    The role of alerting in the attentional boost effect

    Stimuli presented simultaneously with behaviorally relevant events (e.g., targets) are better memorized, an unusual effect defined as the attentional boost effect (ABE). We hypothesized that all types of behaviorally relevant events, including attentional cues, can promote the encoding process for the stimuli paired with them, and the attentional alerting network can amplify the ABE. The two experiments we conducted demonstrated that not all behaviorally relevant events, including alerting cues, benefit the processing of concurrently paired stimuli. We also found that the presence of a cue prior to a target can extend the memory advantage produced by target detection, but this advantage can only be observed within a limited range of time. Overall, our study provides the first evidence that the alerting network plays an important role in the ABE

    Investigating the impact of visual perspective in a motor imagery-based brain-robot interaction: A pilot study with healthy participants

    IntroductionMotor Imagery (MI)-based Brain Computer Interfaces (BCI) have raised gained attention for their use in rehabilitation therapies since they allow controlling an external device by using brain activity, in this way promoting brain plasticity mechanisms that could lead to motor recovery. Specifically, rehabilitation robotics can provide precision and consistency for movement exercises, while embodied robotics could provide sensory feedback that can help patients improve their motor skills and coordination. However, it is still not clear whether different types of visual feedback may affect the elicited brain response and hence the effectiveness of MI-BCI for rehabilitation.MethodsIn this paper, we compare two visual feedback strategies based on controlling the movement of robotic arms through a MI-BCI system: 1) first-person perspective, with visual information that the user receives when they view the robot arms from their own perspective; and 2) third-person perspective, whereby the subjects observe the robot from an external perspective. We studied 10 healthy subjects over three consecutive sessions. The electroencephalographic (EEG) signals were recorded and evaluated in terms of the power of the sensorimotor rhythms, as well as their lateralization, and spatial distribution.ResultsOur results show that both feedback perspectives can elicit motor-related brain responses, but without any significant differences between them. Moreover, the evoked responses remained consistent across all sessions, showing no significant differences between the first and the last session.DiscussionOverall, these results suggest that the type of perspective may not influence the brain responses during a MI- BCI task based on a robotic feedback, although, due to the limited sample size, more evidence is required. Finally, this study resulted into the production of 180 labeled MI EEG datasets, publicly available for research purposes

    Deciphering multiple sclerosis disability with deep learning attention maps on clinical MRI

    Deep learning; Disability; Structural MRIAprendizaje profundo; Discapacidad; Resonancia magnética estructuralAprenentatge profund; Discapacitat; Ressonància magnètica estructuralThe application of convolutional neural networks (CNNs) to MRI data has emerged as a promising approach to achieving unprecedented levels of accuracy when predicting the course of neurological conditions, including multiple sclerosis, by means of extracting image features not detectable through conventional methods. Additionally, the study of CNN-derived attention maps, which indicate the most relevant anatomical features for CNN-based decisions, has the potential to uncover key disease mechanisms leading to disability accumulation. From a cohort of patients prospectively followed up after a first demyelinating attack, we selected those with T1-weighted and T2-FLAIR brain MRI sequences available for image analysis and a clinical assessment performed within the following six months (N = 319). Patients were divided into two groups according to expanded disability status scale (EDSS) score: ≥3.0 and < 3.0. A 3D-CNN model predicted the class using whole-brain MRI scans as input. A comparison with a logistic regression (LR) model using volumetric measurements as explanatory variables and a validation of the CNN model on an independent dataset with similar characteristics (N = 440) were also performed. The layer-wise relevance propagation method was used to obtain individual attention maps. The CNN model achieved a mean accuracy of 79% and proved to be superior to the equivalent LR-model (77%). Additionally, the model was successfully validated in the independent external cohort without any re-training (accuracy = 71%). Attention-map analyses revealed the predominant role of frontotemporal cortex and cerebellum for CNN decisions, suggesting that the mechanisms leading to disability accrual exceed the mere presence of brain lesions or atrophy and probably involve how damage is distributed in the central nervous system.MS PATHS is funded by Biogen.     Decoding spatial location of attended audio-visual stimulus with EEG and fNIRS

