The detection of nonpalpable breast lesions using as a guide the calcifications.

Abstract

OUR MATERIAL CONSISTS OF 162 FEMALE BREAST BIOPSIES WITH SOLE PREOPERATIVE FINDING THE CALCIFICATIONS ON THE MAMMOGRAPHY.THE CALCIFICATIONS ON THE BREAST SPECIMEN RADIOGRAPHY ARE CORRELATED WITH HISTOLOGICAL HYPERPLASTIC BREAST LESIONS.WE HAVE THE FOLLOWING RESULTS: (A). THE CALCIFICATIONS INDICATED CARCINOMA IN 33 CASES,CYSTIC DISEASE AND OTHER BENIGN LESIONS ON 129 CASES,AMONG WHICH THE 33 WERE CLASSIFIED IN THE HIGH RISK GROUP (H.R.G.). (B). DURING THE FROZEN-SECTION DIAGNOSIS WE HAD NO FALSE-POSITIVE RESULTS.FALSE-NEGATIVE RESULTS WERE NOT FOUND IN CASES CONCERNING BENIGΝ LESIONS AND THE PERCENTAGE OF FALSE-NEGATIVE DIAGNOSIS IN CARCINOMA WAS EXTREMELY LOW. (C). THE SIZE,THE SHAPE, THE NUMBER AND THE DISTRIBUTION OF THE CALCIFICATIONS ON THE SPECIMEN RADIOGRAPHY DURING THE GROSEN-SECTION DIAGNOSIS CAN INDICATE THE RPESENCE OF A CARCINOMA BUT NOT PUT A SURE DIAGNOSIIS. (D). THE CALCIFICATIONS ARE ALMOSTΕ EXCLUSIVE LOCATED WITHIN HYPERPLASTIC BREAST LESIONS AND MOST FREQUENTLY ΙN ADENOSIS, APOCRINE METAPLASIA,DUSTUL HYERPLASIA AND DUCTAL CARCINOMA OF COMEDO,CRIBRIFORM,MUCINONS,TUBULAR,PAPILLARY AND APOCTINE TYPE. (E). THE HISTOLOGICAL APPEARANCE OF CALCIFICATIONS TENDS TO BE SEEN MORE FREQUENTLY IN DYCTAL CARCINOMAS WITH A GOOD PROGNOSIS (MICROSCOPIC SIZE,IN SITU,WITHOUT METASTASES,OF LOW GRADE MALIGNANCY HISTOLOGICAL TYPE) AND ALSOIN A CONSIDERABLE NUMBER OF LOBULAR CARCINOMAS.THE INCIDENCE IS ALSO HIGH IN HYPERPLASIAS WHICH DEMAND HISTOLOGICALLY THE CLASSIFICATION OF THE PATIENT IN THE HIGH RISK GROUP.
