후방 십자인대 대치형 인공 슬관절 전치환술 이후 발생한 과신전의 위험인자: 대응 증례-대조군 연구

학위논문 (석사) -- 서울대학교 대학원 : 의과대학 의학과, 2021. 2. 이명철.Introduction: Hyperextension is one of the complications that can occur after total knee arthroplasty (TKA), and it can be difficult to manage. Although many studies have focused on hyperextension developing in the immediate post-operative period, there is only limited research on its occurrence during follow-up after TKA. The purpose of this study was to investigate factors contributing to the de novo development of hyperextension after posterior cruciate ligament substituting (PS) TKA. Materials and Methods: Through a retrospective case-control study, de novo hyperextension patients were compared with patients without hyperextension after primary PS TKA. Eighty-five de novo hyperextension patients were compared with 85 patients in a control group matched by age, sex, surgeon and implant. The clinical and radiographic parameters, including the mechanical axis (MA), joint line convergence angle (JLCA), posterior tibial slope angle (PTSA), posterior condylar offset (PCO), and the gamma angle were evaluated preoperatively and immediate postoperatively. Comparisons between the two groups and logistic regression analysis were performed to investigate factors contributing to the development of de novo hyperextension after PS TKA. Results: Among the clinical factors, preoperative flexion contractures were less (5.2° ± 5.7° vs. 10.5° ± 6.1°, p < 0.001) and the range of motion was greater (124.7° ± 11.5° vs. 117.6° ± 5.4°, p = 0.041) in the de novo hyperextension group than in the control group. Among the radiographic parameters, preoperative and postoperative JLCA were greater (8.1° ± 4.4° vs. 6.1° ± 3.5°, p = 0.002, 1.0° ± 1.3° vs. 0.2° ± 0.8°, p < 0.001, respectively), postoperative PTSA was greater (3.7° ± 2.0° vs. 3.3° ± 1.6°, p < 0.001) and preoperative and postoperative PCO were less in the hyperextension group than in the control group (26.3 mm ± 3.3mm vs. 29.1 mm ± 3.2mm, p < 0.001, 26.4 mm ± 3.2mm vs. 29.1 mm ± 3.0mm, p < 0.001, respectively). In multivariate analysis, the degree of medial soft tissue release [odds ratio (OR) 2.25, p= 0.001], mechanical axis change after surgery [OR 2.28, p < 0.001], preoperative JLCA [OR 1.14, p = 0.002], and preoperative and postoperative PCO [OR 0.79, p < 0.001, OR 0.79, p < 0.001, respectively] were the factors associated with the development of de novo hyperextension after PS TKA. Conclusion: An increased degree of medial soft tissue release, pre- and postoperative JLCA, a change in the mechanical axis and a decreased pre- and postoperative PCO were risk factors of de novo hypertension. To avoid de novo hyperextension after posterior stabilized type TKA, careful soft tissue balancing should be undertaken in patients with these risk factors who have a large range of motion despite advanced osteoarthritis. In addition, residual medial-lateral laxity measured by postoperative JLCA in supine X-ray can predict the development of hyperextension after TKA in these patients.목적: 수술 후 과신전은 인공 슬관절 전치환술 이후 발생하는 치료가 어려운 드문 합병증으로 수술 직후 발생한 과신전이 아닌 추시에 따라 서서히 발생한 과신전에 대해서는 연구된 바가 적다. 본 논문에서는 후방 십자인대 대치형 인공 슬관절 전치환술에서 수술 이후 신생 과신전을 일으키게 되는 임상적, 방사선학적 인자를 알아보고자 하였다. 대상과 방법: 본 연구는 후향적으로 85례의 신생 과신전 발생한 환자 군과 나이, 성별, 집도의, 임플란트 종류를 85례 매칭하여 대조군을 설정한 후 임상적, 방사선학적 지표(기계적 축, 관절선 수렴각, 후방 경골 경사, 대퇴후방과 오프셋, 감마 각)에서 두 군 사이의 차이를 알아보았고 이분형 로지스틱 회귀분석을 통하여 신생 과신전에 기여한 임상적, 방사선학적 지표에 대하여 분석하였다. 결과: 신생 과신전 발생 그룹에서 수술 전 굴곡구축이 더 작았으며 (p < 0.001), 더 큰 운동범위 (p < 0.001)를 보였다. 방사선학적 지표에서는 수술 전/후 관절선 수렴각이 컸고 (각각 p = 0.002, p < 0.001), 수술 후 후방 경골 경사(델타 각)가 컸으며 (p < 0.001), 수술 전/후 대퇴후방과 오프셋이 작았다. (각각 p < 0.001, p < 0.001) 이분형 로지스틱 회귀분석에서도 다중공선성을 고려하여 수술 전/후 대퇴후방과 오프셋을 각각 따로 분석한 결과 모두 내측 연부조직 유리가 많이 된 경우 (p = 0.016, 0.014), 기계적 축을 많이 교정한 경우 (p < 0.001, p < 0.001), 수술 전 관절선 수렴각이 컸던 경우(p = 0.023, p = 0.023)에 신생 과신전을 발생시킨 경우와 연관된다는 결과를 보였다. 결론: 본 연구에서 내측 연부조직 유리를 많이 한 경우, 기계적 축을 많이 교정한 경우, 수술 전/후 관절선 수렴각이 큰 경우, 대퇴후방과 오프셋이 작은 경우가 신생 과신전의 위험인자로 밝혀졌다. 따라서 수술자는 진행된 퇴행성 변화가 있음에도 수술 전 운동범위가 큰 환자의 인공 슬관절 전치환술에 있어서 해당 위험인자가 있는 경우 수술에 주의하여야 한다. 수술 후 측정한 관절선 수렴각이 크게 측정된 환자들에선 이런 이완이 남은 경우로 추후 이로부터 신생 과신전의 발생을 예측할 수 있을 것이다.Abstract.................................................................................................................i Contents .............................................................................................................. iii List of Tables........................................................................................................ iv List of Figures...................................................................................................... v Introduction .......................................................................................................... 1 Materials and Methods.......................................................................................... 3 Results.................................................................................................................. 9 Discussion .......................................................................................................... 19 References.......................................................................................................... 23 Abstract in Korean.............................................................................................. 26Maste

