Antisynthetase syndrome in a male resident of a high-altitude city

Abstract

Antisynthetase syndrome is a rare autoimmune disease which includes polyarthritis, myositis, Raynaud\u27s phenomenon, mechanic\u27s hands, interstitial lung disease and positive anti JO-1 antibody. We report a 44-yearold male patient from Puno, a high-altitude city, who presented with polyarthritis, dyspnea, muscle weakness, edema, and pain in the left lower limb. The patient had positive Anti JO-1, anti SS-A and anti B2 glycoprotein-1 IgG antibodies. A muscle biopsy reported myositis and he was treated with methylprednisolone and cyclophosphamide, with favorable results. Antisynthetase syndrome is a rare disease that slightly predominates in women. The possible role of hypobaric hypoxia in the development of this autoimmune disease is highlighted.El síndrome antisintetasa es una patología autoinmune rara, que incluye poliartritis, miositis, fenómeno de Raynaud, manos de mecánico, enfermedad pulmonar intersticial y anticuerpo anti JO-1 positivo. Reportamos un paciente varón de 44 años, natural de Puno, una ciudad de gran altitud, quien presentó poliartritis, disnea, debilidad muscular, edema y dolor en miembro inferior izquierdo. El paciente presentó anti JO-1 y anti SS-A positivos. La biopsia muscular informó miositis y recibió tratamiento con metilprednisolona y ciclofosfamida, con evolución favorable. El síndrome antisintetasa es una patología infrecuente y con ligero predominio en mujeres. Resaltamos el posible papel de la hipoxia hipobárica en el desarrollo de esta enfermedad autoinmune
