Tracking Foodborne Pathogens from Farm to Table: Data Needs to Evaluate Control Options
Food safety policymakers and scientists came together at a conference in January 1995 to evaluate data available for analyzing control of foodborne microbial pathogens. This proceedings starts with data regarding human illnesses associated with foodborne pathogens and moves backwards in the food chain to examine pathogen data in the processing sector and at the farm level. Of special concern is the inability to link pathogen data throughout the food chain. Analytical tools to evaluate the impact of changing production and consumption practices on foodborne disease risks and their economic consequences are presented. The available data are examined to see how well they meet current analytical needs to support policy analysis. A Dynamic Quantitative Microbial Risk Assessment for Norovirus in Potable Reuse System
This study describes the results of a dynamic quantitative microbial risk assessment (QMRA) for norovirus (NoV) that was used to evaluate the relative significance of foodborne, person-to-person, and person-to-sewage-to-person transmission pathways. This last pathway was incorporated into simulated potable reuse systems to evaluate the adequacy of typical treatment trains, operational conditions, and regulatory frameworks. The results confirm that secondary and foodborne transmission dominate the overall risk calculation and that waterborne NoV likely contributes no appreciable public health risk, at least in the scenarios modeled in this study. De facto reuse with an environmental buffer storage time of at least 30 days was comparable or even superior to direct potable reuse (DPR) when compound failures during advanced treatment were considered in the model. Except during these low-probability failure events, DPR generally remained below the 10−4 annual risk benchmark for drinking water. Based on system feedback and the time-dependent pathogen load to the community\u27s raw sewage, this model estimated median raw wastewater NoV concentrations of 107–108 genome copies per liter (gc/L), which is consistent with high-end estimates in recent literature
Global and regional source attribution of Shiga toxin-producing Escherichia coli infections using analysis of outbreak surveillance data
Shiga toxin-producing Escherichia coli (STEC) infections pose a substantial health and economic burden worldwide. To target interventions to prevent foodborne infections, it is important to determine the types of foods leading to illness. Our objective was to determine the food sources of STEC globally and for the six World Health Organization regions. We used data from STEC outbreaks that have occurred globally to estimate source attribution fractions. We categorised foods according to their ingredients and applied a probabilistic model that used information on implicated foods for source attribution. Data were received from 27 countries covering the period between 1998 and 2017 and three regions: the Americas (AMR), Europe (EUR) and Western-Pacific (WPR). Results showed that the top foods varied across regions. The most important sources in AMR were beef (40%; 95% Uncertainty Interval 39-41%) and produce (35%; 95% UI 34-36%). In EUR, the ranking was similar though with less marked differences between sources (beef 31%; 95% UI 28-34% and produce 30%; 95% UI 27-33%). In contrast, the most common source of STEC in WPR was produce (43%; 95% UI 36-46%), followed by dairy (27%; 95% UI 27-27%). Possible explanations for regional variability include differences in food consumption and preparation, frequency of STEC contamination, the potential of regionally predominant STEC strains to cause severe illness and differences in outbreak investigation and reporting. Despite data gaps, these results provide important information to inform the development of strategies for lowering the global burden of STEC infections

Foodborne illnesses are among the most widespread public health issues, killing about
2.2 million people annually worldwide, and costing hundreds of billions of US dollars
for governments, companies, families and consumers. In Brazil, foodborne illness in
the home accounts for 44% of identified disease outbreaks and in New Zealand it
represents 27% of notifiable disease outbreaks. Several studies have investigated
aspects of consumer behaviour concerning food safety, but it remains a challenge to
obtain a full picture of critical control points (CCPs) and key factors contributing to
food contamination, pathogen growth or survival, when the food is under the
consumer’s responsibility. This study aimed to assess threats to food safety in the
home in Brazil and New Zealand. From August 2011 to March 2012, survey
questionnaires from 2,775 consumers most responsible for cooking in the home in
Brazil were collected. From September 2012 to November 2012, 658 households in
New Zealand responded to the same survey.
Both surveys found similar CCPs with the potential to threaten food safety in the
domestic environment – food preparation, cooking and handling leftovers. Information
from New Zealand suggests that choosing and purchasing food, and for Brazil food
transportation, are also steps of concern. The age, marital status, gender, ethnicity,
first-aid in response to illness and the way a person learned to cook had a significant
influence in the risky practices of consumers in both countries, suggesting that similar
consumer behaviour concerning food safety can be found in countries of substantially
different degrees of economic development and culture. The young, the men, socioeconomic
minorities, people most susceptible to illness and ethnic groups were people
of most concern, often ranked at-risk, demanding special attention of public health
authorities in both countries. The CCPs of most concern and contributing factors
identified in this study were officially reported in New Zealand, helping to validate the
methodology used in this study and its possible use in other countries. Furthermore,
food safety educational campaigns built on the steps of most concern and groups
ranked at moderate or high risk, have the potential to be most effective in reducing
food poisoning in the home
A Multiorgan Trafficking Circuit Provides Purifying Selection of Listeria monocytogenes Virulence Genes.
