O micro-organismo mais comum causador da EI aguda e subaguda é o Streptococcus viridans, encontrado comumente na cavidade oral, que pode causar bacteremia durante o tratamento dentário, podendo levar a ter complicações, tais como, insuficiência cardíaca, arritmia, eventos tromboembólicos e evoluindo até a morte. Sua prevenção é dada pela administração adequada de antibióticos antes de um procedimento odontológico, para reduzir a bacteremia transitória e o risco do desenvolvimento da EI em pacientes de risco. O objetivo deste estudo foi levantar presente projeto expõe a proposta de pesquisa de Trabalho de Conclusão de Curso do Curso de Graduação em Odontologia da Universidade Federal de Santa Catarina. No intuito de investigar as controvérsias na Odontologia em torno da doença cardiológica denominada EI e da necessidade do uso da profilaxia antimicrobiana prévia ao tratamento odontológico para pacientes de risco. Foi realizado um levantamento na base de dados do Sistema de Serviços e Prontuários e do Sistema de Apuração de Custos do número de casos de pacientes internados para tratamento da EI no Hospital Professor Doutor Polydoro Ernani de São Thiago – Hospital Universitário da Universidade Federal de Santa Catarina, no período de 2010 a 2014, para verificação dos custos hospitalares, incluindo diárias, exames diagnósticos e medicamentos. Posteriormente foi efetuado um levantamento de custos dos medicamentos recomendados pela American Heart Association e do American College of Cardiology para a profilaxia antimicrobiana prévia ao tratamento odontológico, através do site da ANVISA, atualizada em abril de 2015, onde foi possível determinar a discrepâncias entre os custos para uma profilaxia antimicrobiana, tanto para o paciente individualmente, quanto ao Sistema Único de Saúde do Brasil, com a média dos gastos hospitalares, por indivíduo, para o tratamento da EI. Os resultados deste estudo evidenciaram que o custo de uma profilaxia antimicrobiana corresponde ao equivalente a 0,09% dos valores médios do custo hospitalar por paciente internado para o tratamento da EI, os quais atingiram R5.564,20,naInstituic\ca~odeestudo.DessaformaesseestudorecomendaqueaProfilaxiaAntimicrobianadeveserutilizadaparapacientesderisco,previamenteaotratamentoodontoloˊgico,conformerecomendac\co~esdaAmericanHeartAssociation,minimizandoamorbimortalidadeemortalidadedosindivıˊduosderisco,bemcomo,osgastospuˊblicosparaotratamentodessapatologia.TheInfectiveEndocarditis(IE)isaseriousdiseaserelatedtosignificantmorbidityandmortality.ThemostcommonmicroorganismrelatedforinducingtheacuteandsubacuteIEistheStreptocccusviridians,commonlyfoundintheoralcavity,whichcancausebacteremiaduringdentaltreatmentandleadtocomplicationssuchasheartfailure,arrhythmia,thromboembolicevents,evolvingtodeath.Thepreventionisachievedbythecorrectadministrationofantibioticsbeforeadentalprocedure,toreducethetransitorybacteremiaandtheriskofdevelopingIEinriskpatients.ThisprojectproposedanresearchfortheTermPaperfortheUndergraduateDegreeinDentistrybyUniversidadFederaldeSantaCatarina.InordertoinvestigatethecontroversiesofdentistryabouttheIE,asurveywasconductedwiththeChartsandServiceSystemandtheCostCalculationSystemdatabase,analyzingthenumberofinternedpatientsforIEtreatmentintheHospitalProfessorDr.PolydoroErnanideSa~oThiago–UniversityHospitalofUniversidadFederaldeSantaCatarina,between2010and2014,asthehospitalcosts,includingdaily,diagnosticexamsandmedicine.Posteriorly,thecostsoftherecommendeddrugsbyAmericanHeartAssociationfortheantimicrobialprophylaxisfordentaltreatment,usingtheANVISAwebsite,updatedonApril2015,whereitwaspossibletodeterminatethediscrepanciesbetweenthecostsofanantimicrobialprophylaxisforthepatientorfortheBrazilianSistemaUˊnicodeSauˊde,andthehospitalcostsoverall,foreachpatient,forthetreatmentofIE.Thisstudyshowedthattheantimicrobialprophylaxiscoststheequivalentof0,095.564,20 by patient for de treatment of the IE at the studied Institution. The results of this study concludes that antimicrobial prophylaxis must be adopted for risk patients, previous to dental treatment as the American Heart Association recommendations, to minimize the morbidity and mortality of this group, as to reduce the public costs for the treatment of this pathology
