Self-Reported Occupational Exposure to HIV and Factors Influencing its Management Practice: A Study of Healthcare Workers in Tumbi and Dodoma Hospitals, Tanzania.
Blood borne infectious agents such as hepatitis B virus (HBV), hepatitis C virus (HCV) and human immune deficiency virus (HIV) constitute a major occupational hazard for healthcare workers (HCWs). To some degree it is inevitable that HCWs sustain injuries from sharp objects such as needles, scalpels and splintered bone during execution of their duties. However, in Tanzania, there is little or no information on factors that influence the practice of managing occupational exposure to HIV by HCWs. This study was conducted to determine the prevalence of self-reported occupational exposure to HIV among HCWs and explore factors that influence the practice of managing occupational exposure to HIV by HCWs in Tanzania. Self-administered questionnaire was designed to gather information of healthcare workers' occupational exposures in the past 12 months and circumstances in which these injuries occurred. Practice of managing occupational exposure was assessed by the following questions: Nearly half of the HCWs had experienced at least one occupational injury in the past 12 months. Though most of the occupational exposures to HIV were experienced by female nurses, non-medical hospital staff received PEP more frequently than nurses and doctors. Doctors and nurses frequently encountered occupational injuries in surgery room and labor room respectively. HCWs with knowledge on the possibility of HIV transmission and those who knew whom to contact in event of occupational exposure to HIV were less likely to have poor practice of managing occupational exposure. Needle stick injuries and splashes are common among HCWs at Tumbi and Dodoma hospitals. Knowledge of the risk of HIV transmission due to occupational exposure and knowing whom to contact in event of exposure predicted practice of managing the exposure. Thus provision of health education on occupational exposure may strengthen healthcare workers' practices to manage occupational exposure
Can disability studies contribute to client-centred occupational therapy practice?
Occupational therapists frequently cite a ‘client-centred’ approach as a fundamental aspect of their practice. However, there are many examples in the narratives of disabled people that suggest that the health and social care services they experience do not quite meet this aspiration. The authors propose that an understanding of
disability from disabled people’s perspectives is elemental to client-centred practice and that knowledge of the academic discipline of disability studies can contribute to authentic client-centred occupational therapy
Routine dipstick urinalysis in daily practice of Belgian occupational physicians
BACKGROUND: Little work has been done to assess the quality of health care and the use of evidence-based methods by occupational physicians in Belgium. Therefore, the main objective is to describe one aspect of occupational health assessments, namely the common use of dipstick urinalysis, and to compare the current practice with international guidelines.
METHODS: A self-administered questionnaire was mailed to 211 members of the Scientific Association of Occupational Medicine in the Dutch speaking part of Belgium.
RESULTS: A total of 120 occupational physicians responded, giving a response rate of 57%. Dipstick urinalysis was a routine investigation for the vast majority of physicians (69%). All test strips screened for protein and in 90% also for blood. Occupational health services offered clinical tests to satisfy customer wants as international guidelines do not recommend screening for haematuria and proteinuria in asymptomatic adults. A lack of knowledge concerning positive testing and referral criteria was demonstrated in almost half of the study participants.
CONCLUSIONS: Belgian occupational physicians still routinely perform dipstick testing although there is no evidence to support this screening in healthy workers. To practice evidence-based medicine, occupational physicians need more instruction and training. Development and implementation of more guidelines is not only of use for the individual practitioner, it may also enhance professionalization and efficiency of occupational health care
Southern occupational therapies: Emerging identities, epistemologies and practices
Indexación: Web of Science; Scielo.For over a decade, debates in occupational therapy have extended into the profession's theoretical foundations as well as epistemological underpinnings thereof A series of proposals have emerged from around the world that aim to link the definition of occupational therapy, its knowledge and practices to contemporary social, political, cultural and economic conditions. Contributing to this is the increasing precariousness of the global social life, the economic crises of many social systems, and the deterioration of the ecological environment.
