Intrapancreatic accessory spleen false positive to 68Ga-Dotatoc: case report and literature review
Background: Intrapancreatic accessory spleen (IPAS) is an uncommon finding of pancreatic mass. Differential diagnosis with pancreatic tumor, especially with non-functional neuroendocrine tumor (NF-NET), may be very hard and sometimes it entails unnecessary surgery. A combination of CT scan, MRI, and nuclear medicine can confirm the diagnosis of IPAS. 68-Ga-Dotatoc PET/CT is the gold standard in NET diagnosis and it can allow to distinguish between IPAS and NET. Case presentation: A 69-year-old man was admitted to our hospital for an incidental nodule in the tail of the pancreas with focal uptake of 68-Ga-dotatate at PET/CT. NET was suspected and open distal splenopancreatectomy was performed. Pathologic examination revealed an IPAS. Conclusion: This is the second IPAS case in which a positive 68Ga-Dotatoc uptake led to a false diagnosis of pancreatic NET. Here is a proposal of a literature review
Roll out the red carpet and they will come : investment promotion and FDI inflows
As red tape in host countries and information asymmetries constitute a
significant obstacle to investment flows across international borders, an important
policy question is: what can aspiring FDI destinations do to reduce such barriers? This
study uses newly collected data on 124 countries to examine the effects of investment
promotion on inflows of US FDI. We test whether sectors explicitly targeted by
investment promotion agencies in their efforts to attract FDI receive more investment
in the post-targeting period, relative to the pre-targeting period and non-targeted
sectors. The results of our analysis are consistent with investment promotion leading
to higher FDI flows to countries in which red tape and information asymmetries are
likely to be severe. The data suggest that investment promotion works in developing
countries but not in industrialized economies
PENGEMBANGAN LKPD INTERAKTIF BERBASIS LIVEWORKSHEET MATERI SUMBER DAYA ALAM IPAS KELAS IV SEKOLAH DASAR
Observations and interviews with fourth-grade instructors at SD Negeri Sungkai 1, Kab. Banjar, revealed that they continued to use the school's conventional LKPDs and textbooks during IPAS lessons. The implementation of this technique resulted in a lack of enthusiasm among students for acquiring knowledge in IPAS. Teachers have not yet implemented instructional resources that could enhance students' understanding of natural resources. Consequently, pupils frequently encounter difficulties in comprehending these concepts, which appear abstract to them. In response to this issue, the study created an interactive Learning Kit for Personal Development (LKPD) specifically designed for active students who prefer collaborative learning. The objective of this study is to develop an interactive LKPD (Learning and Teaching Materials) product using Liveworksheet, with a specific focus on the natural resources theme in IPAS.
The study employed the ADDIE methodology, which consists of five stages: analysis, design, development, implementation, and evaluation. The first stage entailed evaluating the current requirements in the field. The second phase of the project focused on designing the LKPD, which included tasks like designing the cover, organizing the content, and choosing an appropriate design for the LKPD. In the third phase, experts in the subject area and teaching materials transformed the designed LKPD into a product and validated it. The fourth step involved implementing the LKPD in practical settings. The final stage involved conducting an assessment by distributing response questionnaires to both classroom instructors and students.
The material experts' validation process yielded a highly positive score of 91.67%, while the teaching material experts' validation process achieved a score of 81.25%. The validation results confirm the appropriateness of utilizing the interactive LKPD based on the Liveworksheet in an elementary school education setting. The following trials generated highly favorable feedback, with a 90.40% rating from teachers and a 96% rating from students. The results of these experiments demonstrate that the interactive LKPD is extremely captivating and functions as a successful instructional tool for IPAS, specifically in relation to natural resources, for students in the fourth grade
Investment promotion agencies: do they work?
In this paper, we examine the role of investment promotion agencies (IPAs) in promoting outward FDI from Japan and Korea. Looking at two home countries enables us to control for both country-pair time-invariant characteristics and host country time-varying characteristics. Our empirical results suggest that home-country IPAs tend to be more effective in promoting outward FDI in politically risky host countries.
