INTERNAL CARDIOVERSION OF CHRONIC ATRIAL FIBRILLATION. IMMEDIATE AND LONG-TERM RESULTS

Abstract

THE AIM OF THE STUDY WAS TO EXAMINE THE INITIAL AND LONG-TERM RESULTS OF INTERNAL VS EXTERNAL CARDIOVERSION REGARDING THE CONVERSION OF CHRONIC ATRIAL FIBRILLATION AND MAINTENANCE OF SINUS RHYTHM. METHODS: FORTY PATIENTS (MEAN AGE 57 +- 8 YEARS) WITH CHRONIC ATRIAL FIBRILLATION (MEAN TIME OF DURATION 12 +- 20 MONTHS) WERE INCLUDED IN THE STUDY. THE PATIENTS WERE INITIALLY RANDOMIZED TO INTERNAL OR EXTERNAL CARDIOVERSION BUT IF EXTERNAL FAILED INTERNAL WAS TRIED. INTERNAL CARDIOVERSION WAS PERFORMED WITH THE FOURTH POLE OF A QUADRIPOLAR ELECTRODE AS CATHODE AND A BACKPLATE ELECTRODE AS ANODE. MAXIMUM ENERGY WAS 400 JOULES FOR EXTERNAL AND 200 JOULES FOR INTERNAL. INTERNAL OR EXTERNAL CARDIOVERTED PATIENTS WERE NOT STATISTICALLY DIFFERENT REGARDS AGE, HEART DIMENSIONS ORCAF DURATION. RESULTS: EIGHTEEN PATIENTS WERE CARDIOVERTED WITH INTERNAL (GROUP I), 9 WITH EXTERNAL ALONE (GROUP II) WHILE 13 PATIENTS EXTERNAL FAILED ANDINTERNAL WAS TRIED (GROUP III). THE INITIAL SUCCESS WAS 88% FOR GROUP I (16 TO 18) AND 40% FOR THE TOTALLY EXTERNALLY CARDIOVERTED (9 FROM 22) P < 0.01. GROUP III SUCCESS WAS 61%. THE LONG-TERM FOLLOW-UP REGARDING CAF RECURRENCE AT 34 MONTHS (STATISTICAL ANALYSIS KAPLAN-MEIER) SHOWED NO DIFFERENCE BETWEEN GROUP I AND II BUT SIGNIFICANT DIFFERENCES BETWEEN GROUP III AND BOTH GROUPS I AND II P < 0,002 (SIX PATIENTS FROM GROUP I, 6 FROM GROUP II AND 1 FROM GROUP III REMAINED IN SINUS RHYTHM. IN CONCLUSION : INTERNAL CARDIOVERSION IS SAFE ANDMORE EFFECTIVE REGARDING IMMEDIATE SUCCESS. (ABSTRACT TRUNCATED)
