Older patients' prescriptions screening in the community pharmacy: development of the Ghent Older People's Prescriptions community Pharmacy Screening (GheOP3S) tool
Background: Ageing of the population often leads to polypharmacy. Consequently, potentially inappropriate prescribing (PIP) becomes more frequent. Systematic screening for PIP in older patients in primary care could yield a large improvement in health outcomes, possibly an important task for community pharmacists. In this article, we develop an explicit screening tool to detect relevant PIP that can be used in the typical community pharmacy practice, adapted to the European market.
Methods: Eleven panellists participated in a two-round RAND/UCLA (Research and Development/University of California, Los Angeles) process, including a round zero meeting, a literature review, a first written evaluation round, a second face-to-face evaluation round and, finally, a selection of those items that are applicable in the contemporary community pharmacy.
Results: Eighteen published lists of PIP for older patients were retrieved from the literature, mentioning 398 different items. After the two-round RAND/UCLA process, 99 clinically relevant items were considered suitable to screen for in a community pharmacy practice. A panel of seven community pharmacists selected 83 items, feasible in the contemporary community pharmacy practice, defining the final GheOP3S tool.
Conclusion: A novel explicit screening tool (GheOP3S) was developed to be used for PIP screening in the typical community pharmacy practice
Administration of Long-Acting Injections
Expanding the scope of pharmacy practice demonstrates that the profession has been successful at improving public health. Despite being a late adopter, New York\u27s limited experience with vaccines has improved immunization rates and lowered rates of disease. During emergencies, the 2017-2018 flu season for example, the state has turned to pharmacists to go beyond what the pharmacy practice law permits, in this case enabling immunizations in pediatric patients.1 This illustrates recognition of untapped potential within the profession to contribute to the public health. Another opportunity for pharmacists to enhance the public health is embodied in a Bill introduced in the New York State Legislature that would amend the pharmacy practice law to enable administration of long-acting injectables designed to treat mental health disorders including schizophrenia and substance use disorder ( SUD ). The goal of this paper is to review the proposed amendment, the relevant background, and to discuss the implications for patients and the pharmacy profession
Medical Marijuana and Pharmacy Practice
By way of the Compassionate Care Act of 2014, New York has become the 23rd state to create a process that will permit patients suffering from a “serious condition” to receive medical marijuana (or “cannabis”). Among those, it is the second state to prohibit the crude delivery system of smoking (Minnesota), and the third to involve pharmacists in the dispensing process (Connecticut and Minnesota). Because virtually every practicing pharmacist in New York will be caring for patients receiving some form of cannabinoid therapy, it is important to discuss the basic outlines established by the law and regulations
Implementing Pharmacy Informatics in College Curricula: The AACP Technology in Pharmacy Education and Learning Special Interest Group
Many professional organizations have initiatives to increase the awareness and use of informatics in the practice of pharmacy. Within education we must respond to these initiatives and make technology integral to all aspects of the curriculum, inculcating in students the importance of technology in practice. This document proposes 5 central domains for organizing planning related to informatics and technology within pharmacy education. The document is intended to encourage discussion of informatics within pharmacy education and the implications of informatics in future pharmacy practice, and to guide colleges of pharmacy in identifying and analyzing informatics topics to be taught and methods of instruction to be used within the doctor of pharmacy curriculum
Development of a platform to align education and practice: bridging academia and the profession in Portugal
Limited fitness for practice may result from a mismatch between education and practice. Aiming to meet the common interests of academics and practitioners, the Portuguese Pharmaceutical Society (PPS) developed the Education and Practice Platform (EPP). The EPP includes one representative from each pharmacy faculty, and all Councils of Speciality Boards of Practice. Brainstorming with involved parties enabled sharing of interests, concerns and identifying a common path. Aims, mission, vision and values were set. The EPP's mission is to: act as an enabler to foster the quality and adequacy of education through sharing best practices, ultimately leading to facilitate professional integration, and to foster quality development in teaching practices with recognition for autonomy in freedom to teach and to learn. Its vision is an alignment of education and practice with the PPS' statutes to ensure validation of the competences defined for each practice area, and compliance with international guidance. Key performance indicators (KPIs) were set. Activities developed include the creation of a national forum to discuss education and practice, development of workshops on teaching methods and pharmacy internships, enhanced representation in international events and response to global and national requests. Ongoing work focuses on the creation of a common training framework in hospital and community pharmacy practice adapted to Portugal. The EPP is a worldwide case study, encouraging the development of discussion contributing to an open climate of sharing best practices, indirectly leading to foster a better alignment between education and practice. Community Pharmacy: an untapped patient data resource
As community pharmacy services become more patient centred they will be increasingly reliant on access to good quality patient information. This paper describes how the information which is currently available in community pharmacies can be used to enhance service delivery and patient care. With integration of community pharmacy and medical practice records on the horizon the opportunities this will provide are also considered. The community pharmacy held patient medication record, which is the central information repository, has been used to identify non-adherence, to prompt the pharmacist to clinically review prescriptions, identify patients for additional services and to identify those patients at greater risk of adverse drug events. Whilst active recording of patient consultations for treatment over the counter may improve the quality of consultations and information held, the lost benefits of anonymity afforded by community pharmacies needs to be considered. Recording of pharmacy staff activities enables workload to be monitored, remuneration to be justified and critical incidents to be learned from but is not routine practice. Centralisation of records between community pharmacies enables practices to be compared and consistent problems to be identified. By integrating pharmacy and medical practice records, patient behaviour with respect to medicines can be more closely monitored and should prevent duplication of effort. When using patient information stored in a community pharmacy it is however important to consider the reason why information was recorded in the first instance and whether it is appropriate to use it for a different purpose without additional patient consent. Community pharmacies currently have access to large amounts of information which if stored and used appropriately can significantly enhance the quality of provided services and patient care. Integrating records increases opportunities to enhance patient care yet further. Whilst community pharmacies have significant amounts of information available to them this is frequently untapped