Technological Innovation and Economic Growth: The Case of Latin America

지금까지 라틴아메리카의 낮은 경제성장은 주로 수입대체정책이 중심이 되는 무역정책, 낙후된 금융부문, 노동시장의 경직성, 정부부문의 비대와 비효율성, 그리고 여기서 파생되는 재정적자와 높은 물가상승률 등에서 그 원인을 찾았다. '워싱턴 컨센서스(Washington Consensus)'에서도 특히 이러한 점들에 중점을 두어 정책권고를 하였다. 그러나 워싱턴 컨센서스에 따라 이러한 부문에 대한 상당한 구조개혁이 이루어졌음에도 불구하고 라틴아메리카 국가들의 경제성장은 눈에 띄게 높아지지 않았다. 그 동안의 개혁은 물가상승을 억제하고 외국인직접투자를 유치하는 데는 성공하였으나 삶의 질을 향상시키고 빈곤을 퇴치하는데는 아무런 도움이 되지 못하였다. 이러한 결과로 인하여 라틴아메리카 경제를 다시 성장가도에 올려놓을 대안을 여러 기관과 학자들이 제시하고 있으나 현재에는 아무런 컨센서스에 도달하지 못하고 있다. 그 대안들 중에는 워싱턴 컨센서스를 조금 수정하여 사회 안전망을 확충하고, 근로자의 권리를 강화하는 등의 요소를 포함하여야 한다는 대안에서부터, 워싱턴 컨센서스와 정반대의 방향에서 개방화를 중지하고 다국적기업의 활동을 제한하여야 한다는 대안까지 제시되고 있다. 그러나 이러한 정책대안 외에도 최근에는 개발도상국에서의 기술혁신의 중요성을 부각시켜 거시경제적인 정책 이외에도 정부와 민간부문이 협력하여 기술혁신과 새로운 생산활동을 촉진하여야 한다는 주장이 제기되고 있다.

