EFFECTS OF METHYLPREDNISOLONE AND DEXAMETHASONE ON HUMAN'S POLYMORPHONUCLEAR LEUKOCYTE ADHERENCE IN VIVO.

Abstract

HIGH CONCENTRATIONS OF STEROIDS, HYDROCORTISONE OR METHYLPREDNISOLONE, IN VITRO INHIBIT THE ADHERENCE OF THE POLYMORPHONUCLEAR LEUKOCYTES (PMNLS), THAT HAVEBEEN ACTIVATED THROUGH THE COMPLEMENT. DEXAMETHASONE DID NOT INHIBIT THE ABOVE MENTIONED ADHERENCE. THE ACTIVITY OF METHYLPREDNISOLONE AND DEXAMETHASONE WAS STUDIED IN VIVO IN THE PRESENT STUDY. BOTH DRUGS WERE GIVEN IN HIGH EQUIVALENT DOSES OF 30 MG/KGR FOR MP AND 6 MG/KGR FOR DEX BOLUS IN VIVO IN TWO GROUPS OF PATIENTS RESPECTIVELY. BLOOD SERUM FROM THE TWO GROUPS OF PATIENTS WAS TAKEN BEFORE AND ON 4, 8, 12 AND 16 HOURS AFTER THE BOLUS ADMINISTRATION OF THE DRUGS. ADHERENCE OF PLMNS TO NYLON WAS ASSAYED BY THE METHOD OF MAC GREGOR. WHENTHE PMNLS WERE SUSPENDED IN SERA FROM PATIENTS OBTAINED AT 4 AND 8 H AFTER THE ADMINISTRATION OF MP, PMN ADHERENCE WAS MARKEDLY INHIBITED. NO INHIBITION OF PMN ADHERENCE WAS NOTED WHEN THE PMNLS WERE SUSPENDED IN SERA OBTAINED AFTER THE ADMINISTRATION OF DEX. GIVEN THE FACT THAT PMN ADHERENCE TO NYLON FIBERSIS A PROCESS SIMILAR TO THE PMN ADHERENCE TO THE ENDOTHELIAL CELLS DURING THEEARLY PHASE OF MIGRATION INTO INFLAMMATORY FOCI, ONE COULD DEDUCE THAT MP IS MORE EFFECTIVE THAN DEX IN THE TREATMENT OF SEPTIC SHOCK.
