ASUHAN KEPERAWATAN MATERNITAS PADA PASIEN POSTPARTUM SPONTAN TRAUMA PERINEUM DENGAN MASALAH KEPERAWATAN KETIDAKNYAMANAN PASCA PARTUM

ABSTRACT MATERNITY NURSING CARE FOR POSTPARTUM PATIENTS WITH PERINEAL TRAUMA AND POSTPARTUM DISCOMFORT PROBLEM (Case Study at Melati Ward RSU Darmayu Ponorogo) By: Rania Nurhayati NIM 22613563 Postpartum discomfort is one of the common problems experienced by mothers who give birth spontaneously, especially those with perineal trauma. Perineal trauma can cause discomfort that hinders daily activities and disrupts the recovery process during the postpartum period. Appropriate management is required to support recovery. This case study aims to provide maternal nursing care for postpartum patients with perineal trauma who experience the nursing problem of postpartum discomfort. The method used is a nursing process approach, including assessment, nursing diagnosis, intervention, implementation, and evaluation. Nursing care was provided at RSU Darmayu Ponorogo with the subject of the study being Mrs. W, a postpartum patient with perineal trauma, who was treated in the Melati Ward from June 1 to June 3, 2025. Assessment results showed that the patient complained of discomfort in the perineal area due to an episiotomy, grimaced when moving, had a grade II episiotomy wound, and experienced uterine contractions. The primary nursing diagnosis was postpartum discomfort. The interventions provided were based on the Indonesian Nursing Intervention Standards (SIKI), specifically pain management (1.08238) and postpartum care (1.07225). Evaluation results showed reduced discomfort, improved episiotomy wound healing with no signs of infection, and good uterine contraction. This nursing care is expected to serve as a reference for providing postpartum care, helping to accelerate recovery and improve maternal comfort.
