STUDY OF OSTEOGENESIS IN LONG BONES LENGTHENING WITH METHOD OF ILIZAROV: COMPARING WITH PLAIN X-RAYS, ULTRASOUND, QUANTITATIVE AND THREE DIMENSIONAL COMPUTED TOMOGRAPHY - (QCT, 3D - CT)

Abstract

IN 20 PATIENTS WHO UNDERWENT BONE LENGTHENING, BONE DENSITY OF NEWLY FORMED BONE WAS MEASURED WITH QCT SLICES AND THREE DIMENSIONAL RECONSTRUCTION OF IMAGES (3D - CT) WAS PERFORMED AT THE END OF LENGTHENING AND MATURATION PHASE. THE DENSITY OF EVERY SLICE WAS MEASURED WITH RESPECT TO ITS DISTANCE FROM THE CORTICOTOMY BORDER. THUS, WITH COMPARISON OF DENSITIES OF SAME SLICES AT THE END OF BOTH PHASES, THE RHYTHM OF NEW BONE FORMATION WAS STUDIED IN REFERENCE TO TIMEAND SPACE. IN AN EXPERIMENT PERFORMED ON 50 CADAVER TIBIAS, BONE DENSITIES OF QCT SLICES WERE MEASURED THROUGOUT THE WHOLE LENGTH OF TIBIAE. MEAN AND MINIMAL BONE DENSITY VALUES WERE FOUND AND COMPARED TO THOSE OBSERVED IN OUR PATIENTS. IN ANOTHER EXPERIMENT ON 30 CADAVER TIBIAS, BONE DENSITY OF MARKED SLICES WERE MEASURED WITH AND WITHOUT THE ILIZAROV APPARATUS. ARTIFACTS PRODUCEDBY THE PRESENCE OF THE APPARATUS WERE IDENTIFIED IN VARIOUS DISTANCES FROM THE APPARATUS RINGS. IN A THIRD EXPERIMENT WE MEASURED THE IRRADIATION ABSORPTION OF VARIOUS POINTS ON A PLEXYGLASS PHANTOM. OUR RESULTS SHOWED A SMALL DOSE OF IRRADIATION ABSORBED BY THE GONADS, STATISTICALLY INSIGNIFICANT ARTIFACTS FOR SLICES TAKEN>1CM AWAY FROM THE APPARATUS RINGS AND AN EXACT DETERMINATION OF BONE MATURATION. WITH 3D - CT WE OBTAINED AN EXCELLENT ESTIMATION OF THE DISTRIBUTION OF NEW BONE IN SPACE, WHICH LED US TO A SAFER DECISION ON THE APPROPRIATE TIME FOR APPARATUS REMOVAL.