    When analyzing complex scenes, humans often focus their attention on an object at a particular spatial location in the presence of background noises and irrelevant visual objects. The ability to decode the attended spatial location would facilitate brain computer interfaces (BCI) for complex scene analysis. Here, we tested two different neuroimaging technologies and investigated their capability to decode audio-visual spatial attention in the presence of competing stimuli from multiple locations. For functional near-infrared spectroscopy (fNIRS), we targeted dorsal frontoparietal network including frontal eye field (FEF) and intra-parietal sulcus (IPS) as well as superior temporal gyrus/planum temporal (STG/PT). They all were shown in previous functional magnetic resonance imaging (fMRI) studies to be activated by auditory, visual, or audio-visual spatial tasks. We found that fNIRS provides robust decoding of attended spatial locations for most participants and correlates with behavioral performance. Moreover, we found that FEF makes a large contribution to decoding performance. Surprisingly, the performance was significantly above chance level 1s after cue onset, which is well before the peak of the fNIRS response. For electroencephalography (EEG), while there are several successful EEG-based algorithms, to date, all of them focused exclusively on auditory modality where eye-related artifacts are minimized or controlled. Successful integration into a more ecological typical usage requires careful consideration for eye-related artifacts which are inevitable. We showed that fast and reliable decoding can be done with or without ocular-removal algorithm. Our results show that EEG and fNIRS are promising platforms for compact, wearable technologies that could be applied to decode attended spatial location and reveal contributions of specific brain regions during complex scene analysis

    Evaluation of image quality and reconstruction parameters in recent PET-CT and PET-MR systems

    In this PhD dissertation, we propose to evaluate the impact of using different PET isotopes for the National Electrical Manufacturers Association (NEMA) tests performance evaluation of the GE Signa integrated PET/MR. The methods were divided into three closely related categories: NEMA performance measurements, system modelling and evaluation of the image quality of the state-of-the-art of clinical PET scanners. NEMA performance measurements for characterizing spatial resolution, sensitivity, image quality, the accuracy of attenuation and scatter corrections, and noise equivalent count rate (NECR) were performed using clinically relevant and commercially available radioisotopes. Then we modelled the GE Signa integrated PET/MR system using a realistic GATE Monte Carlo simulation and validated it with the result of the NEMA measurements (sensitivity and NECR). Next, the effect of the 3T MR field on the positron range was evaluated for F-18, C-11, O-15, N-13, Ga-68 and Rb-82. Finally, to evaluate the image quality of the state-of-the-art clinical PET scanners, a noise reduction study was performed using a Bayesian Penalized-Likelihood reconstruction algorithm on a time-of-flight PET/CT scanner to investigate whether and to what extent noise can be reduced. The outcome of this thesis will allow clinicians to reduce the PET dose which is especially relevant for young patients. Besides, the Monte Carlo simulation platform for PET/MR developed for this thesis will allow physicists and engineers to better understand and design integrated PET/MR systems

    Desarrollo de una batería de memoria semántica para pacientes con epilepsia del lóbulo temporal

