(The) association of white blood cell count and ischemic stroke, hemorrhagic stroke and total stroke in prospective cohort study

건강증진학과/석사[한글] 이 연구는 전향적 연구 설계를 통하여 백혈구 수치와 뇌졸중 사망률간의 관련성을 알아보고자 하였다.연구 대상자는 국민건강보험공단에 가입한 건강보험가입자 가운데 1992년부터 1995년까지의 건강 검진 수검자 중에서 피부양자인 남녀 총 412,653명이며, 1992년부터 1995년까지 4년간의 자기기입식 설문지를 바탕으로 2년마다 정기검진을 실시하여 2003년 12월까지 11년간 추적되었다. 백혈구 수치는 1993년과 1995년에 피부양자를 대상으로 측정되었다.뇌졸중은 제 9 차와 제 10 차 개정된 국제질병분류(ICD, International Classification of Disease)코드를 기준으로 만들어진 한국질병사인분류를 참고하였으며, 허혈성 뇌졸중, 출혈성 뇌졸중, 허혈성 뇌졸중과 출혈성 뇌졸중을 합친 총 뇌졸중으로 하여 크게 세 분류로 나누었다.통계 분석은 SAS 8.2를 사용하였으며, 연구 결과는 다음과 같다.첫째, 뇌졸중 위험인자와 백혈구 수치에서는 혈압, 체질량 지수, 총 콜레스테롤, 공복 혈당, 흡연, 음주는 정상 범주에 비해 평균 백혈구 수치가 상승함을 알 수 있었으며 통계적으로 유의하였다. 단, 남자의 경우에만 나이의 상승과 상관없는 평균 백혈구 수치를 보였으며 통계적으로 유의하였다.둘째, 뇌졸중 위험인자와 뇌졸중 사망 상대위험비를 알아본 결과, 나이가 한 살 증가함에 따라 남자 1.09배(95% CI 1.09-1.09), 여자 1.12배(95% CI 1.12-1.13)로 큰 차이를 보이지 않았으며, 혈압은 정상 혈압군에 비해 고혈압이 남자 3.06배(95% CI 2.73-3.42), 여자 2.76배(95% CI 2.49-3.07)로 높았으며 모두 통계적으로 유의하였다.흡연의 경우 남자에 있어서 비흡연군에 비해 과거 흡연군은 오히려 0.96배(95% CI 0.87-1.06)로 낮아지나 통계적으로 유의하지 않았고, 현재 흡연군은 1.27배(95% CI 1.16-1.39)였으며, 여자의 경우 비흡연군에 비해 과거 흡연군 1.16배(95% CI 1.03-1.31), 현재 흡연군 1.57배(95% CI 1.44-1.72)로 통계적으로 유의하였으나 약한 상관관계를 보였다.체질량 지수와 총 콜레스테롤의 각각 뇌졸중 사망 상대위험비는 관련성이 없으며 모두 통계적으로 유의하지 않았다.공복 혈당은 ≥126mg/dl군이 <126mg/dl군에 비해 뇌졸중 사망 상대위험비가 남자 1.47배(95% CI 1.32-1.65)였으며, 여자 2.38배(95% CI 2.13-2.66)로 여자에게서 더 관련성이 높았으며, 모두 통계적으로 유의하였다.음주에서는 음주군이 비음주군에 비해 뇌졸중 사망 상대위험비가 남자 0.84배(95% CI 0.79-0.90)로 낮아지고 통계적으로 유의했으며, 여자 1.03배(95% CI 0.94-1.12)이나 통계적으로 유의하지 않았다.셋째, 백혈구 수치의 뇌졸중에 대한 사망 상대위험비는 뇌졸중 위험인자인 나이, 혈압, 흡연, 체질량 지수, 공복 혈당, 총 콜레스테롤, 음주를 모두 통제한 후, Cox's proportional hazard model을 이용하여 분석하였다. 허혈성 뇌졸중과 총 뇌졸중은 백혈구 수치가 1,000cells/mm³씩 상승할수록 남자, 여자, 전체 모두에서 뇌졸중 사망 상대위험비가 증가함을 볼 수 있었다. 그러나 출혈성 뇌졸중은 남자, 여자, 전체 모두에서 백혈구 수치와 뇌졸중 사망 상대위험비의 상관관계를 볼 수 없었으며 통계적으로도 유의하지 않았다.이상의 연구 결과를 종합하여 보면, 백혈구 수치는 뇌졸중의 예측인자로 사용가능하며, 여러 연구에서 입증되었듯이 허혈성 뇌졸중과 관련성이 있으나 출혈성 뇌졸중과는 관련성이 없었다. 이 연구를 통해서 상승한 백혈구 수치는 허혈성 뇌졸중의 사망률을 줄이기 위한 조절 및 통제 역할을 할 수 있음을 보여주었다.뇌졸중은 발생 후 손상된 뇌조직과 그 기능을 완전히 회복할 수 없으므로 이러한 결과를 통해 허혈성 뇌졸중으로 인한 사망을 예방하고자 하나, 사망률보다는 뇌졸중이 발생하는 그 시점, 즉 보건학적으로 의미가 더 큰 발생률을 알 수 있는 백혈구 수치에 대한 연구가 더욱 필요하다고 제언한다. [영문]This study was designed to determine the relationship between white blood cell(WBC) count and ischemic stroke, hemorrhagic stroke and total stroke and to see if WBC count was a significant predictor of outcomes independent of other biomarkers. Inflammation has been shown to play a role in atherosclerosis and cardiovascular disease. The WBC is a commonly performed clinical test, yet much remains to be learn about its variation in the general population and its value as a predictor rather than merely an indicator of disease.The research data used in this