However, this finding depends on whether the home-country firm is listed or unlisted. Connexin 43 plays a role in pulmonary vascular reactivity in mice
Pulmonary arterial hypertension (PAH) is a chronic condition characterized by vascular remodeling and increased vaso-reactivity. PAH is more common in females than in males (~3:1). Connexin (Cx)43 has been shown to be involved in cellular communication within the pulmonary vasculature. Therefore, we investigated the role of Cx43 in pulmonary vascular reactivity using Cx43 heterozygous (Cx43+/−) mice and 37,43Gap27, which is a pharmacological inhibitor of Cx37 and Cx43. Contraction and relaxation responses were studied in intra-lobar pulmonary arteries (IPAs) derived from normoxic mice and hypoxic mice using wire myography. IPAs from male Cx43+/− mice displayed a small but significant increase in the contractile response to endothelin-1 (but not 5-hydroxytryptamine) under both normoxic and hypoxic conditions. There was no difference in the contractile response to endothelin-1 (ET-1) or 5-hydroxytryptamine (5-HT) in IPAs derived from female Cx43+/−mice compared to wildtype mice. Relaxation responses to methacholine (MCh) were attenuated in IPAs from male and female Cx43+/− mice or by pre-incubation of IPAs with 37,43Gap27. Nω-Nitro-L-arginine methyl ester (l-NAME) fully inhibited MCh-induced relaxation. In conclusion, Cx43 is involved in nitric oxide (NO)-induced pulmonary vascular relaxation and plays a gender-specific and agonist-specific role in pulmonary vascular contractility. Therefore, reduced Cx43 signaling may contribute to pulmonary vascular dysfunction
San Diego: Major Providers Pursue Countywide Networks and New Patient Care Models
San Diego has long been a geographically well-defined health care market with high managed care penetration and a consolidated provider sector. In recent years, hospital systems have faced increasing cost pressures as commercial health plans have responded to employer demands for more affordable premiums. Safety-net providers expanded capacity to deal with the large Medi-Cal expansion that began in 2014, but continue to grapple with how to provide adequate care for a new enrollee population that is far sicker, with more complex medical and social service needs, than the providers' previous patient base.Key developments include:While the hospital market remained largely stable in recent years, most of the smaller hospitals have been losing volume and struggling financially.Major systems are pursuing population health strategies and increasingly using provider-sponsored health plans to take full risk for more patients.The challenges of independent practice are leading many primary care physicians to choose employment at system-affiliated groups.In the two years since Medicaid eligibility was first expanded under the ACA, San Diego's Medi-Cal managed care enrollment almost doubled to 700,000. Value of implantable loop recorders in patients with structural or electrical heart disease
Purpose: In patients with structural heart disease (SHD) or inherited primary arrhythmia syndrome (IPAS), the occurrence of unexplained syncope or palpitations can be worrisome as they are at increased risk of sudden cardiac death. An implantable loop recorder (ILR) can be a useful diagnostic tool. Our purpose was to compare the diagnostic yield, arrhythmia mechanism, and management in patients with SHD, patients with IPAS, and those without heart disease. Methods: Retrospective single-center study in consecutive patients who underwent an ILR implantation. Results: Between March 2013 and December 2016, a total of 94 patients received an ILR (SHD, n = 20; IPAS, n = 14; no SHD/IPAS, n = 60). The type of symptoms at the time of implantation was similar between groups. During a median follow-up of 10 months, 45% had an ILR-guided diagnosis. Patients with IPAS had a lower diagnostic yield (14%) in comparison to the other groups (no SHD/IPAS 47%, P = 0.03; SHD 60%, P = 0.01, respectively). Furthermore, patients with SHD had a higher incidence of nonsustained VT in comparison to patients without SHD/IPAS (30 versus 3%, P < 0.01). ILR-guided therapy was comparable between groups. In the SHD group, a high proportion (10%) received an implantable cardioverter-defibrillator; however, this was not statistically significantly higher than the other groups (no SHD/IPAS 3%, IPAS 0%, P = 0.08). Conclusions: In comparison to patients without heart disease, the diagnostic yield of an ILR was lower in patients with IPAS and the prevalence of ILR-diagnosed nonsustained VT was higher in patients with SHD
Aftercare of inward foreign direct investment: A case study of South Africa
Attraction of new inward foreign direct investment (FDI) globally, especially in the developing countries, is problematic. Economic development practitioners have recently started to prioritise the retention and growing of existing investments to enhance their economic development agenda. The purpose of this paper is to analyse and investigate the relationship between inward FDI and investment aftercare in South Africa (SA). Only a few studies have been carried out on the topic at the global level and none on SA as far as could be ascertained. A structured questionnaire was used to collate data, and 30 face-to-face interviews were conducted with 16 investment aftercare practitioners and 14 executives from the investment promotion agencies (IPAs) in seven of the nine provinces of SA. The main finding of the study is that although investment aftercare services are essential, they receive limited funding, staff and attention, and have not yet been developed in SA.