    La epilepsia focal más frecuente es aquella epilepsia cuyo foco epileptógeno está localizado en el lóbulo temporal medial y es secundaria a una esclerosis con atrofia de la región amígdalo-hipocámpica, con una red epileptógena que abarca la porción anterior del lóbulo temporal. En ocasiones los pacientes requieren de un tratamiento quirúrgico que incluye la resección unilateral de ambas regiones, tanto del polo anterior, como del complejo amígdala-hipocampo. Estas estructuras han demostrado tener gran importancia para el procesamiento de la memoria semántica (región anterotemporal) y episódica (región amígdalo-hipocámpica), por lo que los pacientes que son sometidos a esta intervención suelen presentar quejas cognitivas relacionadas con ambos tipos de memoria. Sin embargo, parece que las evaluaciones neuropsicológicas que realizamos de forma rutinaria en las diferentes Unidades de Epilepsia no son capaces de detectar todos los problemas cognitivos que ocurren en estos pacientes ya que, a pesar de las dificultades expresadas por estos, las evaluaciones no muestran alteraciones. La hipótesis principal del presente trabajo es que estas quejas se deben a tipos de memoria que no están incluidos en las pruebas neuropsicológicas actuales y, por tanto, no somos capaces de identificar bien sus problemas. En primer lugar, se propone que la memoria semántica está afectada, pero solamente para palabras de baja frecuencia de uso en la vida diaria, no analizadas en las evaluaciones convencionales actuales. En segundo lugar, otros problemas no objetivados se deben a un problema de la memoria de consolidación, medida como olvido a largo plazo acelerado que se detecta cuando se amplia el periodo de evaluación del recuerdo. Además, estas alteraciones van a manifestarse con mayor intensidad en pacientes cuyo foco epileptógeno está localizado en el lóbulo temporal izquierdo. Los objetivos fundamentales de este trabajo son evaluar en pacientes con epilepsia del lóbulo temporal medial intervenidos quirúrgicamente mediante lobectomía temporal anterior con amigdalohipocampectomía la presencia de alteraciones de la memoria verbal tanto semántica como episódica, así como conocer su valor lateralizador según el hemisferio afectado. El estudio se basó en la comparación de pacientes con epilepsia del lóbulo temporal (ELT) tratados con lobectomía temporal anterior con amigdalohipocampectomía con un grupo control de personas sanas, comparables respecto a edad, nivel educativo y coeficiente intelectual (CI). Las pruebas de memoria semántica mostraron que únicamente los pacientes con ELT izquierda tenían alteraciones, especialmente para ítems de baja frecuencia y tanto en tares de expresión como de comprensión verbal. Asimismo, el tiempo de reacción fue mayor en el grupo de pacientes con ELT izquierda para todos los ítems y únicamente para las palabras o conceptos de baja frecuencia en aquellos con ELT derecha. Además, se incluyó una prueba de memoria episódica estándar (RAVLT) que en lugar de restringir la evaluación a 30 minutos, se evaluó a 7 días para medir el olvido a largo plazo. Los resultados mostraron que los dos grupos de pacientes, tanto los de ELT izquierda como aquellos con ELT derecha, desarrollaron olvido a largo plazo. Por último los resultados mostraron que la presencia de crisis epilépticas no afectó a la presencia de olvido a largo plazo acelerado

