Influencias de las barreras de accesibilidad en pacientes de un centro odontológico universitario

Abstract

INTRODUCTION: Accessibility to oral health is directly influenced by the social determinants of health present in sociodemographic contexts. Therefore, knowledge about economic, social, demographic and epidemiological factors of the population that most needs dental care becomes relevant in planning policies that contribute to ensuring access to oral health. OBJECTIVES: The study aimed to describe sociodemographic and epidemiological characteristics of patients treated at the University Dental Center (COU) and their association with social determinants of health.MATERIAL AND METHOD: A descriptive cross-sectional observational design was performed in a sample of 59 patients from the Admission Service of the University Dental Center of FASTA University. Age, sex, level of education, type of occupation, reason and date of last consultation were evaluated. A voluntary questionnaire was applied by performing the statistical treatment using IBM SPSS Statistics 22.0, 95% CI, p<0.05, and the Pearson correlation coefficient was evaluated. RESULTS: The average age of the sample was 42.1 years, with a fashion and median 38; 45 (76.3%) were women. Secondary education was the highest level of education attended by 33 patients (55.9%, p<0.05). Formal employment was of equivalent value. Regarding the type of coverage, 36 patients (61.0%) reported that they did not have any. Finally, 24 (40.7%) people had attended a dental consultation during the last year, with only 6 (25.0%) having their last visit for prevention or oral health controlCONCLUSION: From the sample analyzed, it was concluded that the group that did not have private dental coverage or through social works was significant (p<0.05), increasing accessibility barriers and inequalities in oral health. There is a need to reorient health policies towards universal health coverageINTRODUCCIÓN: La accesibilidad a la salud bucal está directamente influenciada por los determinantes sociales de la salud presentes en los contextos sociodemográficos. Así, el conocimiento sobre factores económicos, sociales, demográficos y epidemiológicos de la población que más requiere atención odontológica adquiere relevancia en la planificación de políticas que contribuyan a garantizar el acceso a la salud bucal. OBJETIVOS: El trabajo se propuso describir características sociodemográficas y epidemiológicas de pacientes atendidos en el Centro Odontológico Universitario (COU) y su asociación con determinantes sociales de la salud. MATERIAL Y MÉTODO: Se realizó un diseño observacional descriptivo, de corte transversal, en una muestra por conveniencia de 59 pacientes del Servicio de Admisión del Centro Odontológico Universitario de la Universidad FASTA. Se evaluó edad, sexo, nivel de instrucción, tipo de ocupación, motivo y fecha de última consulta. Se aplicó un cuestionario voluntario realizando el tratamiento estadístico mediante IBM SPSS Statistics 22.0, IC 95%, p<0.05, y se evaluó el coeficiente de correlación de Pearson. RESULTADOS: De la muestra analizada, la edad promedio fue 42,1 años, moda y mediana 38; siendo el 45 (76,3%) mujeres. El nivel de educación secundaria fue el máximo nivel educativo asistido por 33 pacientes (55,9%, p<0.05). El empleo formal obtuvo un valor equivalente. Respecto al tipo de cobertura, 36 pacientes (61,0%) pacientes manifestaron no poseer ninguna. Finalmente, 24 (40,7%) personas habían asistido a una consulta odontológica durante el último año, siendo solamente 6 (25,0%) que el motivo de su última consulta había sido por prevención o control de salud bucal. CONCLUSIÓN: A partir de la muestra analizada, se concluye que resultó significativo el grupo que no poseía cobertura odontológica privada o a través de las obras sociales (p<0.05), aumentando las barreras de accesibilidad y las inequidades en salud bucal. Se hace necesario reorientar las políticas sanitarias para que tiendan a una cobertura universal de la salud
