    Flexible intramedullary nailing in the treatment of diaphyseal fractures of the femur in preschool children

    Femur fractures in preschool children are mostly treated in a conservative way, by means of spica cast immobilisation or skin traction. In school age children the use of flexible intramedullary nails (FIN) is widely used and promoted. We performed intramedullary nailing in 9 preschool children aged 1.5-6 years. The mean length of postoperative hospital stay was 4 days (range : 3 to 6). The mean time to solid callus formation was 2.5 months (range : 2 to 3). Follow-up was available in all 9 children for a mean period of 18.9 months (range : 3 to 38). No complications were noted. Flexible intramedullary nailing of femur fractures is a valuable technique in this particular age group. However, further study and long-term follow-up are needed

    Intramedullary Nailing of Periarticular Fractures

    Plate fixation has historically been the preferred surgical treatment method for periarticular fractures of the lower extremity. This trend has stemmed from difficulties with fracture reduction and concerns of inadequate fixation with intramedullary implants. However, the body of literature on management of periarticular fractures of the lower extremities has expanded in recent years, indicating that intramedullary nailing of distal femur, proximal tibia, and distal tibia fractures may be the preferred method of treatment in some cases. Intramedullary nailing reliably leads to excellent outcomes when performed for appropriate indications and when potential difficulties are recognized and addressed

    Fracture healing following high energy tibial trauma: Ilizarov versus Taylor Spatial Frame

    Introduction: The optimal treatment of high energy tibial fractures remains controversial and a challenging orthopaedic problem. The role of external fi xators for all these tibial fractures has been shown to be crucial. Methods: A fi ve-year consecutive series was reviewed retrospectively, identifying two treatment groups: Ilizarov and Taylor Spatial Frame (TSF; Smith & Nephew, Memphis, TN, US). Fracture healing time was the primary outcome measure. Results: A total of 112 patients (85 Ilizarov, 37 TSF) were identifi ed for the review with a mean age of 45 years. This was higher in women (57 years) than in men (41 years). There was no signifi cant difference between frame types (p=0.83). The median healing time was 163 days in both groups. There was no signifi cant difference in healing time between smokers and non-smokers (180 vs 165 days respectively, p=0.07), open or closed fractures (p=0.13) or age and healing time (Spearman's r=0.12, p=0.18). There was no incidence of non-union or re-fracture following frame removal in either group. Conclusions: Despite the assumption of the rigid construct of the TSF, the median time to union was similar to that of the Ilizarov frame and the TSF therefore can play a signifi cant role in complex tibial fractures

    Analysis of the 19th Century Historic Archaeological Material Culture Remains from the Browning Site in Smith County, Texas

    The Browning site (41SM195A) is located on a 3800 m2 alluvial terrace that overlooks the Auburn Creek floodplain in eastern Smith County, Texas. This setting is near the headwaters of a stream system in the Harris Creek drainage; Harris Creek meets the Sabine River ca. 34 km to the north. In the vicinity of the Browning site, the valley, being narrow with steep valley walls, offers few locations suitable for either prehistoric or historic occupations. Soils here arc Entisols; they vary in depth from 30-70 em across the landform, terminating at a sandstone C-horizon. These arc soils that formed mostly under forest vegetation and are dominantly sandy or loamy. The Browning site falls within the Pineywoods vegetation area and represents the western extent of the pine and deciduous forests of the Southeastern U.S. coastal plain. Archaeological investigations at the Browning site have been carried out intermittingly for several years by the junior author. That work has led to the recognition that it is a stratified site with two very distinct occupations, an early to mid-19th century assemblage of historic artifacts primarily in an upper zone (0-20 em bs) overlying a buried (20-50 em bs) Late Woodland period occupation. The historic occupation is in the center of the terrace, covering approximately 500m2• The historic artifacts arc found primarily in the upper sediments, but due probably to pedoturbations, they have been found as deep as 50 em in the underlying prehistoric archaeological deposits. Excavations at the Browning site have consisted of 41 1 x 1 m units (with a total excavated volume of 20.4 m3) and 22 shovel tests. Surface collections were obtained from the site in 1996 and 2002. The 22 shovel tests excavated at the site were conducted first to better ascertain the limits of the site and identify areas of concentrated cultural activity; ST 4, 8-9, 12, and 19 contained 19th century historic artifacts. Once a buried prehistoric occupation zone was identified, units were placed primarily in cardinal directions to better define the occupation zone\u27s boundaries and levels of occupational intensity, and also sample the overlying 19th century component. The I x 1 m units were excavated in arbitrary l 0 em levels and the soil was dry-screened for artifacts through I /4-inch hardware cloth except for a fine screen sample from Unit I that was water-screened through I /32-inch mesh. A level sheet was completed at the end of each level. Profiles were drawn of one wall of each unit or a common wall when several units were joined. The shovel tests followed the same procedures, except they were excavated in arbitrary 20 em levels. One feature had four refined earthenware sherds (as well as two prehistoric artifacts) and 15 small pieces of animal bone. This was a shallow pit with a very dark grayish-brown sandy loam fill (with charcoal flecks) and a rounded bottom that was 63 em in diameter and extended from 22-29 em bs

    This is the third in a series of publications that concern the documentation of prehistoric artifact-collections from sites found along the shoreline of Lake Bob Sandlin in the Big Cypress Creek basin of East Texas. These documentation efforts have demonstrated that sites at the lake have diverse temporal and spatial patterns, with an intensive Caddo occupation from the Middle (ca. A.D. 1200-1425) to Late Caddo (ca. A.D. 1430-1680) periods

    The systematic excavations that began at Golemo Gradište in 2000 were the first major, legal investigations on the site itself. Through survey of the site, researchers had reached a number of conclusions and hypotheses about lines of fortification walls, location of gates, and roads associated with the site. But almost nothing was known about the buildings or the internal arrangement of the site, and there were questions about dating. Therefore, both on the acropolis (2000-2004) and on the northern terrace (2005-present), the first step was to set trenches in several places, to investigate the architecture and the urban plan and to establish a chronology of the settlement. Given the discoveries in those initial trenches, excavation was later focused on a few areas. One consequence of this approach was limited excavation in several areas that were probably residential (or a combination of residential with industrial or commercial), but only a few examples of extensively investigated residential complexes.
