AbstractSecondary sclerosing cholangitis (SCC) in critically ill patients is a largely unrecognized but growing complication. Indeed, there is an increasing number of reports of patients with sclerosing cholangitis who were survivors of the life-threatening disease requiring aggressive intensive care treatment. Here the endoscopic features of SCC in a critically ill patient with massive jaundice are reported in detail. Therapeutic options for most forms of SSC are limited, but some suggest that endoscopic removal of intraductal biliary cast sludge might improve the biliary flow and the course of the disease. In the present case, cholangioscopic clearance of biliary casts and debris did result in transient improvement of bile drainage and jaundice, however, this effect was not sustained.