The current paper critically reflects on the historical conditions that shape the institution of occupational therapy, particularly in the regions of South America and Africa. This involves a political, ethical, and epistemological rethinking of the foundations that underpin identities, knowledge and practices of occupational therapy and their effects on society. These foundations may favour processes of exclusion and ahistorical and individualist views of human occupation, as opposed to social perspectives expressed in collective occupations and human rights promoting practices. The authors propose to problematise the construction of a professional identity, knowledge and practices of occupational therapy, emphasising the need for a liberating discipline, committed to and acting alongside people and communities who are in situations of social exclusion. Putting it to the test
THE BPS Steering Committee on Test Standards (SCTS) has already developed and introduced standards for testing in occupational settings - the Level A and Level B Certificates of Competence in Occupational Testing - to encourage responsible test use by psychologists and nonpsychologists alike. Next month, at the Society's Annual Conference in London, the SCTS will launch the Level A Certificate of Competence in Educational Testing. It is envisaged that the Certificate will help to raise standards of test use and promote a wider knowledge of psychology among test users in education
Comparing Learning Platform Impact on Low Vision Education for Occupational Therapists
This pilot study examines the impact of face-to-face, remote, and hybrid learning platforms on satisfaction, confidence, and knowledge-application of occupational therapy practitioners during a synchronous low vision continuing education program. Fifteen participants were divided into three groups, each corresponding to one learning platform. They engaged in two 45-min learning sessions and completed pre, post, and follow-up surveys to measure the impact of the learning platform on the dependent variables of satisfaction, confidence, and knowledge application. No significant differences were found between learning platforms for the three variables, but improvements from pre to follow-up survey were found to be significant for confidence and knowledge application for all groups. These findings indicate that similar education provided to occupational therapy practitioners may result in improved confidence and knowledge application to clinical practice from the beginning to the end of the educational program, despite the learning platform. Flexibility with online learning options increased participation and adherence rates. Synchronous remote and hybrid learning platforms may be as effective as traditional face-to-face methods, specifically with increasing practitioner confidence and knowledge application. Remote options may reduce peer interactions but increase flexibility and convenience with scheduling for program scalability and accessibility
Careers advisors’ opinion of the College of Occupational Therapists Careers Video
Using a mixed methodology, this study investigated the effectiveness of the
College of Occupational Therapists’ careers video in increasing the knowledge
of six careers advisers about the profession and obtained their views of the
video as a promotional tool. To quantify the advisers’ knowledge, a pre-test
post-test questionnaire was used before and after viewing the video. A short
qualitative interview then ascertained their impressions of the video. Despite a
high baseline knowledge, viewing the video was effective in increasing it. The
advisers had mixed opinions about the video, suggesting that it could be
improved and clarified
An Understanding of Religious Doing: A Photovoice Study
The ability to participate in everyday activities that hold meaning and value is a determinant of health and wellbeing. Occupational therapists work with people when health and social barriers limit this valued participation. However a challenge persists in including religious practice or ‘doing’ within therapy, with many occupational therapists feeling ill-equipped and reluctant to address religious doing. The study reported here examines religious doing within the lives of participants from a number of faith traditions. A photovoice method is used, with participants discussing photographs that they have taken to describe their religious doing. Data are analyzed using a phenomenological reflective lifeworld approach. Findings are grouped into six themes and are explored using both verbatim quotes from transcripts and some of the photographs taken by participants. A reflective description of the core aspects of participants’ practical religious doing is constructed from the data, with the intention of providing occupational therapists with a basis from which to begin to consider practical religious doing within the lives of their clients. It is proposed that occupational therapists do not need an in-depth knowledge of theology and doctrine but rather an understanding of key and familiar occupational principles such as person-centred habits and routines, and community connectedness
The Dr Elizabeth Casson Memorial Lecture 2018: Occupational stories from a global city
The Dr Elizabeth Casson Memorial Lecture 2018, given on June 12th 2018 at the 42nd Annual Conference and Exhibition of the Royal College of Occupational Therapists, held at the Belfast Waterfront, Belfast, UK.
This lecture aims to set out the potential for the global occupational therapy profession to exchange knowledge for social transformation practice. It identifies the profession’s concern with narratives as a vehicle for a socially critical approach to occupation, which can be used to negotiate intervention and action. Drawing on examples from literature, history and service users, the paper suggests that narrative provides a means for relating the value of occupation beyond professional boundaries to capture popular imagination and demand for the profession. Examples are given of the critical discussion of the everyday impact of health inequity, and in addressing diversity both in the profession and engaging service users.
My lecture concludes that occupational therapy is a global network with the population of a city, and thus represents a community that can be a vibrant voice for social transformation through occupation through a reciprocal exchange of narrative. This is a collective and dialogical process which can draw on the experiences of both southern and northern hemispheres