멕시코, 에너지 수출국에서 에너지 수입국으로

멕시코는 산유국으로서 수출과 국가재정의 상당부분을 석유에 의존해 왔다. 2008년 전체 수출에서 석유가 차지하는 비중은 17%에 달했으며, 정부재정에서 석유산업이 차지하는 비중은 37%를 초과하였다. 그 동안 석유는 멕시코 경제와 멕시코 정부를 지탱하는 가장 중요한 자원이었다. 멕시코 정부는 석유로부터 거두어들이는 손쉬운 재정수입에 지나치게 의존하여 석유 이외의 세원을 확보하는데 충분한 노력을 기울이지 않았으며, 오히려 석유 부문에 투자하여야할 지원까지 정부지출로 소짆아ㅕㅆ다. 그 결과 석유 탐사에 대한 투자 부족으로 멕시코의 석유 매장량과 생산량이 급격하게 줄어들고 있으며, 머지않은 장래에 멕시코가 석유 수입국으로 전환될 것이라는 예상까지 나오고 있는 실정이다. 석유 생산량의 감소는 재정수입을 감소시켜 국가경쟁에도 극심한 압박을 줄 것으로 예상된다. 본 연구에서는 멕시코 석유 부문의 이러한 동향과 문제를 살펴보고, 멕시코 정부가 이러한 문제를 어떻게 해결하려고 하는지 살펴보려고 한다

Non-Common External Tariffs and Trade Diversion: The Case of Mercosur

본 논문은 무역전환효과를 남미공동시장(Mercosur)의 회원국별로 분석한 결과 무역전환효과가 역외국으로부터의 수입에 부과하는 관세에 따라 결정된다는 것을 보여준다. 역내수출과 역외수출간의 수출유사성지수와 각국의 수입에서 회원국으로부터의 수입이 차지하는 비중을 분석하여, 역외국가에 대한 관세가 높은 브라질에서 무역전환효과가 가장 강하게 발생한 대신 역외국가에 대한 관세가 낮은 칠레에서는 무역전환효과가 발생하지 않았다는 것을 보여준다. 아르헨티나는 그 중간이었다고 할 수 있다. 아르헨티나에서는 지역통합으로 인한 무역전환효과가 발생하지 않았거나 발생하였더라도 브라질에서보다는 훨씬 적게 발생하였다고 할 수 있다. 따라서 지역통합을 추구할 때 대외관세가 낮을수록 무역전환효과가 적게 발생한다는 것을 남미공동시장의 아르헨티나와 브라질, 칠레의 사례에서 알 수 있다. 따라서 정책적인 시사점은 지역경제통합을 추구할 때 무역전환효과를 줄이기 위해서 대외공동관세를 가능한 한 낮게 책정해야 한다는 것이다.By analyzing the trade diversion effect in each of the Mercosurs member countries, this paper shows that trade diversion effect depends on the import tariffs on non-member countries. In 1990s, trade diversion effect was strong in Brazil, whose import tariffs on non-member countries were high, whereas it was weak in Chile, whose import tariff on non-member countries were relatively low. Argentina was an intermediate case. Even if Mercosur aimed at a common market, it was far from being one. There were many exceptions to common external tariff, and this was the cause of the different trade diversion effect across member countries. Before the formation of Mercosur, Argentinas trade regime was relatively open whereas Brazil maintained some protection. This tendency remained even after Mercosur was established. Chile participated in Mercosur as an associate member and was able to maintain low tariff rates. Such different tariff levels among member countries resulted in different trade diversion effect, the strongest effect appearing in Brazil, whose tariffs on non-member countries were the highest. The implication of the analysis is that even in regional trade agreements with common external tariff, as common markets, the trade diversion effect may differ greatly across countries if there are many exceptions to the common external tariff. A policy implication is that, when establishing a regional trade agreement, each member country has to reduce the tariff rates to the non-member countries in order to minimize the trade diversion effect

