George Higoumenakis (1895-1983): Greek dermatologist

This paper describes the Higoumenakis sign, enlargement of the sternal end of the clavicle in patients with late congenital syphilis and the dermatologist after whom it is named. Several professors and doctors from the Medical School of the University of Athens opposed his actions especially at the University in Greece. His persistence led him to productive scientific activity in syphilis, leishmaniasis and psoriasis. fie became a member of the Greek Parliament from 1964 to 1967 and eventually Minister of Hygiene - even though this may have been an imprudent political choice, due to the unstable socio-political status of that period. He died on 27 December 1983 at the age of 88

Korrekturosteotomien post-traumatischer Fehlstellungen

Zusammenfassung: Grundlagen: Zur Behandlung von post-traumatischen Deformitäten stehen neben den definitiven Arthrodeseverfahren verschiedenste Osteotomietechniken zur Verfügung. Sie haben den Vorteil, daß Funktion und Bewegung des Fußes weitgehend erhalten bleiben. Methodik: Die Korrektur posttraumatischer Fehlstellungen bedarf einer eingehenden Untersuchung des betroffenen und gesunden Fußes. Eine wichtige Grundlage bilden die Röntgenaufnahmen im Stehen beider Füße; eine pedographische Analyse kann ebenfalls sehr hilfreich sein, die zugrunde liegende Pathologie besser zu verstehen. Die Korrekturoperation sollte prinzipiell am Ort, wo die Fehlstellung entstanden ist, durchgeführt werden. Wenn nötig, sollte ein Bewegungsverlust in Kauf genommen werden, wenn damit ein regelrechte Stellung und Belastbarkeit des Fußes erreicht werden kann. Ergebnisse: Korrekturosteotomien von posttraumatischen Fehlstellungen haben sich in der Klinik als besonders erfolgreiche Verfahren zur Wiederherstellung des Alignements, Statik und Belastung des Fußes erwiesen, sofern sich noch keine Arthrosen etabliert haben. Schlußfolgerungen: Korrekturosteotomien sind, wenn am richtigen Ort und technisch einwandfrei durchgeführt, besonders erfolgreiche Behandlungsoptionen bei posttraumatischen Fehlstellungen. Bedingungen sind aber intakte Gelenkverhältnisse und normale Muskelfunktione

Osteoma of external acoustic meato: report of nine cases and literature review

The osteoma of external acoustic meatus corresponds to an abnormal and benign bony growth, that is developed under the skin of the auditory canal and that can cause total or partial obstruction of the external acoustic meatus. STUDY DESIGN: Serie study. MATERIAL AND METHOD: We reported in this study nine cases of osteoma of external acoustic meatus. The patients' ages varied between 23 and 70 years old, being the average age 40,6 years. These patients were 5 of the feminine sex and 4 were of the masculine sex. RESULTS: The more common symptoms were hipoacusia, caused by the partial or total obstruction of the canal, appealing external otitis and also the accumulation of cerumen deep in the external auditory canal with difficult removal. CONCLUSION: We recommended in the presented cases surgical treatment, which had good evolution, with no recidivism.Osteoma de meato acústico externo corresponde a um crescimento ósseo anormal e benigno, que se desenvolve abaixo da pele do conduto e pode causar obstrução total ou parcial do meato acústico externo. FORMA DE ESTUDO: Estudo de série. MATERIAL E MÉTODO: Apresentamos neste estudo nove casos de osteoma de meato acústico externo. A idade dos pacientes variou de 23 anos (idade mínima) e 70 anos (idade máxima), sendo a média 40,6 anos. Desses nove pacientes, 5 eram do sexo feminino e 4 eram do sexo masculino. RESULTADO: Os sintomas mais comuns foram: hipoacusia, causada pela obstrução parcial ou total do conduto, otites externas recorrentes e também impactação de cerume de difícil remoção. Wertigkeit der Kernspintomographie bei kindlichen Knieverletzungen

