Fen Bilimleri Öğretmenleri için “Teknolojik Pedagojik Alan Bilgisi Temelli Argümantasyon Uygulamaları” Eğitiminin Değerlendirilmesi
In science education, creating learning environments supported with technology and students’ use of theory-evidence coordination when expressing their ideas is emphasized regarding the development of students’ scientific reasoning, critical thinking, decision making skills, and etc. In this process, great responsibilities are fallen to teachers as planner and designer of a learning environment. In this study, it is aimed to assess the training which aims the development of science teachers’ technological pedagogical content knowledge (TPACK) through argumentation practices. In this context; this study evaluated the science teachers’ argumentation skills, self-efficacy perceptions towards TPACK and the teachers’ views about the training. 37 science teachers working at different cities in Turkey participated in the one group pre-test post-test experimental training study, which was lasted 54 hours during a week. The training is composed of both hand and minds on argumentation practices based on TPACK. The participants joined different activities such as collaborative group works, drama, modeling, thematic games, art activities, problem-based learning, field trips, observation and workshops. In the study, Argumentation Test, TPACK Self-Efficacy Belief Scale were used as data collection tools. At the end of the training written views of science teachers towards activities were taken. In the light of the findings, this training was effective on the participants’ self-efficacy levels towards technological pedagogical content knowledge. Moreover, this training resulted in a positive change in the participants’ views about how a statement could be accepted as an argument. However, the increase in scores of argumentation skills was not significant. Additionally almost all of the participants stated that they found the activities useful and can use in their classes. By considering these results, some suggestions were given.
Real-world efficacy and safety of Ledipasvir plus Sofosbuvir and Ombitasvir/Paritaprevir/Ritonavir +/- Dasabuvir combination therapies for chronic hepatitis C: A Turkish experience
Background/Aims: This study aimed to evaluate the real-life efficacy and tolerability of direct-acting antiviral treatments for patients with chronic hepatitis C (CHC) with/without cirrhosis in the Turkish population.Material and Methods: A total of 4,352 patients with CHC from 36 different institutions in Turkey were enrolled. They received ledipasvir (LDV) and sofosbuvir (SOF)+/- ribavirin (RBV) ombitasvir/paritaprevir/ritonavir +/- dasabuvir (PrOD)+/- RBV for 12 or 24 weeks. Sustained virologic response (SVR) rates, factors affecting SVR, safety profile, and hepatocellular cancer (HCC) occurrence were analyzed. Results: SVR12 was achieved in 92.8% of the patients (4,040/4,352) according to intention-to-treat and in 98.3% of the patients (4,040/4,108) according to per-protocol analysis. The SVR12 rates were similar between the treatment regimens (97.2%-100%) and genotypes (95.6%-100%). Patients achieving SVR showed a significant decrease in the mean serum alanine transaminase (ALT) levels (50.90 +/- 54.60 U/L to 17.00 +/- 14.50 U/L) and model for end-stage liver disease (MELD) scores (7.51 +/- 4.54 to 7.32 +/- 3.40) (p<0.05). Of the patients, 2 were diagnosed with HCC during the treatment and 14 were diagnosed with HCC 37.0 +/- 16.0 weeks post-treatment. Higher initial MELD score (odds ratio [OR]: 1.92, 95% confidence interval [CI]: 1.22-2.38; p=0.023]), higher hepatitis C virus (HCV) RNA levels (OR: 1.44, 95% CI: 1.31-2.28; p=0.038), and higher serum ALT levels (OR: 1.38, 95% CI: 1.21-1.83; p=0.042) were associated with poor SVR12. The most common adverse events were fatigue (12.6%), pruritis (7.3%), increased serum ALT (4.7%) and bilirubin (3.8%) levels, and anemia (3.1%).Conclusion: LDV/SOF or PrOD +/- RBV were effective and tolerable treatments for patients with CHC and with or without advanced liver disease before and after liver transplantation. Although HCV eradication improves the liver function, there is a risk of developing HCC.Turkish Association for the Study of The Liver (TASL
Evaluation of nutritional status in pediatric intensive care unit patients: the results of a multicenter, prospective study in Turkey
IntroductionMalnutrition is defined as a pathological condition arising from deficient or imbalanced intake of nutritional elements. Factors such as increasing metabolic demands during the disease course in the hospitalized patients and inadequate calorie intake increase the risk of malnutrition. The aim of the present study is to evaluate nutritional status of patients admitted to pediatric intensive care units (PICU) in Turkey, examine the effect of nutrition on the treatment process and draw attention to the need for regulating nutritional support of patients while continuing existing therapies.Material and MethodIn this prospective multicenter study, the data was collected over a period of one month from PICUs participating in the PICU Nutrition Study Group in Turkey. Anthropometric data of the patients, calorie intake, 90-day mortality, need for mechanical ventilation, length of hospital stay and length of stay in intensive care unit were recorded and the relationship between these parameters was examined.ResultsOf the 614 patients included in the study, malnutrition was detected in 45.4% of the patients. Enteral feeding was initiated in 40.6% (n = 249) of the patients at day one upon admission to the intensive care unit. In the first 48 h, 86.82% (n = 533) of the patients achieved the target calorie intake, and 81.65% (n = 307) of the 376 patients remaining in the intensive care unit achieved the target calorie intake at the end of one week. The risk of mortality decreased with increasing upper mid-arm circumference and triceps skin fold thickness Z-score (OR = 0.871/0.894; p = 0.027/0.024). The risk of mortality was 2.723 times higher in patients who did not achieve the target calorie intake at first 48 h (p = 0.006) and the risk was 3.829 times higher in patients who did not achieve the target calorie intake at the end of one week (p = 0.001). The risk of mortality decreased with increasing triceps skin fold thickness Z-score (OR = 0.894; p = 0.024).ConclusionTimely and appropriate nutritional support in critically ill patients favorably affects the clinical course. The results of the present study suggest that mortality rate is higher in patients who fail to achieve the target calorie intake at first 48 h and day seven of admission to the intensive care unit. The risk of mortality decreases with increasing triceps skin fold thickness Z-score