El Desarrollro Económico y la Política de Comercio Libre en Chile

무역자유화에 있어서 하나의 흥미로운 사실은 무역자유화정책이 경제위기의 시기에 시작되는 경우가 많고, 또 무역자유화의 과정에서 경제위기에 봉착하여 무역자유화정책이 실패로 끝나게 되는 경우도 많다는 것이다. 브라질이 1964년에 시작한 무역자유화정책은 1974년 석유위기가 발생하자 실패로 돌아가고 말았다. 1974년 경제위기시에 시작된 아르헨티나의 무역자유화는 1982년 외채위기로 종말을 고하였다. 이 외에도 많은 라틴아메리카 국가들이 비슷한 경험을 갖고 있다. 그 이유는 먼저 경제위기시에 하나의 돌파구로서 무역개방을 추진하게 되는 경향이 있기 때문이다. 전통적으로 수입대체정책을 추구하던 라틴아메리카 국가들이 경제위기에 직면하면 그 동안의 수입대체정책으로부터 무언가 새로운 정책으로 전환하려 하였다는 것은 어쩌면 당연한 결과일지도 모른다. 물론 1960년대에는 멕시코를 비롯한 많은 나라들이 경제 성장의 둔화라는 위기의 해결책으로써 더 강력한 수입대체정책을 추진하였던 경우도 많았다. 그러나 1970년대 이후에는 무역자유화라는 정책이 새로운 경제발전 전략으로 어느 정도 받아들여지기 시작하였다. 그러나 이러한 무역개방의 추세는 1982년의 외채위기라는 경제위기로 다시 반전되었다. 외채를 갚기 위해 외화가 필요하였던 대부분의 라틴아메리카 국가들은 수입을 통제함으로써 외화를 절약하여 이것으로 외채를 갚아 나갔던 것이다

Análisis de la Influencia del TLC en la Industria Mexicana

이 글의 목적은 북미자유무역협정에 합의된 내용을 살펴보고 이것이 멕시코산업에 미칠 수 있는 영향을 살펴보는 데 있다. 북미자유무역협정의 조항에는 일반적인 제반 부문에 적용되는 것과 특수한 부문에만 적용되는 것이 있다. 여기서는 일단 전반적인 산업에 적용되는 관세 철폐, 원산지규정 및 투자에 대한협정 등을 분석한 다음 특수한 분야에 적용되는 규정 등을 살펴보려 한다

Study on Fertility of Seoul Inhabitants

The investigation concerning Productivity. Natural and artificial abortion. Contraception and Still-birth was carried out by special survey through home visiting of 1. 160 households out of 32 Dong from March to August 1962 and also special survey through reporting to Donghoe from 1960 to 1961. It was summerized as follows: 1. The most frequent age of marriage was 25 in male and 20 in female but mean age of marriage 27.7 in former and 21.7 in latter 2. In average difference of marital age between men and women the pairs of 3 years elder male than female were at most. 3. Number of production was at most in age group of 25-29. 4. Month specific number of production showed less in April and May and more in September and November. 5. Natural abortion occured frequently in 3 months pregnancy and the frequency was 1 time at most. 6. Artificial abortion showed same as above. 7. Artificial abortion was performed at most in age group of 25-29. 8. The age of practising contraception was at most in age group of 25-29 and the reason of contraception was Unneccessary of kids" at around half. and the methods of contraception was instruments (Condom & Pessary) at most. 9. Production as well as Still-birth showed decreasing in 1961 compared with 1960. The productivity was 32.6 in 1960 and 27.7 in 1961