    EVALUACIÓN ANALGÉSICA PERIOPERATORIA DEL ACETAMINOFÉN EN PERRAS SOMETIDAS A OVARIOHISTERECTOMÍA ELECTIVA

    Tesis de doctorado que evalúa el efecto analgésico del acetaminofén en perras ovarihisterectomizadas.La administración de analgésicos antiinflamatorios no esteroidales (AINES) para el control del dolor post-quirúrgico en perros es una práctica común, debido a sus efectos analgésicos, antiinflamatorios y antipiréticos. En el presente trabajo se realizaron dos estudios. En el experimento 1, el objetivo fue evaluar la analgesia post-operatoria del acetaminofén (paracetamol) a través de la utilización de las escalas de reconocimiento clínico del dolor DIVAS (Escala Dinámica e Interactiva Analógica Visual) y UMPS (Escala de la Universidad de Melbourne), en perras sometidas a ovariohisterectomía electiva. Además de valorar la seguridad y eficacia clínica del uso del acetaminofén en perros mediante pruebas de funcionamiento hepático y renal en el post-operatorio inmediato. Para ello, se utilizaron 30 perras de diferentes razas que fueron asignadas aleatoriamente a uno de los tres grupos de tratamiento: acetaminofén [GACET; n=10, 15 mg kg-1 intravenoso (IV)], carprofeno (GCARP; n=10, 4 mg kg-1 IV) y meloxicam (GMELOX; n=10, 0.2 mg kg-1 IV). Todos los tratamientos se administraron 30 minutos antes de la cirugía y posterior a esta durante 48 horas. En este período el acetaminofén se administró por vía oral cada 8 horas (15 mg kg-1); el carprofeno (4 mg kg-1) y el meloxicam (0.1 mg kg-1) se administraron por vía IV cada 24 horas. Durante el postoperatorio, los sistemas de puntuación del dolor DIVAS y UMPS fueron medidos a las 1, 2, 4, 6, 8, 12, 16, 20, 24, 36 y 48 horas post-cirugía. Para evaluar la seguridad clínica de los tratamientos, se recolectaron muestras de sangre de la vena yugular para realizar la medición de enzimas ALT, AST, ALP, y los metabolitos bilirrubina directa, bilirrubina indirecta, bilirrubina total, creatinina, urea, albúmina y glucosa. Esto fue realizado en T0 (pre-anestesia; TBASAL), 48 y 96 horas después de la cirugía (T48, T96). Los resultados indican que en la evaluación clínica del dolor de todos los grupos de estudio, hubo una reducción gradual en la percepción del mismo durante el postoperatorio en ambos sistemas de puntuación; no obstante, también fue observado que ninguna escala difirió significativamente entre los tres grupos de tratamiento (P>0.05) en cada momento de evaluación durante las 48 horas post-cirugía. En cuanto a los parámetros bioquímico séricos, sólo la ALT aumentó significativamente en T96 en el GACET y GCARP con respecto a los valores basales (P<0.01). El resto de los analitos séricos evaluados se mantuvo en rangos normales. En el experimento 2 bajo el mismo diseño experimental de tratamientos administrados, el objetivo fue evaluar el efecto analgésico perioperatorio del acetaminofén 2 administrado pre y post-quirúrgicamente en perras sometidas a ovariohisterectomía electiva a través de la medición del índice de la actividad del tono parasimpático (PTA). Este parámetro hemodinámico fue medido 60 minutos antes de la cirugía (TBASAL) y durante el transquirúrgico en la aplicación de estímulos nociceptivos: colocación de las pinzas de campo backhouse (TPINZ), incisión de piel y abordaje quirúrgico primario (TINC), ligadura y extracción de pedículo ovárico izquierdo (TOVI) y derecho (TOVD), ligadura y transfixión del cuello uterino (TLIGUT), sección quirúrgica del cuello uterino (TCUT), reconstrucción de peritoneo y planos anatómicos musculares (TMUSC) y sutura de piel (TSUT). Durante el postoperatorio, el índice PTA fue valorado a las 1, 2, 4, 6, 8, 12, 16, 20, 24, 36 y 48 horas, en los mismos tiempos en que fueron evaluadas las escalas de reconocimiento de dolor DIVAS y UMPS. Los resultados obtenidos en la medición del índice PTA basal para GACET fue 65 ± 8, para GCARP 65 ± 7 y para GMELOX 62 ± 5. Durante los diferentes tiempos transquirúrgicos, los valores promedio de índice PTA indican que GACET (76 ± 14) y GMELOX (72 ± 18) muestran tendencia a manifestar mayores niveles en comparación con GCARP (62 ± 13) desde el inicio del procedimiento quirúrgico sin que esto pudiera comprobarse estadísticamente, ya que no hubo diferencias significativas entre grupos de tratamiento ni entre los tiempos quirúrgicos evaluados (P>0.05). En el postoperatorio, el índice PTA fue de 65 ± 9 en el GACET, 63 ± 8 en el GCARP y 65 ± 8 en el GMELOX. Los resultados tampoco mostraron diferencias estadísticamente significativas con los valores basales o entre los tratamientos (P>0.05). El índice PTA postoperatorio mostró una sensibilidad del 40%, especificidad del 98.46% y valor predictivo negativo del 99.07% con respecto a la escala validada de UMPS. En conclusión, el acetaminofén puede considerarse una herramienta para el tratamiento efectivo del dolor perioperatorio agudo en perros, ya que mostró la misma eficacia clínica que el meloxicam y el carprofeno para la analgesia postquirúrgica en perras sometidas a ovariohisterectomía electiva. Además, la evidencia del uso de este medicamento no condujo a reacciones adversas o cambios en los parámetros evaluados, lo que indica su seguridad clínica. Finalmente, destacar que el índice PTA representa una medición objetiva del comfort y analgesia postoperatoria, por lo que es una herramienta que podría ayudar a predecir las respuestas hemodinámicas asociadas con el dolor

