THE EFFECT OF PYRAZINAMIDE AND PROBENECID ON URATE TRANSPORT AND THE PERMEABILITY OF PERITONEAL MEMBRANE DURING CONTINUOUS AMBULATORY PERI- TONEAL DIALYSIS (CAPD)

Abstract

DRUGS AND HORMONS HAVE BEEN ADMINISTERED ,MOSTLY INTRAPERITONEALLY , TO STUDYTHE PERITONEAL PHYSIOLOGY AND EXPLORE POSSIBLE WAYS TO OVERCOME MEMBRANE'S OR TECHNIQUE'S FAILURE. IN THE PRESENT STUDY WE USED PYRAZINAMIDE (PZA) AND PROBENECID (PB) , TWO WELL- KNOWN INHIBITORS OF REAL URATE SECRETION AND REABSORPTION RESPECTIVELY , TO EVALUATE THEIR IMPACT ON URATE TRANSPORT THROUGH PERITONEAL MEMBRANE . IN 20 STABLE , NON OBESE CAPD PATIENTS , ON THE STUDY DAY , FOLLOWING AN OVERNIGHT EXCHANGE , A 4-HOUR,2L,1,3 6% GLUCOSE EXCHANGE WAS PERFORMED (CONTROL EXCHANGE), PZA 3GR , WAS GIVEN ORALLY AND AN OTHER IDENTICAL EXCHANGE WAS PERFORMED (STUDY EXCHANGE). THE SAME PROTOCOL WAS REPEATED , AT LEAST 15 DAYS LATER WITH 2GR PB. KT/V , PERITONEAL CLEARANCES OF UREA , CREATININE , URATE FOR EACH EXCHANGE , MASS TRANSFER AREA COEFFICIENTS (MTAC) FOR THE THREE SOLUTES AND THEIR DIALYSATE TO PLASMA CONCENTRATION RATIOS (D/P) WERE USED TO ESTIMATE PERITONEAL TRANSPORT. ADMINISTRATION OF PZA RESULTED IN DECREASED CLEARNESS AND MTAC VALUES OF THE THREE SOLUTES. HOWEVER D/P RATIO DECREASED SIGNIFICANTLY ONLY FOR URATE , INDICATING A MORE INTENSE INFLUENCE OF PZA ON URATE. AFTER ADMINISTRATION OF PB , WE OBSERVED INCREASED CLEARANCES OF UREA , CREATININE AND URATE . HOWEVER MTAC AN D D/P RATIO INCREASED SIGNIFICANTLY ONLY FOR URATE (P<0,05). PB APPEARED THUS TO HAVE AN ACTION SIMILAR TO THAT IT EXERTS TO THE RENAL TUBULE. PERITONEAL CLEARNCES DID NOT CORRELLATE WITH ANY OTHER OF THE PERMEABILITY INDICES , WHILE MTAC FOR ALL THE THRE E SOLUTES WERE POSITIVELY CORRELLATED TO THEIR D/P RAT
