    Optimizing Anterior Implant Outcome Immediately After Implant Placement and Grafting by Using Patient’s Extracted Teeth: A Case Report

    Optimizing anterior implant outcome using patient’s extracted teeth. Successful implant treatment in the anterior maxilla is predicated on providing a functional and esthetically acceptable outcome. As well, achieving a predictable ideal emergence profile and soft tissue contour is expected. Placing immediate provisional restorations utilizing the patient’s modified extracted teeth can facilitate initiate facial soft tissue contouring while precluding undue pressure on the surgical site during the early healing phase. This allows for a one stage surgical procedure, fixed provisionalization and ideal tissue contouring. This clinical report presents the use of a patient’s modified extracted permanent teeth as bonded fixed provisional restorations placed immediately after implant placement and bone grafting. The gingival-lingual and cingulum tooth structure was recontoured to allow no contact with the implant or bone graft material. A combination of cement and screw-retained definitive restorations were fabricated with contours reflecting the tissue contours established by the interim restoration

    Friction Testing of a New Ligature

    Objective: To determine if American Orthodontics\u27 (AO) new, experimental ligature demonstrates less friction in vitro when compared to four other ligatures on the market. Methods: Four brackets were mounted on a custom metal fixture allowing an 0.018-in stainless steel wire attached to an opposite fixture with one bracket to be passively centered in the bracket slot. The wire was ligated to the bracket using one of five types of ligatures including the low friction test ligatures (AO), conventional ligatures (AO), Sili–TiesTM, Silicone Infused Ties (GAC), Synergy® Low-Friction Ligatures (RMO), and SuperSlick ligatures (TP Orthodontics). Resistance to sliding was measured over a 7 mm sliding distance using a universal testing machine (Instron) with a 50 Newton load cell and a crosshead speed of 5 mm/min. The initial resistance to sliding (static) was determined by the peak force needed to initiate movement and the kinetic resistance to sliding was taken as the force at 5 mm of wire/bracket sliding. Fifteen unique tests were run for each ligature group in both dry and wet (saliva soaked for 24 hours with one drop prior to testing) conditions. Results: In the dry state, the SuperSlick ligature demonstrated more static friction than all of the other ligatures, while SuperSlick and Sili–Ties demonstrated more kinetic friction than the AO conventional, AO experimental and Synergy ligatures. In the wet condition, SuperSlick and the AO experimental ligature demonstrated the least static friction, followed by the AO conventional and Sili–Ties. The most static friction was observed with the Synergy ligatures. In the wet condition, the SuperSlick, AO experimental and AO conventional exhibited less kinetic friction than the Sili-Ties and Synergy ligatures. Conclusions: AO\u27s experimental ligature exhibits less friction in the wet state than conventional ligatures, Sili–Ties and Synergy and is comparable to the SuperSlick ligature. These preliminary results suggest that the AO experimental ligature and the SuperSlick ligature create less friction, but direct conclusions regarding in vivo performance cannot be made and randomized controlled clinical trials are needed to determine if these ligatures have clinical significance in treatment efficiency

    A phantom based evaluation on the effects of patient breathing motion on Stereotactic Body Radiotherapy treatment volumes

    Aim: The aim of the study was to design an upper body phantom to mimic the movement of the lesion inside the lungs during a breathing cycle. Phantom design included an assessment of the motion observed for lung lesions, identification of suitable phantom materials as well as design of a motorized arm to mimic the movements observed inside the lung area of the phantom. Introduction: Expansion margins are added to clinical target volumes contoured by Oncologists in order to safeguard against under- or over-treatment of the target volume. They are designed to account for errors during setup, inaccuracies on the linear accelerator, and movement of targets inside the patient. If the margins are too small, there is a risk that the lesion/target may not receive the necessary dose, due to being partially missed. On the other hand, if the margins are too wide, the lesion will be covered, but normal tissue may receive unnecessary dose, resulting in additional side effects to the patient. Assessment of the impact of these margins is not possible in a static phantom and the availability of a low-cost motorized phantom would assist in the validation of these margins. Method: Previously treated patients' 4D CT scanning data were used to quantify the amount of movement seen for lesions within the lung. A phantom was then designed and built in an attempt to mimic both patient anatomy and movement. Materials were identified to replicate anatomical shape and densities of various organs in the thorax, as seen on CT scan data. Two treatment planning systems (Monaco, (Elekta) and Eclipse (Varian)) were used to determine the dosimetric characteristics of the materials. This was compared to actual dose as delivered by a linear accelerator (Elekta Synergy). Results: Paths were calculated from the breathing cycles during the 4D-CT scan sets and templates designed to mimic these movements. A thorax phantom was built with the appropriate materials suitable and matched densities to replicate a human thorax. Comparing transmission for these materials on a linear accelerator for 6MV and 10MV energy, the deviation from planned versus measured dose varied between 1.67% to 3.32% and 0.45% to 2.30%, respectively for the silicon material and between 0.77% to 3.22% and 0.17% to 2.57% for the 3D printed bone for 6MV and 10MV. iv Conclusion: The measurements done on the linear accelerator matched closely with the calculated values on the treatment planning system for transmission through the materials in the customised phantom. Various proposals were put forward to mimic the movement of the targets within the lung regions. However, it was not possible to manufacture a mechanically based working model due to the small movements observed (<5mm). It is recommended that a robotic solution be investigated as alternative to mimic these small movements

    A new stroke mimic prediction scale in a stroke center with a high thrombolysis rate