Einleitung: Die Diagnostik kindlicher Knieverletzungen wird heutzutage häufig durch die Kernspintomographie ergänzt, um relevante Kniebinnenschäden zu erkennen. Der klinische Nutzen dieser Zusatzuntersuchung wird in Abhängigkeit vom Beschwerdebild des Patienten und in Relation zum apparativen und wirtschaftlichen Aufwand unterschiedlich bewertet. Ziel dieser Studie war es, die Bedeutung der Kernspintomographie in Abhängigkeit von der klinischen Verdachtsdiagnose und dem arthroskopischen Befund zu bewerten. Material und Methoden: Es wurden insgesamt 195 Patienten im Alter von 3 bis 17 Jahren in diese Studie aufgenommen. 83 dieser Patienten wurden aufgrund der klinischen Diagnose unmittelbar der Kernspintomographie zugeführt. 165 der Patienten wurden, entweder aufgrund der klinischen Untersuchung, oder aufgrund des Befunds der Kernspintomographie, arthroskopiert. Ergebnisse: Die klinische Verdachtsdiagnose wurde bei 117 der 165 Patienten (71%), bei denen eine Arthroskopie durchgeführt wurde, bestätigt. Die größten Übereinstimmungen der Diagnosen in den unterschiedlichen Verfahren gab es dabei bei der Verdachtsdiagnose Patellaluxation (87%) und bei der Verdachtsdiagnose ligamentäre Verletzung (74%). Die Verdachtsdiagnose einer Meniskusläsion brachte eine Übereinstimmung von 48%.Von den 83 Patienten, bei denen eine kernspintomographische Untersuchung durchgeführt wurde, wurden daraufhin 53 Patienten arthroskopiert und 30 Patienten konservativ behandelt. Der MRT Befund wurde in der durchgeführten Arthroskopie bei 42 Patienten (81%) bestätigt. Die MRT Diagnose Patellaluxation wurde hierbei in allen Fällen (100%) in der Arthroskopie bestätigt. Die Übereinstimmungen der Kernspindiagnose einer ligamentären Läsion lag bei 83% und die Übereinstimmung bei der Meniskusverletzung bei 56%.Bei den 30 Patienten, die ausschließlich die Kernspintomographie erhielten und daraufhin konservativ behandelt wurden, wurden hauptsächlich Band- und Meniskuseinblutungen (37%) und extraartikuläre Veränderungen wie Bone Bruise, Fibrom oder Exostose (13%) im MRT diagnostiziert. Letztlich konnte durch den Einsatz der Kernspintomographie der Anteil der rein diagnostischen Arthroskopien von 22% auf 13% reduziert werden. Schlussfolgerung: Bei klinischem Verdacht auf eine Meniskus- oder Bandverletzung ist die Kernspintomographie ein wertvolles Hilfsmittel zur Überprüfung der Diagnose, da häufig klinisch keine sichere Beurteilung möglich ist. Bei der klinischen Verdachtsdiagnose einer Patellaluxation ist die Kernspintomographie zur Diagnosesicherung selten erforderlich, kann jedoch zur Beurteilung von Begleitverletzungen (z.B. Flake Frakturen) wertvolle Zusatzinformationen liefern. Bei klinisch unklarem Befund kann die Anzahl rein diagnostischer Arthroskopien deutlich gesenkt werden

Aluminum and bone: Review of new clinical circumstances associated with Al(3+) deposition in the calcified matrix of bone

Several decades ago, aluminum encephalopathy associated with osteomalacia has been recognized as the major complication of chronic renal failure in dialyzed patients. Removal of aluminum from the dialysate has led to a disappearance of the disease. However, aluminum deposit occurs in the hydroxyapatite of the bone matrix in some clinical circumstances that are presented in this review. We have encountered aluminum in bone in patients with an increased intestinal permeability (coeliac disease), or in the case of prolonged administration of aluminum anti-acid drugs. A colocalisation of aluminum with iron was also noted in cases of hemochromatosis and sickle cell anemia. Aluminium was also identified in a series of patients with exostosis, a frequent benign bone tumor. Corrosion of prosthetic implants composed of grade V titanium (TA6V is an alloy containing 6% aluminum and 4% vanadium) was also observed in a series of hip or knee revisions. Aluminum can be identified in undecalcified bone matrix stained by solochrome azurine, a highly specific stain allowing the detection of 0.03 atomic %. Colocalization of aluminum and iron does not seem to be the fruit of chance but the cellular and molecular mechanisms are still poorly understood. Histochemistry is superior to spectroscopic analyses (EDS and WDS in scanning electron microscopy)

Hearing evaluation of Portuguese school communities: School-based Audiology versus Hearing Screening

Objectivo: Estudo da incidência da perda auditiva e de problemas otológicos em comunidades escolares do Norte do país de um total de 2550 participantes, entre os 3 e os 17 anos de idade. Desenho do Estudo: Levantamento estatístico nas próprias instituições de ensino sendo realizado um protocolo de avaliação auditiva de rastreio. Material e Métodos: A todos os participantes foi realizado o mesmo protocolo de avaliação que consistiu numa anamnese audiológica, otoscopia e exame audiométrico de rastreio, sendo considerado como critério de inclusão a autorização prévia por parte do encarregado de educação. Resultados: Foram identificados diversos problemas otológicos e a audiometria tonal de rastreio contabilizou limiares auditivos indicativos de hipoacusia, uni e bilateralmente, em cerca de 5,7% dos casos. Conclusões: O rastreio auditivo deve ser realizado o mais precocemente possível e fazer parte integral dos cuidados de saúde primários, de modo a orientar a criança para uma educação e acompanhamento apropriados.info:eu-repo/semantics/publishedVersio
