Recurrent pneumonia uncovering pulmonary sequestration in adult with HIV infection

Abstract

Pulmonary sequestration (PS) is a rare congenital lung anomaly characterized by nonfunctional pulmonary tissue, aberrant systemic arterial supply, and absence of communication with the tracheobronchial tree. Although it is usually diagnosed in childhood, it may present in adulthood as recurrent respiratory infections. We report the case of a 35-year-old man with HIV infection and an undetectable viral load, with a history of recurrent pneumonia, who presented with cough, fever, and dyspnea. Contrast-enhanced chest computed tomography revealed a heterogeneous lesion in the right lower lobe with systemic arterial supply arising from the descending aorta, consistent with intralobar pulmonary sequestration. The patient underwent lobectomy, and histopathological examination confirmed the diagnosis. Clinical outcome was favorable. Pulmonary sequestration in immunocompromised adults is uncommon and may lead to delayed diagnosis.El secuestro pulmonar (SP) es una anomalía congénita rara caracterizada por tejido pulmonar no funcional, irrigación arterial sistémica anómala y ausencia de conexión con el árbol bronquial. Aunque suele diagnosticarse en la infancia, puede manifestarse en la adultez como infecciones respiratorias recurrentes. Presentamos el caso de un varón de 35 años con infección por VIH y carga viral indetectable, con antecedentes de neumonías repetidas, que consultó por tos, fiebre y disnea. La tomografía de tórax con contraste evidenció una lesión heterogénea en el lóbulo inferior derecho con irrigación arterial proveniente de la aorta descendente, compatible con secuestro pulmonar intralobar. Se realizó lobectomía, y el estudio histopatológico confirmó el diagnóstico. La evolución fue favorable. El SP en adultos inmunocomprometidos es infrecuente y puede retrasar el diagnóstico
