    [[abstract]]中文摘要 本研究目的乃針對照顧服務員之工作滿意度探討影響其留任之因素,而以個人基本屬性、機構屬性為中介變項。採結構式問卷調查,為描述性、推論性之研究,自2003年3月至4月收集資料。以台北縣、市立案之15家長期照護之機構, 196位照顧服務員為研究對象。 研究結果在照服員基本屬性上:長期照護之照護主力以女性居多。平均年齡為45.35歲。教育程度以高中職最多。照服員自覺家中收入「好」以上有65.8﹪;是家中主要經濟來源者為60.8﹪。 家庭狀況,以已婚者最多;擁有子女者為88.8﹪,與子女同住者為79.1﹪。宗教信仰上以佛教為主。照服員之服務年資以五年以上最多。而40.8﹪的人在過去有照護工作之經驗,大部分的照服員自覺健狀況良好。每天自家中到機構上班所需之交通時間以30分內最多。 機構屬性:本次參與研究之機構佔床率大多在90﹪以上。機構之排班方式採三班制、無固定班別居多。每週上班時數以44小時為主。機構對於人員配置上大多符合法定機構設置最低標準。福利措施中退休制度、績效獎金制度、升等制度有待管理制度上再努力,以增加照服員之保障。 工作滿意度對留任意願有正面之影響,照服員之留任意願為「有意願」之傾象。而針對本研究提出以下之建議: 一、針對照服員之需求:(一)合理公平的對待照服員,「落實勞基法」。(二)經營者重視照服員之角色功能與工作職責。(三)重申照服員有責任於工作中維護自我健康,應依據正確作業原則使用各項工作輔具。 二、因應外在滿意度提升之建議: (一) 管理方面:1.機構將部分清潔工作採外包制。2建立支持管道與團體。3管理者鼓勵照服員實際運用支持管道與團體。 (二) 政策方面:政府政策於擬定機構照護人力標準時,宜以住民實際情況為標準,以符合實際照護需求。 2. 建議政策與機構,應建立非正式與正式之訓練或晉升制度 關鍵字:照顧服務員、留任、長期照護機構[[abstract]]Abstract The purpose of this research is focusing on the working satisfaction which affects the willingness to retented in the long term care facilities of Health Care Aides, based on the changes in the attribution of person, and facility mediating variable. The data were collected between March and April 2003, and used a questionnaire form for its depicting and inferring research purpose. It used196 Health Care Aides of 15 long term care facilities located in Taipei City and Taipei country as research samples. The outcome is based on the attribution of person: women are the main forces of Health Care Aides. The average age is 45.4. The majority people have high school education. The house income of the 65.8 ﹪of Health Care Aides is above average. And 60.8 % of Health Care Aides'' salaries are their families'' main incomes. In terms of marriage situation, married people are the majority and 88.8% of them have children, 79.1% of them live with their children. Most of Health Care Aides are Buddhism, and have worked as Health Care Aide over five years. 40.8% among them have experiences in taking care of patients, and most of the Health Care Aides consciously feel conformable about their health. The everyday commuting time from home to facilities is about 30 minutes of the most. The attribution of facility: The participating facilities have over 90% of bed usage rate. The arrangement of shifts is three-shift, and varied shift is the most. The working-hour is 44 hours per week. In the worker arrangement, most of the facilities follow the minimum-working standard under the state''s law. However, the benefits of retirement plan, reward plan, and promotion could need more efforts in order to enhance the protection of workers. Most of the Health Care Aides have positive attitude toward their jobs, and are willing to continue to work. However, the satisfaction of their jobs affects their intentions. According to the data, this research may suggest the following: 1. In connection with the need of Health Care Aide: (1) The Health Care Aide should be treated equally, and the labor law should be carried out precisely. (2) The manager should be serious about the roles, functions and duties of the Health Care Aide. (3) The manager should restate that Health Care Aids should work under practice standards, and take care of themselves while they are in working. 2. Comments on the extrinsic factors that affect the promotion of working satisfaction: (1) Management factor: (Ⅰ)Facilities should hire outsider to do the cleaning works. (Ⅱ)Facilities should establish support channels, and groups to support their workers. (Ⅲ)The manager should encourage the usage of these support channels, and groups. (2) Policy factor: (Ⅰ) Government should consider the factor of current resident when drawing up the policies of practice standards.     Factors Associated with Nurse Aides Who Willingness to Retention: Example of Long-Term Care Facilities in Taipei

    [[abstract]]Taiwan has an aging population, which means the need for long-term care is increasing. Long-term care is a human-oriented service industry whose main manpower is provided by health care workers; therefore, the purpose of this research was to focus on the factors associated with nursing aides who are willing to stay on the job in order to improve the quality of patient care. Data were collected between March and April 2003 using cross-sectional study design and based on a questionnaire. Research samples were taken from 196 nurse aides from 15 long-term care facilities located in districts of Taipei city. The results showed that (1) there was a statistically significant negative correlation between nursing aides' level of education and willingness to stay on the job(r =-.24, p<.01); (2 )there was a statistically significant positive correlation between nursing aides' work experience (r=.16, p<.05), health condition (r =.18, p<.05) , and willingness to stay on the job; (3) there was a statistically significant positive correlation between bed occupancy rates and nursing aides' willingness to stay on the job (r=.31, p<.01); (4) there was a statistically significant positive correlation between nursing aides' job satisfaction and willingness to stay on the job (r =.63, p<.01). According to the results of regression analysis, the predictors of willingness to stay on in long-term care are level of education, bed occupancy rates, and job satisfaction (40% of total variance). It is hoped that this information will be applied to future policy development of retention strategies in long-term care in Taiwan