study was obtained from the National Health Insurance Corporation. The total number of subjects participating in the initial physical examination of this study was 412,653 without past history of hypertension, diabetes, cancer and cardiovascular disease. This document is based on revised International Classification of Disease, Ninth and Tenth Revision. The definition of stroke included ischemic stroke, hemorrhagic stroke and total stroke. In order to analyze the risk of white blood cell count on ischemic stroke, hemorrhagic stroke and total stroke, it used the Cox''s proportional hazard model.Based on the results of this study, WBC count was increased as the blood pressure, total cholesterol, fasting blood sugar, smoking and alcohol consumption except age were increased.After adjusted age, blood pressure, smoking, body mass index, fasting blood sugar, total cholesterol, alcohol consumption, Cox''s proportional hazard model was used to predict the mortality risk ratio of ischemic stroke, hamorrhagic stroke and total stroke as WBC count stratified six groups, <5.0, 5.0~<6.0, 6.0~<7.0, 7.0~<8.0, 8.0~<9.0, ≥9.0×1,000cells/mm³, was increased.As WBC count indicates inflammation was increased by 1,000cells/mm³, mortality risk ratio of ischemic stroke and total stroke was increased.The stratum with the highest WBC count(≥9.0×1,000cells/mm³) had higher mortality risk ratio from ischemic stroke and total stroke respectively in total study population (hazard ratio［HR］, 2.007; 95% confidence interval［CI］,1.600-2.519 and HR, 1.464; 95% CI ,1.319-1.624), in men (HR, 1.737;,95% CI, 1.246-2.422 and HR 1.447;95% CI 1.234-1.697) and in women (HR, 2.307; 95% CI, 1.676-3.175 and HR, 1.468; 95% CI, 1.273-1.692) compared with the stratum with the lowest level (<5.0×1,000cells/mm³).On the other hand, the mortality risk ratio from hemorrhagic stroke didn''t show the relationship with WBC count and not show statistical significance.These results show that the WBC count is associated with ischemic stroke, but not associated with hemorrhagic stroke. This study shows that individuals with increased WBC count need to be monitored and managed in order to reduce the increased risk of ischemic stroke. This study indicates that if the measured WBC count are increased, healthcare should make every effort to encourage their patients to control the risk factors that lead to cardiovascular events. But WBC count is very fluctuant in acute or chronic inflammation and health conditions. The little evidence was presented about the relationship between inflammation and hemorrhagic stroke. In the future many similar studies were recommended repetitive. Furthermore, in subgroups of hemorrhagic stroke the deeper study is needed.prohibitio