Listeria monocytogenes can cause a life-threatening illness when the foodborne pathogen spreads beyond the intestinal tract to distant organs. Many aspects of the intestinal phase of L. monocytogenes pathogenesis remain unknown. Here, we present a foodborne infection model using C57BL/6 mice that have been pretreated with streptomycin. In this model, as few as 100 L. monocytogenes CFU were required to cause self-limiting enterocolitis, and systemic dissemination followed previously reported routes. Using this model, we report that listeriolysin O (LLO) and actin assembly-inducing protein (ActA), two critical virulence determinants, were necessary for intestinal pathology and systemic spread but were dispensable for intestinal growth. Sequence tag-based analysis of microbial populations (STAMP) was used to investigate the within-host population dynamics of wild-type and LLO-deficient strains. The wild-type bacterial population experienced severe bottlenecks over the course of infection, and by 5 days, the intestinal population was highly enriched for bacteria originating from the gallbladder. In contrast, LLO-deficient strains did not efficiently disseminate and gain access to the gallbladder, and the intestinal population remained diverse. These findings suggest that systemic spread and establishment of a bacterial reservoir in the gallbladder imparts an intraspecies advantage in intestinal occupancy. Since intestinal L. monocytogenes is ultimately released into the environment, within-host population bottlenecks may provide purifying selection of virulence genes.IMPORTANCE Listeria monocytogenes maintains capabilities for free-living growth in the environment and for intracellular replication in a wide range of hosts, including livestock and humans. Here, we characterized an enterocolitis model of foodborne L. monocytogenes infection. This work highlights a multiorgan trafficking circuit and reveals a fitness advantage for bacteria that successfully complete this cycle. Because virulence factors play critical roles in systemic dissemination and multiple bottlenecks occur as the bacterial population colonizes different tissue sites, this multiorgan trafficking circuit likely provides purifying selection of virulence genes. This study also serves as a foundation for future work using the L. monocytogenes-induced enterocolitis model to investigate the biology of L. monocytogenes in the intestinal environment
The food safety impact of salt and sodium reduction initiatives
Excessive or high salt or sodium intake is known to cause hypertension and other diseases. Within the United Kingdom voluntary targets for salt reduction have been set and laid out in the Secretary of State responsibility deal. This review considers the options available to food manufacturers to enable them to reduce salt and the potential food safety risks associated with those options. Gaps in research and knowledge within the areas of information supplied to food manufacturers, alternative solutions for salt replacement and the food safety impact of salt reduction are discussed
Economic Assessment of Food Safety Regulations: The New Approach to Meat and Poultry Inspection
USDA is now requiring all Federally inspected meat and poultry processing and slaughter plants to implement a new system called Hazard Analysis and Critical Control Points (HACCP) to reduce potentially harmful microbial pathogens in the food supply. This report finds that the benefits of the new regulations, which are the medical costs and productivity losses that are prevented when foodborne illnesses are averted, will likely exceed the costs, which include spending by firms on sanitation, temperature control, planning and training, and testing. An Analysis of the U.S. Department of Agriculture's Proposal to Allow Irradiation of Meat
Foodborne pathogens in the United States kill thousands and sicken millions each year, despite major efforts by federal agencies and industry to combat the problem. Food irradiation could safely prevent a substantial part of those deaths and illnesses and has been endorsed by all major public health organizations. Yet federal regulations prohibit the use of irradiation to kill pathogens on meat, eggs, and seafood and restrict its use on poultry, and so contribute to avoidable deaths and illnesses. The U.S. Department of Agriculture should expedite its rulemaking to allow irradiation of meat, and it should not require that food labels mention irradiation more prominently than they identify food additives. Most importantly, to boost food irradiation in the marketplace, the Food and Drug Administration should promptly determine that irradiation of any food, including precooked meats, eggs, and seafood, is generally recognized as safe.
Status and potential of bacterial genomics for public health practice : a scoping review
Background: Next-generation sequencing (NGS) is increasingly being translated into routine public health practice, affecting the surveillance and control of many pathogens. The purpose of this scoping review is to identify and characterize the recent literature concerning the application of bacterial pathogen genomics for public health practice and to assess the added value, challenges, and needs related to its implementation from an epidemiologist’s perspective.
Methods: In this scoping review, a systematic PubMed search with forward and backward snowballing was performed to identify manuscripts in English published between January 2015 and September 2018. Included studies had to describe the application of NGS on bacterial isolates within a public health setting. The studied pathogen, year of publication, country, number of isolates, sampling fraction, setting, public health application, study aim, level of implementation, time orientation of the NGS analyses, and key findings were extracted from each study. Due to a large heterogeneity of settings, applications, pathogens, and study measurements, a descriptive narrative synthesis of the eligible studies was performed.
Results: Out of the 275 included articles, 164 were outbreak investigations, 70 focused on strategy-oriented surveillance, and 41 on control-oriented surveillance. Main applications included the use of whole-genome sequencing (WGS) data for (1) source tracing, (2) early outbreak detection, (3) unraveling transmission dynamics, (4) monitoring drug resistance, (5) detecting cross-border transmission events, (6) identifying the emergence of strains with enhanced virulence or zoonotic potential, and (7) assessing the impact of prevention and control programs. The superior resolution over conventional typing methods to infer transmission routes was reported as an added value, as well as the ability to simultaneously characterize the resistome and virulome of the studied pathogen. However, the full potential of pathogen genomics can only be reached through its integration with high-quality contextual data.
Conclusions: For several pathogens, it is time for a shift from proof-of-concept studies to routine use of WGS during outbreak investigations and surveillance activities. However, some implementation challenges from the epidemiologist’s perspective remain, such as data integration, quality of contextual data, sampling strategies, and meaningful interpretations. Interdisciplinary, inter-sectoral, and international collaborations are key for an appropriate genomics-informed surveillance
