Expulsión y sublimación; manejo de los objetos internalizados : (análisis de un sueño)

Abstract

This work shows, through the analysis of a dream and the subjects with which it is related, a section of the analytic treatment of a patient. The latter, a woman who suffered from several hypochondric troubles, conversion symptoms, depressions and difficulties in her sublimations and object relations, expresses a critical moment in her analysis and her life in this dream. Many “beasts” and other elements of the manifest content, express the present state of her internalised objects. The persecutory aspect of the latter was formerly to be seen in the diverse “pains” of the patient, and the dream constitutes the patient’s attempt to drive out these objects and to use them in a sublimation, thus modifying the type of control and of handling used up to that moment. The analysis of the affections of the dream leads us to another theme, which is very important for the patient: the experience of the magic and the wondrous. One gets to understand the feeling of the marvellous in the dream as an elaboration of sinister phantasies in connection with the mother identified with death. One tries to understand the nature and genesis of the persecutors, discovering that, in the patient’s infantile experiences, the splitting process of the mother (with dissociation of the good aspects, placed on the father and the latter’s idealisation) seems to be the origin of many of her difficulties.One investigates how, for this patient, the sublimation implies, aside from the outdriving of internalised persecutors, their harmonious integration which enables a control over them and the disappearance of their persecutory qualities. This integration is experienced by the patient, in the transference situation, according to the model of analytical work.Este trabajo presenta, a través del análisis de un sueño y de los temas relacionados con él, un corte del tratamiento analítico de una paciente. Esta, una mujer que padecía de varios trastornos hipocondríacos, de síntomas de conversión, de depresiones y de dificultades en sus sublimaciones y en sus relaciones de objeto, expresa en este sueño un momento crítico de su análisis y de su vida. Muchos "bichos" y otros elementos del contenido manifiesto expresan el estado actual de sus objetos internalizados. El aspecto perseguidor de éstos se manifestaba anteriormente en los múltiples "dolores" de la paciente, y el sueño constituye una tentativa de la paciente de expulsar a estos objetos y de utilizarlos en una sublimación, modificando el tipo de control y de manejo utilizado hasta entonces. Se examina cómo en esta paciente, la sublimación implica, además de la expulsión afuera de los perseguidores internalizados, su integración en una forma armoniosa que permite su control y la desaparición de sus calidades perseguidoras. Esta integración es vivida por la paciente, en la situación transferencial, según el modelo del trabajo analítico. El análisis de los afectos del sueño lleva hacia otro tema muy importante para la paciente: las vivencias de lo mágico y de lo maravilloso. Se llega a entender el sentimiento de lo maravilloso en el sueño como elaboración de fantasías siniestras en relación con la madre identificada con la muerte. Se trata de entender la naturaleza y la génesis de los perseguidores, descubriendo en las vivencias infantiles de la paciente el proceso de fragmentación de la madre (con disociación de los aspectos buenos, ubicados en el padre, y la idealización de éste) como el origen de muchos de sus trastornos