    Patients reaching the emergency department (ED) with symptoms of acute ischemic stroke (AIS) may be affected by a stroke mimics (SMs). A prompt clinical diagnosis could avoid unnecessary thrombolysis. We evaluated a new and rapid approach, the Santa Maria Nuova-Stroke Mimic (SMN-SM) scale, to improve a prompt clinical diagnosis. 340 consecutive patients admitted to the ED with suspected AIS were evaluated. The final diagnosis was: AIS in 267 (78,5%) and SMs in 73 (21,5%) patients. Multivariate logistical analysis showed that the following features – lack of facial paralysis, dizziness, migraine, seizure disorders, blood pressure <150, cognitive impairment, and female sex – were significantly more abundant in patients with SMs than in AIS. To each of these features we assigned a numerical score and we performed a receiver operating characteristic analysis. When the score of the scale was above 8 (cut-point), we obtained a specificity of 93% and a sensitivity of 56% for a SM diagnosis. Thus, the SMN-SM scale seems a rather useful tool to improve SMs diagnosis

    Endothelial Progenitors as Tools to Study Vascular Disease

    Endothelial progenitor cells (EPCs) have great clinical value because they can be used as diagnostic biomarkers and as a cellular therapy for promoting vascular repair of ischaemic tissues. However, EPCs also have an additional research value in vascular disease modelling to interrogate human disease mechanisms. The term EPC is used to describe a diverse variety of cells, and we have identified a specific EPC subtype called outgrowth endothelial cell (OEC) as the best candidate for vascular disease modelling because of its high-proliferative potential and unambiguous endothelial commitment. OECs are isolated from human blood and can be exposed to pathologic conditions (forward approach) or be isolated from patients (reverse approach) in order to study vascular human disease. The use of OECs for modelling vascular disease will contribute greatly to improving our understanding of endothelial pathogenesis, which will potentially lead to the discovery of novel therapeutic strategies for vascular diseases

    Primary total knee arthroplasty (TKA) is a widespread procedure to address end stage osteoarthritis with good results, clinical outcomes, and long-term survivorship. Although it is frequently performed in elderly, an increased demand in young and active people is expected in the next years. However, a considerable dissatisfaction rate has been reported by highly demanding patients due to the intrinsic limitations provided by the TKA. Bicruciate-retaining (BCR) TKA was developed to mimic knee biomechanics, through anterior cruciate ligament preservation. First-generation BCR TKA has not gained popularity due to its being a challenging technique and having poor survival outcomes. Thanks to implant design improvement and surgeon-friendly instrumentation, second-generation BCR TKA has seen renewed interest. This review will focus on surgical indications, kinematical basis, clinical results and latest developments of second-generation BCR TKA

    Hypothalamic Hamartomas: Evolving Understanding and Management

    Hypothalamic hamartomas (HH) are rare, basilar developmental lesions with widespread comorbidities often associated with refractory epilepsy and encephalopathy. Imaging advances allow for early, even prenatal, detection. Genetic studies suggest mutations in GLI3 and other patterning genes are involved in HH pathogenesis. About 50-80% of children with HH suffer from severe rage and aggression and a majority of cases exhibit externalizing disorders. Behavioral disruption and intellectual disability may predate epilepsy. Neuropsychological, sleep and endocrine disorders are typical. The purpose of this paper is to provide a summary of the current understanding of HH, and to highlight opportunities for future research

    An investigation into the effects of commencing haemodialysis in the critically ill

    &lt;b&gt;Introduction:&lt;/b&gt; We have aimed to describe haemodynamic changes when haemodialysis is instituted in the critically ill. 3 hypotheses are tested: 1)The initial session is associated with cardiovascular instability, 2)The initial session is associated with more cardiovascular instability compared to subsequent sessions, and 3)Looking at unstable sessions alone, there will be a greater proportion of potentially harmful changes in the initial sessions compared to subsequent ones. &lt;b&gt;Methods:&lt;/b&gt; Data was collected for 209 patients, identifying 1605 dialysis sessions. Analysis was performed on hourly records, classifying sessions as stable/unstable by a cutoff of &gt;+/-20% change in baseline physiology (HR/MAP). Data from 3 hours prior, and 4 hours after dialysis was included, and average and minimum values derived. 3 time comparisons were made (pre-HD:during, during HD:post, pre-HD:post). Initial sessions were analysed separately from subsequent sessions to derive 2 groups. If a session was identified as being unstable, then the nature of instability was examined by recording whether changes crossed defined physiological ranges. The changes seen in unstable sessions could be described as to their effects: being harmful/potentially harmful, or beneficial/potentially beneficial. &lt;b&gt;Results:&lt;/b&gt; Discarding incomplete data, 181 initial and 1382 subsequent sessions were analysed. A session was deemed to be stable if there was no significant change (&gt;+/-20%) in the time-averaged or minimum MAP/HR across time comparisons. By this definition 85/181 initial sessions were unstable (47%, 95% CI SEM 39.8-54.2). Therefore Hypothesis 1 is accepted. This compares to 44% of subsequent sessions (95% CI 41.1-46.3). Comparing these proportions and their respective CI gives a 95% CI for the standard error of the difference of -4% to 10%. Therefore Hypothesis 2 is rejected. In initial sessions there were 92/1020 harmful changes. This gives a proportion of 9.0% (95% CI SEM 7.4-10.9). In the subsequent sessions there were 712/7248 harmful changes. This gives a proportion of 9.8% (95% CI SEM 9.1-10.5). Comparing the two unpaired proportions gives a difference of -0.08% with a 95% CI of the SE of the difference of -2.5 to +1.2. Hypothesis 3 is rejected. Fisher’s exact test gives a result of p=0.68, reinforcing the lack of significant variance. &lt;b&gt;Conclusions:&lt;/b&gt; Our results reject the claims that using haemodialysis is an inherently unstable choice of therapy. Although proportionally more of the initial sessions are classed as unstable, the majority of MAP and HR changes are beneficial in nature
