Transitions into a care home.

Abstract

There is no doubt that the move from one’s\ud own home to a care home is a major, and often\ud final, life transition. For many it may signify a\ud decline in physical and mental heath, a reduction\ud in, or even the end of, independence, or be the\ud corollary of the death of a partner.As such, the\ud move can have negative connotations but, with\ud appropriate planning and support, the transition\ud can be managed so that it produces benefits,\ud including improved quality of life.The extent to\ud which this happens in practice, and the ways in\ud which the transition can be facilitated, will be\ud examined in this chapter
