NEOCYTES TRANSFUSION IN PATIENTS WITH THALASSEMIA

Abstract

NEOCYTE (YOUNG RED CELL) TRANSFUSION IS A NEW APPROACH TO THE TREATMENT OF THALASSEMIA MAJOR. THE AIM OF THIS STUDY WAS TO EVALUATE: 1) THE EFFECTIVENESS OFERYTHROCYTES SEPARATION TO NEOCYTES AND GEROCYTES USING A SPECIFIC TECHNIQUE, THE NEOCEL SYSTEM. TO INVESTIGATE THE EFFECTIVE SEPARATION THE FOLLOWING PARAMETERS WERE COMPARATIVELY STUDIED IN NEOCYTES AND GEROCYTES PREPARATIONS: A) THE PHTHALATE ESTER DENSITY GRADIENT CURVES, B) THE MEAN CORPUSCULAR VOLUME, C) THE PYRUVATE KINASE ACTIVITY. 2) THE CLINICAL EFFECTIVENESS OF NEOCYTES TRANSFUSIONS IN PATIENTS WITH B-THALASSEMIA. TO THIS END DAILY HEMOGLOBIN DROP ANDDAILY BLOOD CONSUMPTION WERE EVALUATED IN 18 PATIENTS (AGED 9-24,5 YRS) AND COMPARED DURING TWO PERIODS. IN THE FIRST PERIOD EACH PATIENTS RECEIVED 10 TRANSFUSIONS WITH FRESH BLOOD, AND IN THE SECOND PERIOD 10 TRANSFUSIONS WITH NEOCYTES. ANALYSIS OF THE RESULTS SHOWED: 1) PROVIDED ALL SEPARATION PROCEDURES ARE DONE METICULOUSLY THE SEPARATION OF ERYTHROCYTES TO NEOCYTES AND GEROCYTES IS FEASIBLE, USING THE NEOCEL SYSTEM. 2) THE RATE OF HB FALL OVER THE PERIOD OFNEOCYTES TRANSFUSIONS DECREASED IN 15 OUT OF 18 PATIENTS (MEAN DECREASE VALUE19,48%. 3) THE DAILY REQUIREMENTS IN BLOOD VOLUME PER KGR BW, DECREASED IN ALL PATIENTS AFTER NEOCYTES TRANSFUSIONS COMPARED TO FRESH BLOOD ONES (MEAN DECREASE VALUE 20,18%).
