The inspiratory capacity/total lung capacity ratio as a predictor of survival in an emphysematous phenotype of chronic obstructive pulmonary disease.
BackgroundForced expiratory volume in 1 second (FEV1) grades severity of COPD and predicts survival. We hypothesize that the inspiratory capacity/total lung capacity (IC/TLC) ratio, a sensitive measure of static lung hyperinflation, may have a significant association with survival in an emphysematous phenotype of COPD.ObjectivesTo access the association between IC/TLC and survival in an emphysematous phenotype of COPD.MethodsWe performed a retrospective analysis of a large pulmonary function (PF) database with 39,050 entries, from April 1978 to October 2009. Emphysematous COPD was defined as reduced FEV1/forced vital capacity (FVC), increased TLC, and reduced diffusing capacity of the lungs for carbon monoxide (DLCO; beyond 95% confidence intervals [CIs]). We evaluated the association between survival in emphysematous COPD patients and the IC/TLC ratio evaluated both as dichotomous (≤25% vs >25%) and continuous predictors. Five hundred and ninety-six patients had reported death dates.ResultsUnivariate analysis revealed that IC/TLC ≤25% was a significant predictor of death (hazard ratio [HR]: 2.39, P<0.0001). Median survivals were respectively 4.3 (95% CI: 3.8-4.9) and 11.9 years (95% CI: 10.3-13.2). Multivariable analysis revealed age (HR: 1.19, 95% CI: 1.14-1.24), female sex (HR: 0.69, 95% CI: 0.60-0.83), and IC/TLC ≤25% (HR: 1.69, 95% CI: 1.34-2.13) were related to the risk of death. Univariate analysis showed that continuous IC/TLC was associated with death, with an HR of 1.66 (95% CI: 1.52-1.81) for a 10% decrease in IC/TLC.ConclusionAdjusting for age and sex, IC/TLC ≤25% is related to increased risk of death, and IC/TLC as a continuum, is a significant predictor of mortality in emphysematous COPD patients

IntroductionExposure to secondhand smoke (SHS) is associated with occult obstructive lung disease as evident by abnormal airflow indices representing small airway disease despite having preserved spirometry (normal forced expiratory volume in 1 s-to-forced vital capacity ratio, FEV1/FVC). The significance of lung volumes that reflect air trapping in the presence of preserved spirometry is unclear.MethodsTo investigate whether lung volumes representing air trapping could determine susceptibility to respiratory morbidity in people with SHS exposure but without spirometric chronic obstructive pulmonary disease, we examined a cohort of 256 subjects with prolonged occupational SHS exposure and preserved spirometry. We elicited symptom prevalence by structured questionnaires, examined functional capacity (maximum oxygen uptake, VO2max) by exercise testing, and estimated associations of those outcomes with air trapping (plethysmography-measured residual volume-to-total lung capacity ratio, RV/TLC), and progressive air trapping with exertion (increase in fraction of tidal breathing that is flow limited on expiration during exercise (per cent of expiratory flow limitation, %EFL)).ResultsRV/TLC was within the predicted normal limits, but was highly variable spanning 22%±13% and 16%±8% across the increments of FEV1/FVC and FEV1, respectively. Respiratory complaints were prevalent (50.4%) with the most common symptom being ≥2 episodes of cough per year (44.5%). Higher RV/TLC was associated with higher OR of reporting respiratory symptoms (n=256; r2=0.03; p=0.011) and lower VO2max (n=179; r2=0.47; p=0.013), and %EFL was negatively associated with VO2max (n=32; r2=0.40; p=0.017).ConclusionsIn those at risk for obstruction due to SHS exposure but with preserved spirometry, higher RV/TLC identifies a subgroup with increased respiratory symptoms and lower exercise capacity
A New Phytochemical Screening Programme used for Crops grown with Organic and Conventional Methods
A broad screening programme, covering the most general phytochemical groups of compounds, was developed on the basis of Thin Layer Chromatography (TLC). A total of 46 TLC systems, comprising 26 derivatization reagents, 3 stationary phases, and 4 mobile phases, were included. The TLC systems were classified according to the groups of phytochemical compounds detected: Alcohols and phenolic compounds; Carbohydrates; N-containing compounds; Organic acids and lipids; P-containing compounds; S-containing compounds, and Terpenoids. Furthermore, one group of TLC systems detected compounds from several of the mentioned groups.
The screening programme was applied in the screening of potatoes (S. tuberosum L.), peas (P. sativum L.), kale (B. oleracea L.), carrots (D. carota L.), and apples (M. domestica Borkh.), cultivated with combinations of organic and conventional methods for plant protection and nutrient supply, for phytochemical differences (biomarkers).
Distinctive phytochemical differences were found between the differently cultivated samples of these crops. In peas and carrots only one biomarker was found. In peas the biomarker was related to the soil conditions, while the biomarker in carrots was related to the use of pesticides. In potato, two biomarkers related to the use of pesticides were found. Three biomarkers were found in kale. Two of these could be related to the use of pesticide, while the last was related to either fertiliser or soil conditions. Several biomarkers were found apples, but a relation to the cultivation methods was not clear. Three of the biomarkers in apples could be related to either the use of pesticides or fertiliser, while no conclusions could be drawn from the other biomarkers found.
The results of the screening programme form the basis for a potential development of a kit to detect whether crops are organically- or conventionally cultivated. Furthermore, the results from this part and other parts of the project "Organic food and health – a multigenerational animal experiment" provide basis for the selection of which secondary compounds to quantify by specific chemical analysis, isolate, and/or structure elucidation
Productivity estimated as in vivo chlorophyll a fluorescence and biochemical composition in Chlorella fusca (Chlorophyta) grown in outdoor thin-layer cascades
The photosynthetic performance by using in vivo chlorophyll a florescence, biomass composition and productivity of three cultures of Chlorella fusca (Chlorophyta) grown in thin-layer cascades (TLCs) in different locations and time of the year were evaluated. Biomass productivity was higher in 120 m-1 S/V TLC (3.65 g L-1 d-1) compared at 27 m-1 S/V TLC (0.08-0.15 g L-1 d-1). Online monitoring of in vivo chlorophyll fluorescence provided data with high temporal resolution. The simultaneous measure of the PAR irradiance and the effective quantum yield together to the determination of absorption coefficient allowed the determination of daily Electron transport rate (ETR), what led to the estimation of biomass productivity by using conversion factors of mol photons per oxygen produced and relation between carbon assimilated and oxygen produced. The relation between estimated productivity and measured productivity estimated as g l-1 d-1 was 0.90 with R2=0.94. Final biochemical composition was similar in summer culture independent of the S/V of the TLC i.e 30-37% DW lipids, 30-34 % DW proteins and 16-19% DW starch. Lower accumulation was found in autumn cultures, ~23, 28% and 13% DW for lipid and starch content, respectively. The S/V ratio of the system was a key factor to obtain high biomass and storage product productivity. This strain was able to grow outdoors in TLC, exhibiting very high productivity according to the optimal photosynthetic performance and accumulating high lipid and protein content. It is shown as first time a good estimation of algal biomass productivity by using daily integrated values of electron transport rate (ETR) converted to estimated biomass productivity.Universidad de Málaga. At the second phase of THE SPHINX PROJECT, a hybrid TLC-PAIR METER was designed for measuring high energy neutrino sources (E upsilon * TeV), searching high energy muon sources (E mu TeV) and measuring muon group (E mu 1 TeV). The principle of PAIR METER has been already proposed. In this TLC-PAIR METER, electromagnetic shower induced by cosmic ray muons are detected using TL (Thermoluminescence) sheets with position counters