Severe Metabolic Acidosis and Pulmonary Edema: A Near-Drowning Case
Drowning is defined as a situation which results in the death of the patient within the first 24 hours, while “near-drowning” refers to all conditions of submersion which does not cause death but brings about morbidity and damaging effects. A twenty-five-month-old boy was found immobile in a bathtub filled with water. The patient was transported to the intensive care unit from the emergency department where he vas ventilated with a bag-mask, then intubated, and diagnosed with respiratory insufficiency, and his respiration was supported by mechanical ventilator. Physical examination of the patient showed that he was hypothermic, not breathing spontaneously, his heart rate was 120/min., blood pressure 100/80 mmHg, he was unconscious, and Glasgow Coma scale was 5. The first blood analysis results were mmol/L. The pulmonary edema regressed, cardiopulmonary hemodynamic returned to normal. Then, at the 48th hour he was weaned from mechanical ventilator support. The patient was discharged from the hospital in good health, without negative cognitive and motor symptoms on the 10th day. The present case report aimed to highlight the importance of basic and advanced life support in cases of near-drowning which happens frequently among children, and to discuss the management of pulmonary edema and other complications
Comparison Of Less Invasive Surfactant Delivery Techniques In Respiratory Distress Syndrome
Background: This study was conducted to evaluate and compare the effectiveness of newly developed minimal invasive methods for surfactant administration (LISA by using Magill forceps, INSURE)
Methods: The research was conducted on 15 patients between 685-2100 gr weight, requiring surfactant administration. Poractant alpha, dose 200 mg/kg, was administered to patients with spontaneous breathing on continuous positive airway pressure support. LISA was used to administer surfactant to 7 of 15 patients, whereas 8 of them had surfactant administered using the INSURE method.
Results: The data regarding the delivery method, birth weight, gender, prenatal history, usage of steroids, APGAR scores and other data about the patients until hospital discharge were recorded. Desaturation and bradycardia for 3-4 minutes were observed in 71% (5/7) of LISA and 87.5% (7/8) of INSURE cases. Bradycardia was observed in 4 of 8 patients from the INSURE group and positive ventilation was needed. From the INSURE group, 3 patients died; one of 15 had ROP and this patient was from the INSURE group. No difference was found among the groups regarding required duration of O2 and days of hospital stay (p<0.05). The thin catheter group had significantly fewer median days on mechanical ventilation and lower rate of mortality (p<0.05).
Conclusions: When there is no need for mechanical ventilation, intubation for surfactant administration was determined not to be necessary in the present study. LISA was observed to be the best method for surfactant administration among the methods without intubation
The structural, electronic, elastic, phonon, and thermodynamical properties of the SmX (X = P, Sb, Bi) compounds
WOS: 000274222000019A detailed theoretical study of structural, electronic, elastic, phonon, and thermodynamical properties of SmX (X = P, Sb, Bi) compounds is presented by performing ab-initio calculations based on density-functional theory using the Vienna ab-initio simulation package (VASP). For describing the interaction between electrons and ions, the projector-augmented wave (PAW) method is used. The generalized-gradient approximation (GGA) is chosen for the exchange-correlation functional. The calculated structural parameters, such as the lattice constant with and without spin-polarization (SP), bulk modulus, cohesive energy, second-order elastic constants, the electronic band structures and the related total density of states with and without SP and spin-orbit coupling (SOC) are presented. The high-pressure phase of all compounds is investigated and phase transition pressure from NaCl (B1) to high-pressure phase is determined. For B1 structure, the calculated values of lattice constants are compared with the available experimental and other theoretical data and bulk modulus values are compared with available other theoretical results. In order to gain further information, we have calculated Zener anisotropy factor (A), Poisson's ratio (nu), Young's modulus (E), shear modulus (C), elastic wave velocities, Debye temperature, phonon frequencies and one-phonon density of states for B1 structure of these three compounds. The temperature dependent behavior of heat capacity and entropy for these compounds in B1 phase is also presented.Gazi University [05/2008-16]This work is supported by Gazi University Research-Project Unit under Project No: 05/2008-16
The structural, elastic and thermodynamic properties of intermetallic compound CeGa2
WOS: 000297743800024The structural, elastic and thermodynamic characteristics of CeGa2 compound in the AlB2 (space group: P6/mmm) and the omega trigonal (space group: P-3m1) type structures are investigated using the methods of density functional theory within the generalized gradient approximation (GGA). The thermodynamic properties of the considered structures are obtained through the quasi-harmonic Debye model. The results on the basic physical parameters, such as the lattice constant, the bulk modulus, the pressure derivative of bulk modulus, the phase-transition pressure (P (t) ) from P6/mmm to P-3m1 structure, the second-order elastic constants, Zener anisotropy factor, Poisson's ratio, Young's modulus, and the isotropic shear modulus are presented. In order to gain further information, the pressure and temperature-dependent behavior of the volume, the bulk modulus, the thermal expansion coefficient, the heat capacity, the entropy, Debye temperature and Gruneisen parameter are also evaluated over a pressure range of 0-6 GPa and a wide temperature range of 0-1800 K. The obtained results are in agreement with the available experimental and the other theoretical values
