공사 의료원은 3개 기관이며 2개 지방공사 의료원은 뚜렷한 변화가 없었다. 4. 진료미수금은 지방공사화 이후 계속 증가하였다. 5. 자본잉여금 수혜액은 지방공사화 이후 1984년도 까지는 뚜렷한 변화가 없었으나, 1985년도 이후에는 급증하였다. 결론적으로 본 연구를 통하여 발견하게된 지방공사화 이후의 경영성과를 다음과 같이 결론 내릴 수 있다. 시·도립병원의 지방 공사화 이후 병원에 따라서 다소의 차이는 있었지만 환자진료 실적은 증가하였고, 재무상태의 경우 향상된 기관이 3개 기관, 뚜렷한 차이가 없는 기관이 2개 기관으로서, 기관에 따라서 차이는 있었으나 재무상태가 호전되었다. [영문]Public Hospitals established by the Korean Government were undergone incorporation as the Local public corporation medical center. The purpose of this incorporation was to improve efficiency of management and quality of medicine due to the serious financial deterioration impaling the role of its service popuration. This study was conducted to understand and evaluate how the local public corporation program affected financial performances of the public hospitals. In attempt to identify these findings, the study employed "Time series design". The data were collected from the local financing section of the Ministry of Home Affairs, the health section of the provincial, and five of six local public corporation medical centers in the Kang-won Provinces. There are 31 local public corporation medical canters in Korea. The five local public corporation medical centers in Kang-won province were selected for the study. These five hospitals was incorporates in July 1983, from the original initiation. These findings are discussed in terms of their implications for public officials, hospital managers, and professors. The major results were as follow: First, changes in medical ease utilization showed increase after the local public corporation program. Changes in the case-mix showed decrease in non-insured patients and increase in health insurance patients. The number of medical aid patients was similar after the local public corporation program. Second, three local public corporation medical centers showed increase in the hospital profit-income ratio after the local public corporation program. However, the other two medical centers did net show any improvement. Third, the debt ratios were increased in three medical centers, and no remarkable changes in the other two medical centers. Fourth, the accounts receivables were increased continuously in five local public corporation medical centers, and the fund surplus showed no remarkable changes in the fiscal year 1984, and an increase in 1985, respectively.restrictio

Mobile Survey for Maternal and Child Health Field Research: Cost and Benefit Analysis

본 연구는 에티오피아 홀레타 지역에서 Mobile Survey(M-Survey)를 이용하여 실시된 모자보건 사업의 설문조사 사례를 설명하고 기존의 서면방식으로 진행하는 설문조사 방식과 비교분석을하는데 목적이 있다. 자료수집은 인구 4만 명의 에티오피아 홀레타 지역의 15~40세의 가임기 여성에 대한 전수조사를 목표로 진행되었으며, 가구방문을 통하여 수행되었다. 본 M-Survey로 진행된 설문에 응답한 가임기 여성은 4,200명이었고, 기존의 서면방식 설문조사와 가격, 시간, 인건비, 데이터의 정확성에 대해 비교 하였다. 분석 결과, M-Survey의 경우 서면 설문조사에 비해 한 명당 2,507.8원(42.2%)를 절감한효과가 있는 것으로 나타났고, 설문조사에 소요된 시간은 한 명당 29.067분(57.7%)을 절감한 효과가 있었다. 설문조사의 오류율 역시 서면 설문조사에 비해 M-Survey가 98.5% 낮게 나타났다. 이를 통해 개발도상국에서 진행되는 지역조사 연구 활동에서 M-Survey 방식이 서면 설문조사 방식에 비해 시간과 예산을 크게 절감하고, 설문조사의 정확도를 높여주는 효과적인 조사방식임을 확인하였다.2
