Il en découle une gouvernance plutôt efficace par la création d'une situation gagnant-gagnant pour les acteurs impliqués, au moins à court terme, tant que la coexistence avec les variétés conventionnelles peut demeurer. Étude sur Les Perceptions et les Attentes des Professionnels de Santé, des Bénévoles et des Familles des Malades dans le Cadre de la Prise en Charge des Soins Palliatifs

This qualitative study, conducted in three metropolitan areas, Champagne Ardenne, Ile-de-France, and Languedoc-Roussillon explores current palliative care practices in 37 facilities, both for dependent elderly (nursing homes) and for adults and children with severe disabilities (MAS and IME). The study confronts the viewpoints of the different actors that revolve around a patient: doctors, nurses and other health professionals (physiotherapists, occupational therapists, psychologists, etc.), relatives, volunteers and patients themselves. 40 cases of sick children and adults were observed. From the analysis of the interviews, it appears that the palliative approach is still not fully adopted by teams that are reluctant to deal with the end of life and postpone the most difficult cases. Among the priorities to be developed are: the harmonization of the initial training of caregivers and ongoing training and the mobilization of specialist palliative care that could better support and assist caregivers in this sector

La présente étude porte sur les besoins d'accompagnement d'entrepreneurs qui innovent de façon responsable dans le domaine agro-alimentaire au Burkina Faso. Il s'agit d'identifier d'une part les décalages entre ces besoins et les services support existants et d'autre part les façons dont la recherche peut jouer un rôle plus important pour renforcer les dynamiques d'innovation responsable en cours qui répondent de façon satisfaisante aux enjeux du développement durable

Voluntary Counselling, HIV Testing and Sexual Behaviour Among Patients with Tuberculosis in a Rural District of Malawi.

OBJECTIVES: A study was conducted in new patients registered with tuberculosis (TB) in a rural district of Malawi in order to 1) verify the acceptability of voluntary counselling and testing for human immunodeficiency virus (HIV) infection; 2) describe sexual behaviour and condom use; and 3) identify socio-demographic and behavioural risk factors associated with 'no condom use'. DESIGN: Cross-sectional study. METHODS: Consecutive patients diagnosed with TB between January and December 2000 were offered voluntary counselling and HIV testing (VCT) and were subsequently interviewed. RESULTS: There were 1,049 new TB patients enrolled in the study. Of these, 1,007 (96%) were pre-test counselled, 955 (91%) underwent HIV testing and 912 (87%) were post-test counselled; 43 (4%) patients refused HIV testing. The overall HIV infection rate was 77%. Of all HIV-positive TB patients, 691 (94%) were put on cotrimoxazole. There were 479 (49%) TB patients who reported sexual encounters, of whom only 6% always used condoms. Unprotected sex was associated with having TB symptoms for over 1 month, having had less than 8 years of school education, being single, divorced or widowed or having sex with the same partner. CONCLUSIONS: Offering VCT to TB patients in this setting has a high acceptance rate and provides an opportunity to strengthen and integrate TB and HIV programmes

Émergences et paradoxes dans la nébuleuse de l'accompagnement de la pluriactivité en Languedoc-Roussillon

La communication a pour but de présenter et d'analyser comment est prise en compte la pluriactivité dans l'offre de conseil et d'accompagnement en Languedoc-Roussillon. Partant de l'hypothèse que la pluriactivité occupe une place de plus en plus grande dans ces organisations, notre méthodologie a consisté à dresser un inventaire des organisations de l'accompagnement, puis, par l'analyse de leurs documents de communication et une série d'enquêtes, à repérer des pratiques et des postures, pour in fine caractériser les changements que la prise en compte de la pluriactivité induit au sein des organisations inventoriées. La recherche révèle une plus forte reconnaissance de la pluriactivité par les acteurs du conseil et de l'accompagnement agricole. Elle atteste d'une disjonction marquée entre les pratiques et les discours des organisations, et permet finalement de formuler des hypothèses sur l'émergence de véritables dispositifs d'accompagnement spécifiques aux pluriactifs autour de slogans, de pratiques, de réseaux, d'outils mais surtout de changements de postures. Accompaniment and action-research follow steps having numerous resemblances (constructivism, importance of doubt and uncertainty, reflexivity, project frame), which tend to blur the borders between research process and accompaniment action. To be able to produce knowledge on the accompaniment, a device of piloting of the action-research was set up, completed along the way to integrate a reflexive analysis by the researcher on its own practice of accompaniment.