    The Neuroscience of Moral Judgment: Empirical and Philosophical Developments

    We chart how neuroscience and philosophy have together advanced our understanding of moral judgment with implications for when it goes well or poorly. The field initially focused on brain areas associated with reason versus emotion in the moral evaluations of sacrificial dilemmas. But new threads of research have studied a wider range of moral evaluations and how they relate to models of brain development and learning. By weaving these threads together, we are developing a better understanding of the neurobiology of moral judgment in adulthood and to some extent in childhood and adolescence. Combined with rigorous evidence from psychology and careful philosophical analysis, neuroscientific evidence can even help shed light on the extent of moral knowledge and on ways to promote healthy moral development

    Der Einfluss der nichtinvasiven Vagusnervstimulation auf die absteigende Schmerzhemmung

    Hintergrund: Klinische Studien zur Therapie chronischer Kopfschmerz- und Schmerzerkrankungen zeigen vielversprechende analgetische Effekte der nichtinvasiven Vagusnervstimulation (nVNS). Die zugrunde liegenden Mechanismen sind trotz intensiver Forschung nur teilweise verstanden. Tierexperimentelle Studien geben Hinweise auf eine nVNS-vermittelte Aktivierung des Nucleus tractus solitarii (NTS) und dessen Projektionen zum Locus coeruleus und den Raphekernen. Von diesen Hirnstammregionen reichen absteigende serotonerge und noradrenerge Bahnen („absteigende Schmerzhemmung“) in das Rückenmark und bewirken dort eine Reduktion der nozizeptiven Übertragung. Eine Modulation affektiver Schmerzkomponenten durch nVNS könnte ebenfalls zur schmerzhemmenden Wirkung der nVNS beitragen. Ziel der hier dargestellten Arbeit war die Untersuchung eines möglichen Effekts der zervikalen nVNS auf die absteigende Schmerzhemmung, quantifiziert mit dem nozizeptiven Flexorreflex (RIII-Reflex) und dem Conditioned Pain Modulation (CPM-) Paradigma. Veränderungen in der supraspinalen Schmerzverarbeitung wurden mittels später somatosensibel evozierter Potentiale (SEPs) und Schmerzintensitäts- und Unangenehmheitsbewertungen erfasst. Methoden: 27 Probanden nahmen an dieser doppelblinden crossover Studie teil. In 2 experimentellen Sitzungen wurde, in randomisierter Reihenfolge, über 4 Minuten entweder nVNS oder eine Sham-Stimulation appliziert. RIII-Reflexe, Schmerz-/Unangenehmheitsbewertungen und SEPs wurden zur Baseline, während, 5, 15, 30 und 60 Minuten nach Intervention (nVNS/Sham-Stimulation) erfasst. Im Anschluss wurde der CPM-Effekt erhoben. 4 Teilnehmer mussten aus methodischen Gründen ausgeschlossen werden, sodass die statistische Analyse 23 Teilnehmer umfasste. Ergebnisse: nVNS zeigte keinen statistisch signifikanten Effekt auf die RIII-Reflexflächen, die RIIIReflex- und Schmerzschwellen, die Schmerzintensitätsbewertungen, die Amplituden der späten SEP-Komponenten und den CPM-Effekt im Vergleich zur Sham-Stimulation. Die Unangenehmheitsbewertungen der Schmerzreize wurden im Vergleich zur Sham-Stimulation während nVNS signifikant von 4,4 ± 2,4 (NRS 0 - 10) auf 4,1 ± 2,5 reduziert (F [1,22] = 6.82, p = .016, η2 p = .237). Es wurden keine länger andauernde Effekte (5 - 60 Minuten nach Intervention) der nVNS gefunden. Zusammenfassung: Die dargestellten Ergebnisse stützen nicht die Hypothese eines akuten Effekts der nVNS auf die absteigende Schmerzhemmung oder auf die supraspinale Nozizeption (Schmerzintensitätsbewertungen, SEPs) bei gesunden Probanden. Es zeigte sich jedoch ein kleiner und kurzfristiger Effekt der nVNS auf die Unangenehmheitsbewertungen von Schmerzen, was für eine nVNS-vermittelte Modulation affektiv-emotionaler statt sensorisch-diskriminativer Schmerzaspekte spricht
